The different shift nurses aren't kept within the loop. Mom feel and went to hospital, was discharged and I was surprised the shifts didn't know about her fall. Also mom was acting out of character, I had to tell the NO and she also didn't know about it (she was on vacation) until a week later and then was put on an antibiotic (no urine collected) but could have had a UTI which is why she was acting out. She is much better. If I didn't go see my mom 2-4 times a week. I wouldn't know. I still rate the facility a 5 because the caregiving staff and director is kin and works with me when I do have an issue. Mom likes the place
