A beautiful facility with spacious rooms and nice amenities. My experience with Hollybrook is probably not typical, but during a very stressful 3 weeks, the staff stepped up and helped my family through a very difficult time. Shortly after moving into the assisted living, my dad was moved to memory care with hospice and died shortly thereafter. We are very grateful for everyone’s help during that time. There are two lovely, fully furnished guest suites that were made available to us. That said, weekend and holiday staffing is weak, but I always expected to be be hands-on and in the facility everyday. That is my only complaint. As absorbed as we were in our own little family drama, the other residents always seemed engaged and happy, well treated and well fed.

I have lived here for about a year and a half. I have enjoyed it immensely. Very friendly people live and work here. I recommend it for people 65 years old and older. We get three meals a day plus snacks. We get activities that are great. We have exercise programs for 45 minutes at least 2 times a week. There is also bingo and occasionally we will get entertainment from the community. That is great and very enjoyable. We have sing a longs and bell choir. During the holidays, the staff is wonderful in surprising us with different kinds of activities that we don't do on a regular basis. We also have someone come in to do chair yoga with us. To me, it is a great place to live. The staff is very helpful.

Everything is wonderful and clean at this community. The staff are very nice and attentive to my aunt and especially in her time of need after losing her dog. My aunt is adjusting well and has gained 10 pounds since staying with the community. My aunt loves the meals provided, she reads me the menu everyday! Overall it is a good place for my aunt and I would most certainly recommend this community.

Assisted living communities provide apartment style housing and social interaction as well as individualized care. The types of service offered often involve assistance with ADLs. Occupants may be kept under supervision and assessment. In certain instances, communities may offer skilled nursing, dementia care, or hospice. The Illinois Department of Public Health supervises this community type. Staff members receive education in a number of areas. These include: abuse, hygiene, choice, and resident rights. This type of community is required to offer a number of services. These include assistance with activities of daily living, laundry, medication administration, and emergency response systems. This community type has a number of provisions pertaining to medication assistance. These provisions include documenting whether or not the resident took the medication, supervision of self-administered medications, and only a licensed health care professionall employed by the community may administer medications. This community has a number of provisions concerning food and diet which include communities must provide three daily meals. Before a resident is accepted, they must go through a service plan screening to assure their needs are going to be met. This screening includes a number of areas of interest such as assessment must be updated by a physician annually or upon a significant change in condition, evaluation of a prospective resident’s psychosocial condition, evaluation of a prospective resident’s cognitive condition, and provider must ensure that the resident and the resident’s representative, if any, are informed of the potential consequences of activities that the provider considers to pose risks to health and/or safety. The minimum number of residents for this community type is 3. The maximum occupancy is 1 residents per room. There must be one toilet for every 1 residents and one bath or shower for every residents. This type of community is required to have a staff member on duty 24 hours a day. Public assistance is available through A Medicaid 1915(c) waiver called the Supported Living Program and a 1915(b) waiver called the Managed Long-Term Services and Supports program.
