I recommend Cantata because they have a robust rehab gym staffed with professionals in physical therapy, occupational therapy, balance, speech/memory therapy. It is located on a quiet tree lined landscaped campus with onsite Assisted Living, Skilled Nursing and Independent Living. The staff communicates very well with the resident and the family. Price is fair market/moderate.

They're short of help, all the way around. That's the main challenge. Sometimes it's hard to find a nurse on the nursing floor. But I really like the assisted living building. They're very good with their activities and keeping their people up to date on things. I had one relative move to the wellness center from assisted living, and I would not do that if you can avoid it. They don't have the activities, and they're kind of isolated in wellness.

There must be one bath or shower for every residents and one toilet for every 1 residents. In total, there can be no more than 1 residents to a room. The minimum number of residents for this community type is 3. This type of community is required to have a staff member on duty 24 hours a day. Public assistance is available through A Medicaid 1915(c) waiver called the Supported Living Program and a 1915(b) waiver called the Managed Long-Term Services and Supports program. Staff must receive education in a variety of areas. These include: self-determination, privacy, abuse, and independence. This community type has a number of provisions pertaining to medication assistance. These provisions include documenting whether or not the resident took the medication, only a licensed health care professionall employed by the community may administer medications, and medication reminders. Before a resident is accepted, they must go through a service plan screening to guarantee their needs are going to be met. This screening includes a number of areas of interest such as A comprehensive assessment, provider must ensure that the resident and the resident’s representative, if any, are informed of the potential consequences of activities that the provider considers to pose risks to health and/or safety, assessment must be updated by a physician annually or upon a significant change in condition, and evaluation of a prospective resident’s physical condition. This community has a number of provisions concerning food and diet which include communities must provide three daily meals. This type of community is required to offer a number of services. These include non-medical services, medication reminders, supervision of self-administered medications, and emergency response systems.
