1. 4 - Unfortunately, when my mom was being admitted another management company was on its way out and nothing was said when we were admitted. Only found out the next week that a transition was about to happen. With transitions are also staff turnovers. This is hard on individuals with dementia. 2. 4 - Cleanliness. I have had to ask that my mom's room be cleaned a couple of times. Once because some cleaner was used on the bathroom floor that made the floor very slippery. The second time was when moving a fall mat next to her bed, the floor under the mat was soaked in urine. 3. Friendliness - 4/5 the staff has been very friendly towards my mom. It would be helpful if they would always wear a name tag that is visible to residents and family. 4. 3/4 Training would be beneficial for the staff to correctly respond and talk with dementia patients to encourage participation in activities and to encourage self care whenever possible. Easy to get busy and just "do" for the individual rather than give simple verbal cues to get resident to do for themselves. They have been very caring though. 5. Meals - 5 Haven't seen many meals, but what I have witnessed they have been sufficient and with variety. 6. Activities -5 The activity director has been very good at providing a variety or activities to engage the residents. Very well rounded, devotional, reading stories, karaoke, group games, art - not just all color pages, spa nail treatments to name a few that I have witnessed. 7. Value for cost - 5 Compared to other places, the memory care side was all inclusive at the time when we signed up. I believe care costs have been added since, but my mother came in when it was included in her monthly rent. Other places have LVN's around the clock, but not so at Legacy. You get what you pay for. As a side note, because it is a new management company, most of the administrative staff are interim. Most of the individuals I have dealt with from the administration have been extremely helpful and supportive. Staff is kind. When questioned specifically, they are communicative. I have been told that they used text or speak directly to family, but the aides have been asked to allow one of the daytime nurses to interact with the family instead. This particular nurse hasn't been responsive with me so much. I go around her because of that and communicate with the Interim Nursing Director. She is kind and communicative.... Read more

Local Staff tries, but management, especially corporate representatives don't care and are responsible for most issues by lack of involvement and supervision.

The Activities Director [name removed] is "above and beyond" All of the staff are kind and helpful in our experience. The facility is very updated and nice. They offer plenty of fun and enriching activities for the residents! Our family member is in a double room in memory care (hasn't always had a roommate, but does now) and its quite large.

Since my father has been at Sodalis there has been a huge amount of staff turnover. I think the constantly changing faces has made it difficult for him to feel at home. There do not seem to be any activities or social events for residents and there seem to be many inconsistencies with things such as meal time and room cleaning. We are hopeful things will improve with a new executive director.

We like Sodalis Oak Hill a lot. The recent turnover is a bit concerning but Mom seems very happy there and we feel very comfortable having her there. I definitely would recommend this community to other families. It's a lovely facility, the best of what we looked at.

It's a lovely facility but it is very understaffed. The staff that is there is wonderful, but overwhelmed. There has been turn over in the management which has lead to a general confusion amongst the staff about what benefits residents have enrolled in. There is no communication from the facility to the families about any events. In fact, no emails at all from the facility about billing, coordinating care, or anything at all. Until they are better staffed, I would not recommend this facility to other families.

High turnover in staff, lack of communication as a result. Activities Director is a superstar. Without her, my ratings would be lower.

There is not enough staff, even for half full. On a good day, there might be three aides and a med tech at night. Recently, they had to call in two people who were scheduled to be off because all five of the people scheduled to work called in sick. Just to have staff there over night. The door greeters are friendly and the three kitchen staff also asks how you're doing. The 2nd Director is trying, but overwhelmed. The chef makes tasty meals. My mom doesn't eat them, but I sampled a few and they were very good. The Activities Director is the best. She tries hard to get clients to participate in activities. She does a lot of things. At a cost of several thousand dollars a month, I am getting robbed because of lack of people to care for my mom. There is not enough staff to run this facility.

My loved one is currently a resident at Sodalis Oak Hill and most everything has been very good for him at this facility so far. Sodalis has recently acquired this facility, which had declined in residency. Building the facility back up has presented some challenges, but the staff are working hard on it and they are very attentive to the residents. I would recommend Sodalis Oak Hill to others!

We were kind of in a rush to find a place to be honest. They had an opening and the monthly payment was something we could afford without sacrificing too much. Plus the incentive ($2k off October rent) was very nice! It turned out that it was a really nice place!! I would recommend to others.

Based on the experience we have had so far with Sodalis Oak Hill, we would recommend them to other families. However we would like to see more scheduled activities or the residents being able to participate in.

Response from Sodalis Oak Hill
September 21, 2023Hello Lori, We appreciate your positive feedback about your experience at The Crossing at Oak Hill Assisted Living and Memory Care. We take pride in serving our residents and their families, and if you have any further questions, please let us know.
If I could give this facility negative stars, I would. Both of beloved parents live there. It used to be called [community name removed] and in July it switched over to Spectrum. It was bad already, but under Spectrum it has gotten even worse. They are TERRIBLY understaffed. A caregiver approached me on a recent weekend and said, "This place is not safe. You should move your parents somewhere else." The food is not good. You cannot get anyone on the phone. Extremely lengthy response times to hitting call buttons. Activities are limited unless you like Bingo. They WILL NOT call you if there is a problem (parents getting Covid no call...parents falling down multiple times...no call). My dad fell down twice after NO RESPONSE from hitting the call buttons he just got up to do for themselves. We use [company name removed] for occupational therapy and they have said that the place is not good. Look elsewhere.

I have mixed emotions. They are very short-staffed, although that's true of a lot of places. I think they're doing the best they can. But I get disappointed often. Laundry often goes missing. I haven't found a comparable place for the price though. There was a recent change in ownership, and they said they there would be no increases for 90 days. We'll see what happens after that. But ultimately, the people are good to my loved one. They're kind, and they care about their residents. They encourage them, and they prompt them to attend activities. And I feel my loved one is in the best situation that's available to us.
Overall have been very happy with The Crossing at Oak Hill. When we first went in, they did an ownership transfer. We feel this could have been communicated slightly better upon us entering this facility. But overall they worked us through the process. The meals are fantastic and the facility is very clean and caring.

A Place for Mom has scored Sodalis Oak Hill 8.4 out of 10 using our proprietary review score.
We assign review scores to give a more reliable view into senior living communities and home care agencies. Our review scores prioritize reviews that are recent — the past 24 months — because we know families need current information when choosing senior care.
Those with many recent, positive reviews receive a high review score, while providers with few recent reviews — regardless of how positive — receive a lower review score. Communities with no recent reviews will not have a review score, even if older reviews are positive. The maximum A Place for Mom review score a community can receive is 10 points.
