Mali and her team of caregivers really care for their clients and their loved ones. Mali brought my parent to the hospital and waited for me to meet her there before leaving the ER to return back to her staff and other residents. This is not what I experienced from another PCH in the past. Her team cares not just for your loved one but for you as well. She and her team have saved me and my child's sanity. Very gracious and kind. The facility and residents are kept clean at all times. They feed the residents well, monitor their medications and even help keep your loved one manicured and entertained. These include: 24 hours of continuing education during the first year and 16 hours annually thereafter, general infection control principles, assisting residents in transferring and ambulation, and emergency preparedness. This community has a number of provisions concerning food and diet which include at least three meals, one nutritious snack, and any therapeutic diets ordered by a resident’s health care provider must be provided each day. Before a resident is accepted, they must go through a service plan screening to ensure their needs are going to be met. This screening includes a number of areas of interest such as assessment to determine the resident’s medical needs, A written care plan must document the assessment findings, assessment to determine the resident’s physical care needs, and care plan must be updated at least annually and when there is a change in the resident’s needs. This community type has a number of provisions pertaining to medication assistance. These provisions include assisting residents with storage of medications, assisting residents with self-administration, and cognitive and functionally able residents may self-administer medications . Public assistance is available through 1915(c) Medicaid Waiver programs- ) the Community Care Services Program is managed by the Department of Community Health’s Division of Medical Assistance Plans and partners with the Division of Aging Services and the Service Options Using Resources in a Community Environment (SOURCE) program.
