This place is terrible. They have mold issues. My ins room is filthy. They do not even clean feces from the elderly here. On recent visits, I’ve witnessed aids yelling at dementia patients for 15 minutes. They do not assist patients with hand washing prior to eating. I’ve walked in and seen no nurse nor aid around on many occasions. They don’t even provide toilet paper for the weekend for patients. If you don’t care then this is the place to be.

Did not change his driveline dressing for this LVAD patient for the 2 weeks there; he/patient has to continuously ask for his meds. The worst [removed] is the 3rd shift. The OT/PT was great only. They (3rd shift) ignored him the first night there. The RN on duty was mouthy. They had 2 desk nurses.

We moved my grandmother there over a year ago. The facility is well kept and clean. The nurses for the most part have been great. When [name removed] is on duty or [name removed], we feel at ease because they are so friendly and good. Others don't give us peace of mind just because we don't know them as well. At times, we have to ask for cleaning of the room, toilet paper and at times we have had to take it or do it ourselves. The front desk is not always friendly or helpful. Some of the things promised at the beginning arent the way it was proposed. I would give is a "C" rating.

The facility is okay. However, management should do a better job with staffing. Taking care of the elderly, and children is not an easy job. You should be more thorough in the staff you hire. That goes from the highest to the lowest.

Every employee I have encountered has been professional and caring. They have addressed any issue we've has. Their responsiveness to Covid-19 was early and effective. I've been impressed with their commitment and ability to keep their residents safe and healthy. We appreciate them very much!

OT/PT are okay. But I don't feel microwaving food counts as "cooking"... however, the medical staff told my husband his sugar (diabetic) was 800 yes 800, and often it's 350+. If it was 800 he'd be dead! So i brought all his needs and he is doing his own on the sly. Scary food not cooked on the weekend!! Also, they will not share his OT/PT med reports with me. PCP just wrong!!! The excuse is different network... B.S.!!!

The management took care of any issues I encountered. When I entered Hyde Park Health Center I was instructed on what I would expect and what to do if I did not receive the care I felt I should get. The therapy department was so good. They helped me regain my independence.

I think you guys try hard but you don't get to talk to the same person more than once (big turnover in help?). Nobody returns phone calls.

Approximately 7-8 years ago this facility was very professional, great healthcare workers, and phenomenal staff. Over the past few years this facility has lost a lot of its charisma. The aides aren't friends and very unprofessional and immature. The overseers don't handle situations appropriately or at all. I have had to replace several items that were either damaged or stolen. Overall I am very dissatisfied with this facility.

I have a family member in this facility and I can't express enough how great her experience and my families experience has been. The nurses, her caregivers and even the management has been extremely friendly, helpful and attentive to her every need. The facility is always very clean and the food is as good as it gets. Please keep up the great work and doing what you all do for families.

This review is coming from someone who has worked in two different nursing homes and medical settings...please do not leave your loved one in this facility. It's seems poorly ran and lacking communication. We encountered 2 nurses and one nurse aide. The aide was friendly and helpful. The nurse on the floor acted as if we were bothering her by asking for information reguarding our loved one. The nurse supervisor was condescending when speaking to us about the care plan for our loved one. When she realized she couldn't pull a fast one on us she because agitated and aggressive with her words. Seems to me scare tactics are used in this facility by staff members to manipulate the situation. I don't think the nurse expected the response she got from us. The facility is deplorable and needs attention. The rooms are small, dirty(dust on window sill in rooms and well as on curtain panels) halls smell of urine, rust around sink in room, panel coming off near sink, light above sink did not work, stains on bed cover(this cover is used as the main cover, not just as bedspread), there was a recliner in the room and who knows where it came from or if it had been cleaned and if your loved one is addmitred on the weekend there doesn't seem to be sufficient supervision of staff to set up incoming resident. A room phone was not given to resident until the next morning after being there from 5pm the day prior. They were of sound mind and alertness so a phone was not given because of a medical condition. One was asked for but always forgotten. There was also a food menu of options that our loved one couldn't have but was in the room on the bedside table. The nurse said she didn't know much about our loved one and would print us off what she had...this is where it all went south. Seriously save yourselves the aggravation and nightmare and get pre-cert recommendations for other facilities.

The care is great! The food has a lot to be desired. My mother is a resident there and is a very picky eater. Many times she will not eat her meals. My sister has been to visit her during many meal times (especially dinner) and she says the dinners are horrible and always cold. So this is the biggest issue. Also, the floor my Mother is on is a little dark and depressing. But, as I mentioned the care is great, and my Mother seems to be very happy there so that is the most important thing.
