1/31/15 - Met with Kristy Barton and toured the facility. Kristy was extremely knowledgeable and helpful. Liked the fact that they offer different levels of care and group patients with similar issues within the same area. Also liked the fact that a geriatric nurse practitioner is on staff to monitor vital signs, check for UTI's, and symptoms that mimic dementia. Seemed to be a strong focus on mediation monitoring to reduce issues with drug sensitivities or ineffectiveness. The 1 BR is adequate size - cottages have open concept/common living spaces, kitchen and dining areas.

Overall I think they do a great job at Foxbridge Assisted Living. The staff are friendly and professional, and quick to respond to any questions or requests that you have for them. The facility itself is clean and well kept for the most part, they do a good job keeping things tidy.

I do like the facility but they did a wrong evaluation. We basically had to move in the month to another part of the building. I told them all that she would need but they sent someone to the house to evaluate her. And they decided she needed less care than what I had told them. She was not getting the care she needed. So we were told that she would need to be moved to another level of the building. I was a little disappointed in that ordeal. It does seem to be getting better.

January 30, 2015Thank you for taking time to submit your comments. I'm sorry you were disappointed that your loved one had to be moved to a higher level of care soon after moving to Foxbridge. Our Wellness Department conducts an initial assessment on potential residents prior to their move to Foxbridge, and makes every effort to place residents in the appropriate level of care upon move-in. An assessment is a fact finding tool used to learn more about a person's care needs. Unfortunately, it can be difficult to get a complete understanding of a person during the hour long assessment. Sometimes when a person is assessed in their own home or a very familiar environment, they may present themselves as being more able to care for their daily needs. When a resident moves to Foxbridge and is being observed 24 hours a day, it may become evident that they need a different level of care than originally believed. Our staff discusses needs with the family, and the resident is then moved to the cottage providing the appropriate level of assistance. We understand change can be difficult, and we make every effort to minimize the disruption. We're so happy to hear your loved one is doing well now. They are great! I am very grateful for Foxbridge Assisted Living & Memory Care. I love the staff and the young people they have there. It has been a blessing, and i would highly recommend them. The only small reservation I have about the entire process is this: The tracking of possessions is less than wonderful. We lost a wheelchair somewhere in the deal. I am not sure how you fix that issue in a place where people are often wonder around. I wish there was a way to keep track of possessions so that didn't happen. Other than that it was a wonderful experience and I would highly recommend Foxbridge Assisted Living & Memory Care!

Overall I think they've done well at Foxbridge Assisted Living, and are doing a good job taking care of my loved one. With one exception the staff have all been wonderful to work with. You couldn't ask for a nicer bunch of caregivers, and the gentleman who runs things at night is very nice. We have had a few issues with the "house manager", her lack of attention to detail doesn't inspire a lot of confidence. Another thing I'd like to see them do a bit better would be to have a larger variety of activities. But overall we're pleased with this place, and would recommend it to others.

This is actually premature, as we have not moved our mother into Foxbridge yet. She had a minor stroke a week and a half ago, spent a week in the hospital, and is now at King's Daughters and Son's rehab facility. The marks I gave above for Foxbridge, is only based on mine, and my brothers, visits there so far. But it is still in our long term plan, to use Foxbridge.

We toured this community when we were looking into care options for our loved one. We didn't really like the look of this one, it would not have been a good fit for our loved one. It was a larger community, and getting to activities or meals would require walking between different buildings, which we thought would be hard on someone with memory issues.

I'm so sorry there was a misunderstanding during your tour, but I appreciate the opportunity to clarify! Each of our 5 one level buildings has its own dining room, living room and activities area, and residents don't have to leave their buildings unless they'd like to go for a walk around our 8 acre property or visit with other residents across the community. Our staff welcomes the opportunity to accompany our residents on leisurely strolls around the property and to our lake to fish or feed the geese! My loved one has been here a few months, and so far everything has been good overall. It's a nice looking facility, and the staff are all friendly and professional. I think they just generally do a good job taking care of all of their residents, and they've been able to provide all the care my loved one needs.

They are very clean and very friendly. They do a pretty good job with everything. The food is very good, I would give it a five. I really like Kristy Barton, Donna, and the leadership there. The leadership really sets them apart from other communities.

We toured this location when we were looking into care options for our loved one. This was not the location we ended up selecting, but I did like what I saw here. The grounds themselves were beautiful, it was located next to a lake, and it appeared to have a lot more potential for outdoor activities than the other communities I toured. I also like how the facility was arranged, with the residents being organized into separate "pods" of those with similar care needs. The drawback to this was that there didn't seem to be much interaction between the "pods" which limited the social options to just those with similar care levels. I only met the one staff member who gave me the tour, but she was very friendly and helpful.

All the staff is very friendly and take very good care of my father. The food is tasty and there is plenty for my father to eat. The grounds, however, need a bit of renovations and care. Potholes galore. I would recommend Foxbridge.

From what I remember, Foxbridge could have been an option for us as well. It was more on the assisted living side and it wasn't as new as some of the other places we were looking at so that's why we didn't choose them.

We are so happy with Foxbridge and the assisted living they provide for our parents. It's a beautiful place with paved walkways meandering through the campus and around a lake. You can tell that the staff is carefully chosen in that ALL have a positive & cheerful disposition and put the residents first. Our parents were so reluctant to leave their home, but Foxbridge staff has made that transition go very well for them. They are able to live independently and can request assistance as they need it.

For the most part Foxbridge is very nice. Although the residents with dementia have no independence and are put to bed by 8pm. Their level of care at times can be frustrating, but I have experienced worse care at another facility. Donna, the director, is new to the property and she is trying to make a difference. I think the meals could be better. Foxbridge has four buildings; the Oak building is for assisted living residence, the Pine building is for assisted living and the beginning stages of dementia, Spruce is for residents with later stages of dementia, and the Magnolia building is for residents in the poorest health. Most of my experience has been with the Spruce building. The staff is very friendly but they have to arrange resident's medication schedules around their own schedules. For example if a medicine is directed to be given on or after 8pm in the Spruce building the medicine is administered at 7pm when the nurses are readying the residents for bed. I do not agree with this method, but the nurses have to follow a schedule so they can be at the next building to administer medications. Foxbridge, like other facilities, is understaffed. Some of the residents in the Spruce building look bored, there are no activities for them to help slow the progression of dementia. Sometimes housekeeping can be inconsistent as well. But for the most part they take care of their residents and all the residents here seem happy. This place is much better than where mom was previously.
