Brookdale Oakwell

As you know my Wife was diagnosed, by the VA with dementia. I moved into assisted living to be with her Once I moved in it was determined that she passed all the dementia testing and no longer needed to be in Memory Care. Rather than pay for her to move in with me, we made the decision to relocate permanently to a 2 bedroom apartment at [community name removed] enabling us to utilize Long Term Care Insurance. Brookdale at Oakwell Farms is going through a transitional period following an ineffective Executive Director. The transitional administration is working hard to restore this facility to its previous state of the art facility

Send your memory care candidate's to Charters. We moved there. I would say go to charters living instead

The staff is very friendly and informative. Housekeeping is average, often requiring requests for certain services like vacuuming or mopping floor of the apartment. Communication with the family is good, but could be improved with residents that are mentally competent to keep them fully informed. Move-in process was good, however, during the first few weeks staff did not develop a care schedule that the resident could rely on for toileting, showering, dressing, etc. Staff needs to improve responsiveness to new residents' needs and wants. Food is great, good chef, responds to requests and takes time to get to know residents' likes and dislikes. Overall, I would recommend the facility.

They consistently leave dirty dishes in my in-laws room. My father-in-law has been hurt because he wasn't properly lifted. He has been left in soiled, diapers and bedding, which have resulted in an infection. They have called for help and not received it for long periods of time most recently I found out that it took about 40 minutes for somebody to come and check on them when they rang for assistance the carpet in the facility makes it difficult for my mother-in-law to get around and nobody comes to take her Outside or to dinner or to social events. every time that we go by to visit or drop off supplies, the dining area is always messy. There are always dishes from the previous meal left out.

The director and staff always return my calls and they are always aware of what is happening. Everything is good They accepted both skilled nursing and memory care residents. I would recommend it.

This place was perfect for my parents! It's conveniently located, clean and well maintained. The staff is very friendly, professional and willing to find answers to any question. Compared to other places we've seen or tried, this was the best.

Since our overall experience was not good, I would say to others looking to make sure and ask lots of questions about the day to day routine. Meet the doctor(s) taking care of your loved one. So, you can ask questions about care/medication to ensure medication is being given correctly. Based off of our experience, I would not personally recommend Brookdale Oakwell.

All staff have consistently been helpful and professional. Rare issues are handled quickly and Directors have always made themselves approachable and respond effectively. Regarding updates and community activities.

Met with a warm welcome by friendly staff. Provided an open discussion with questions and comments regarding my specific needs. Explanation of services were concise and thorough along with printed information. Property tour offered an idea of what a day in the life of a resident would entail.

Always a warm, inviting greeting. A smile with a cheerful attitude. Consistent care for all the residents.

I stayed at BRookdale for almost two months after a serious fall. I wanted two things to happen, physical healing and confidence in my ability to live alone. Brookdale made it possible for me to reach my goals

People and staff are nice and helpful, they made the move in and transition easy and trouble free

We picked this community, because it's smaller, and the location. They need to improve, on following-up on special services, and taking care room/laundry. The new staff could use more training. They have some fabulous people. I would recommend to other families.

When looking for care for your loved one, I suggest that you talk to other residents if you can. Overall the staff is very good with memory care patients here. They're extremely good about communicating to the family. I would like to see them be more observant to cleanliness of the rooms. Also, if a patient only eats a certain type of food, I would like to see them make sure that food is available to the patient.
