The staff truly was friendly. Unfortunately it didn't work for us, and we wanted it to. I'd encourage families to review the state website for any reports and/or complaints about any community they are considering.

We left after a year because the management had a problem. The care staff had massive turn over and new care staff often did not have experience and the training was not uniform. My mom ended up in the ER at least twice because she wasn't getting showers. They weren't always following the care plan. Nursing staff and care staff were often outside agencies who also need constant training by the client.

Bonaventure of Salmon creek is very clean. The staff are friendly, just don't do what they promise due to lack of staff. With the care services, there is minimal quality of care for those needing lots of assistance. Meals and dining ok,. The activities are N/A as our loved one did not have access. It started out being a pleasant experience until they were unable to give the assistance needed. It would be ok place for independent living, but not for those needing toileting and other assistance. They are not able to keep staff and staff at a level needed for the services they promise.

We were on the independent living side so the care services did not apply to us. We moved out several months ago. The staff changed to often and their was a lack of consistency which was a problem. Based on the experience we had with Bonaventure of Salmon Creek, we would not recommend this community to other families.

Staff and most of the residents are very friendly. Meals are mostly very good but we can order alternatives if we want. Activities are good. I have been here 3 months. At first it was quite an adjustment but am really happy! I still have my car and still volunteer every week at a food pantry in Oregon. Still independent and hope to be for a long time! Am happy I chose this place and appreciate all of your help in getting me here

Overall, I did not like getting run around to be able to speak to manager on site. Felt like I was being avoided. The cleaning was ok. [name removed] didn't let them in much. Staff was friendly enough. Quality of care is questionable- nurse (name removed) wasn't able to tell difference between anxiety and my mom not being able to breath because she had pneumonia and avian flu after one week there. I had her taken to hospital. She never came back obviously for those reasons. I would say meals were ok, never heard otherwise from her husband. No participation with the activities. Overall I am glad that someone could look after her husband but some other problems seen indicate that the site has other problems as well. Would not recommend the place

A nightmare from day 1. They were way understaffed and what staff they had was very inexperienced. Mother's care was so terrible we didn't leave leave her side during the day. We got her out of there as soon as we could

My mom is currently a resident at Bonaventure of Salmon Creek. Communication is spotty. The staff are nice. The food is great. I'm not yet sure if I would recommend this facility to others.

My loved one seems comfortable and cozy at Bonaventure with his kitty. He has a nice little balcony where he can enjoy beautiful sunsets. The residents and staff are very pleasant. Bonaventure provides lots of activities and outings for those who wish to participate. The location where my loved one is, is close to family and shopping, which is another bonus.
We've been through a little bit of a journey with my dad and getting him settled somewhere we truly felt we found the perfect place with (Prestige Bridgewood) on the day of preparing to move. We found out that they did not have a transition to memory care at that facility. feeling like I hit a wall. [name removed] was wonderful and started sending me information on places, One of the first places that open their door to us was Bonaventure salmon creek as we were ready to move, and they had availability. We decided that would be the best place for dad to start assisted living. We soon realized he was wandering a lot and one night as he walked away from the facility, they moved him into memory care the staff was wonderful, although I was noticing that most of them were new to the facility and there were some red flags as far as paperwork being lost, making sure the we were being heard as far as his needs where he is now in memory care I feel comfortable he is safe he is being well cared for but my concern now is they do not have a buy down to Medicaid so I have started a new search trying to take my time and find a better place for him where his finances will last and we can do a buy down to Medicaid in memory care.

Main complaint is that there is limited communication with family about anything specific to mom. Also, showers frequently skipped. But my mom loves her room and the CNA's respond very quickly. My mom wants to stay there. They treat her well, though always rushed

We had an in person tour experience with Bonaventure of Salmon Creek, and the community was very clean and the staff members we met with were friendly. Overall a great tour experience.
I fill sorry for Bonaventure Memory Care residents . There is always a shortage of workers . The rooms are never cleaned. Disgusting food . From the promised service , approximately about 10% . People are constantly bumping and falling .If you love your parent or any relatives , you never them live in Bonaventure Salmon creek Memory Care .

Upper management made promises that were never followed through with. High employee turnover, terribly understaffed. Meals and meal service is horrific. Sub-standard at best. Housekeeping staff is overworked and not given sufficient time to actually clean. Communication with management was next to impossible. Had to call several times to ever get a call back. Loved one ended up with serious injury, which was not reported, or family notified. Loose meds were found in loved one's room, that were supposed to have been dispensed and witnessed by med tech. It is a beautiful facility, but the looks of the place don't outweigh the negative experience of having a loved one treated as just a rent check. Utterly disappointing.

My father lived at Bonaventure Salmon Creek for 19 months until we had to move him to another facility because they were not honoring the care plan agreed upon. He was in assisted living and was supposed to be notified of meals and made sure he ate. He lost 12 pounds in about 9 weeks. He also fell and broke 2 ribs and had been complaing to caregivers about the pain and the family was never notified. He also cut himself enough shaving that we found the bathroom trash full of bloody tissue and again we were not notifed. We reported this to the Washington Department of Human Services and subsequently they werecited.

I would recommend Bonaventure. Had a bit of frustration over my mother-in-law being able to take her own vitamins. But it was (eventually) straightened out. The staff is approachable and generally available. We are very satisfied with the situation.

What we do like about the community, is the openness, brightness and most of the staff are really sweet. The staff members are friendly but there aren't enough of them to provide the services that the spouse expects and is paying for. And the administration is difficult to find and deal with.

It was not a good experience for us. In the beginning it was fine, but no one ever did anything that they said they were going to do. We got no orientation or greeting. Administration was never there or in contact. The aides were great who worked with my loved one daily! I think it might be a good place for those who are more independent and can take care of themselves. But as far as assisted living goes, I would not recommend them. My loved one just needed more care than they could provide.
