At Autumn Leaves of Cityview, residents are treated like family and cared for by experts. This purpose-built community in Fort Worth, Texas, is proud to provide residents with memory care services in a peaceful environment — while still being near all the major attractions that the Dallas-Fort Worth area has to offer.

Residents at Autumn Leaves of Cityview receive personalized care because no two residents are the same. These personalized care services range from assistance with activities of daily living to on-site palliative care. Residents have the choice between studio, one-bedroom, and semi-private suites to call their own. These suites are spacious, sunlit, and come with amenities to help ensure resident comfort and safety.

A professional chef thoughtfully prepares meals in the restaurant-style dining room where nutrition and taste are celebrated. In between meals, residents are encouraged to participate in on-campus activities and to take advantage of on-campus common areas. Between game nights, religious services, outdoor common areas and more, residents are truly home when they step into Autumn Leaves of Cityview.

My Husband

My husband has been residing at Autumn Leaves Memory Care Facility in Southwest Fort Worth since Oct. 1, 2017. We chose this place because it is small, everything is on one floor, and they provide a lot of activities. It is also close to where I live. We have been very happy with the treatment he is receiving. The entire staff know him and call him by name whenever they see him. They are always giving him a little touch on the shoulder or a pat on the hand to show him that they care. I have not heard one person ever talk down to him or raise their voice. Since the disease has taken over more, someone sits with him at every meal to help him eat I highly recommend Autumn Leaves

Terrible Service

Staff rude [Removed] rude leave patients dirty patient's roaming the facility 1 trying to leave out the front door wondering around entering other patient's room's facility staff attitudes are [Removed]. Correct information not given most the staff the few there aren't trained to properly care for patients I wouldn't recommend.

Wonderful Place for your loved one

My mom has been there for about two months and it has been great. My mom has adjusted well. The caregivers are very compassionate and attentive to my mom needs. The food is great she has actually gained 5lbs since her move in. The upper management team is awesome the executive director is always available. The nurse is great along with the doctor who have come together and made adjustment to her meds and her behaviors have decreased tremendous. They have activities going on around the clock. This community is great and would recommend to anyone who is looking for a wonderful loving atmosphere for their loved one .

We have our loved one here and it’s a lovely Place. The executive director is a nurse also and her door is always open to families. The nurse is attentive and caring . The activity director is great and has our loved one up and doing things . The food is great tasting . [Removed] at the front door is always inviting and willing to help. The building is small which helps so much with my mother , we had her at a larger building and it was just to much for her . She’s adapting quite well here. The caregivers are very good and trained in dementia. The building is decorated nice and always smells good ! Our mother had quit walking and now she’s walking with assistance. The staff truly love their job all the way to the executive director we have came in on weekends and at 9pm and she is there .

Very Disappointed

We are very disappointed with all the lies we have been told at this community and we are about to move our loved one out. Our loved one is declining due to not getting the care that is needed or deserves for what we are paying. They went 3 weeks without a shower or brushing our loved ones teeth. They say they can't figure out how to do it. They would ask and of course said no they have dementia and they just left our loved alone in bed. Needs a little motivation to do things. They have fired the activity director and haven't replaced her yet. And the nurse quit that we had a good relationship with and the new one I can't put in words how we are not pleased with her. All the residents are just sitting in the common area when you walk in just sitting with nothing no music even. The caregivers are nice as can be but they are overworked and doing the best that they can.

Doing Okay From What I Know

From what I understand mom is doing okay here. Her eating has picked up so she appears to be liking the food. She says she is participating in activities and meeting new people. The staff seems to be friendly and helpful.

Taking Good Care of My Loved One

We are satisfied with my loved one move to this community. He is doing well, and I know he is receiving great care. The staff have been very supportive, and helpful. They have been good to work with. The community is well-kept, and they do a good job with the cleanliness. This is a community I would recommend, it is a really nice place.

Friendly Staff!!

My mother was in this community for a period of time before her passing and I was pleased with the level of care she had been receiving. They where very friendly and made sure to give my loved one great care and attention. I would defiantly recommend them to anyone interested in looking into an assisted living center for them or there loved ones.

Seemed very nice, but not a good fit for us

I thought Autumn Leaves of Cityview seemed like a very nice place, and would be a great option for someone who needs the level of memory care that they provide here. The facility was well kept and clean, and all of the staff I met seemed friendly and professional. However, I don't think the care was the best match for what my loved one was needing at the time.

The Best Choice

Everyone at this care facility was always concerned about taking care of my loved-one with the utmost dignity, while also giving myself and other family members constant support and guidance throughout the stay. We were very impressed with the staff's professional demeanor, and also the wonderful accompanying services of Grace Hospice. I would highly recommend Autumn Leaves to anyone in search of a place to care for their own loved-ones.

Very Nice

The Autumn Leaves of Cityview is very nice. I like the activities director. I like what she does for the patients. My ex husband Richard lives there. The food is good. Everyone is nice. I like to come visit and have lunch with Richard. This is a very nice community. I enjoy visiting. Everything has went very well. This is a 5 star community. I would recommend it.
