We were looking for a place for my mother (94) who had multiple issues after breaking hip. The Staff were exceptional in providing assistance and care for all my Mothers Needs . The facility is very nice and clean and the food preparation was excellent . Every question we had was answered professionally and promptly. My Mother is very happy with the choice we made. This has been a huge relief for me and my family Thanks Autumn Leaves for providing the care we needed at this time . We , in turn have used your name for reference to my friends with older parents that need assistance also

My loved one is currently a resident at Autumn Leaves of Cityview and things are great! The facility is very "home like" and the staff are very friendly. Communication with the family is very good. I feel like they listen if/when I have any questions or concerns. The food is pretty good and there's plenty. I would definitely recommend Autumn Leaves of Cityview to others.

My dad moved into this facility on 1[date removed] healthy and walking. We discovered him 4 days later in a horrific condition and we had to notify and alert the staff at the facility that and insist they call an ambulance. He arrived at the nearby hospital with a broken femur, hip, multiple lumbar fractures, black and blue bruising all over his backside and contracted MRSA, all within 4 days. He spent 2 weeks in the hospital and then died 10 days after that in hospice on [date removed]. This all occurred within the 4 days he lived there and no one has any answers or has offered any suggestions or any help whatsoever into what happened. RUN. Please do not subject your loved one to these conditions.

These people were under staffed not always trained to know how to care for your loved ones, and they let all of my loved one's clothing be covered in pee and placed in trash bags reeking off ammonia the stench in their room was unbearable ( my loved one had 18 pairs of pants and 27 shirts plus 2 coats a sweater ) and not 1 thing was clean even their bed and the comforter on the bed was soaked and puddled up on the mattress protector 2 days before my loved one died. The photos of my loved one's dead body on the third day after that were so bad the man who picked up my loved one's body and the funeral director said my loved one was beaten to death I have photos they (the home) would not unwrap the new gauze after my loved one died to let me see the wounds but even so you can still see how bad my loved one was treated . My loved one died on the [date removed] day of the month and their room and all possessions were removed within two hours of their death but there was no return of the balance of the bill( no money) sent back to me for use associated with my loved one's funeral. The lady who was running it had left their employment at some time before this (Why) ...... I believe an employee or another guest was responsible for my loved one's death but I cannot prove it . Yet not enough supervision of patients. Too short handed. I recommend that you not send anyone else here.

We were very happy with Autumn Leaves while we were here. Their Executive director that they had was awesome, her name was [name removed]. She is a nurse as well and she was very hands on with everything but she is gone now. I have heard it has down hill since she left. I would be reluctant to go their now because of all of the management left and they did not replace them and they do not have the proper training.

They had community showers and seemed like a lot of staff turnover here. The marketing wasn't the best. This wasn't the right fit for my loved one.

Communication between the staff and the family was not very good. I had to initiate any communication and it seemed like my question had to get passed to several people before I got an answer. We did not have a care consultation the whole time my loved one was there. I would like to have seen better care coordination, it seemed there was not a plan to follow. I would not recommend this community to others

They have just increased their rent prices against his month, it seems to happen a lot. They are slow about passing on information especially about covid. They are never ahead of the curve and always seem behind. Overall I would not recommend to others.

Our first impression of Autumn Leaves was excellent, we really liked the director and executive director of health services. Everyone there was very open, they answered all of our questions. They were all extremely friendly and caring. The activities they offered looked excellent but due to COVID things had to be modified

My mother was cared for here from February 2016 until she passed this June 2020. After touring other facilities, Autumn Leaves immediately rose to the top due to the lovely surroundings, caring staff and caregivers, and their desire to treat each resident with respect and kindness. The food is good, there was a variety of activities and events (more so before Covid hit), and the community looks more like a hotel than a nursing home with beautiful furnishings and decor. Obviously, no place is perfect, but in my experience I would choose them all over again. My mother was well loved there and I'll forever be grateful.

visited Autumn leaves today along with several other places. This was the only one that had visible staff and the people living there were actually being tended to. It smelled nice and the staff was extremely friendly. I visited several other places today and most took quite a while to come to the door, the stench of urine hit as soon as they opened their door. I didn't get to see much of the places but people didnt look happy. I don't know how the care will be but I do plan on pursuing moving my aunt.

Our mom has been at this facility for about 1 year now and we are very comfortable with how she is treated and cared for on a daily basis. She seems more relaxed than we have seen her in several months, not nervous and anxious. The staff is very attentive to her needs and they are very good to her. She is relaxed around them and this is a huge improvement from the previous facility. We completely trust the staff at Autumn Leaves Cityview and trust the care they are providing to our mom. We are there 3 or 4 times a week checking on her the staff can always update us on her appetite and activities. If for some reason we cannot go for a day or two, we can always call and any staff member can update us immediately.

Horrible place. Understaffed, over-medicating the patients so they don't have to watch them, and management doesn't do what they say they are going to do. My mother fell over 25 in 2 months.

I researched several facilities before selecting Autumn Leaves of City View. The deciding factors were; that Autumn Leaves is only memory care facility, the size of the facility with its staff-to-patient ratio, the cleanliness of the facility, and the politeness/attitude of the staff. The services which Autumn Leaves provided were equal to or exceeded what they had advertised or verbally expressed.

We began feeling great about our move to Autumn Leaves. Having a camera in Mom's room gave us a reassurance she was receiving kind, compassionate care. We are a family who visits every day and I know that helps also. As the months progressed we began to have problems with medications not being given properly and even allowed her to run out of medications. In each incident, we had to be the ones who brought these oversights to the administration after we observed these problems on our camera. Excuses were made and the administration would never take responsibility for the problems. They did try to cover things up and even questioned employees as to how we (the family) found out about these problems. We even tried speaking to the area excutive director with no help. The next thing we knew, suddenly the WiFi for everyone's personal cameras was being turned off. We gave our 30 day notice and found a better place, along with several other families with loved ones in residence at Autumn Leaves. We felt our mom received great personal care from the care givers despite the lack of support they received. The administrators, at all levels, were strictly there for the money in our opinion.
