We toured a few different communities and this one seemed to be the best fit for my mom. She is doing well here. She is starting to meet new residents and make some friends. She is participating in some of the activities. She really likes the food. The community is always clean and well maintained.
Great facility that is meeting all my parents needs! Great for memory care. The staff is very friendly and helpful to get them adjusted
Mom moved in to the Oakmont in San Rafael less than a few months ago. The Traditions memory care team at Oakmont and other team members already feel like family members for Mom and me. This has been a tough transition for both of us and the team has really been so compassionate and supportive. I live close by and I'm able to drop by every day to visit and join in activities like daily morning stretches, brain games, garden picnics, and Thursday live music (so great!) Everyone from the receptionists, the management team, the med techs, the care givers, and the activities team truly care about what they are doing and give 150%. My family is so grateful for the loving care that the the Oakmont team provides for my Mom. And, I appreciate the emotional support and responsiveness whenever I have any questions or concerns. When faced with challenges, they are quick to find solutions. We feel lucky we found Oakmont.

Nursing homes, also called skilled nursing facilities, provide long-term care for seniors with significant physical or mental health conditions, and short-term care to people recovering from an injury or hospital stay. Nursing homes provide personal assistance and medical care from licensed professionals. Services in skilled nursing facilities often include:
A nursing home offers lodging to at least two persons who are not related by blood or by marriage to the administrator. This type of establishment offers its occupants 24-hour care administered by on-site nurses. These nurses offer healthcare services for those who cannot adequately take care of their needs.
This type of community is a helpful and comfortable care alternative for persons 65 years or older, however nursing homes will routinely admit individuals 55 and greater (a rarity in the world of senior care). Some services come at an extra cost which can include cosmetic and grooming items, personal reading materials, and personal comfort items. A significant number of nursing home residents are persons who experience difficulty with hearing and sight, require assistance with daily tasks such as bathing, have incontinence issues, or have mental or physical disabilities. Examples of care services and amenities offered at nursing homes might include social and recreational activities, care coordination and case management, 24-hour emergency care, and help with legal questions. Nursing homes are required to provide room and bed maintenance services, routine personal hygiene items, dietary services, and specialized rehabilitative services (treatment for mental illness or intellectual disability) to residents. The most frequent mental disease effecting residents of nursing homes is dementia, which can cloud their decision making capabilities, memory recall, and and familiarity.
