Inexperienced and incompetent management results in emotion-driven decisions (rather than logical/rational) that can endanger the welfare of their residents. I'll report healthcare violations with Florida's Agency for Healthcare Administration to request full investigations. The formal complaints may take time to appear on the AHCA website. Beware of this facility. There are many better options. Qualified and experienced management is paramount. This facility lacks both

They have recently gone through some changes but I'm happy to say they are all for the better. They currently have a very caring staff. Now they just need more residents! My mother is 100, and says her caretakers are very nice to her.

I've explored several assisted living communities both in-state and out of state and decided to move my mom to Florida because of the high quality environment and value of care at Fountains of Hope. It is a clean, loving, friendly community that caters to a calm senior lifestyle, which I feel is important. I was also able to arrange onsite medical visits by coordinating with the staff and my mom's healthcare insurance company.

The Fountains of Hope is a warm, comfortable living center with delicious food and fun entertainment and friendly people. The space is beautifully decorated, very clean, and brightly lit with high ceilings.

I would recommend this facility to others. Especially to people who are quiet and easy to get along with. The staff, from top to every level, are kind and empathetic. The place is kept very clean. No pool, not fancy, not for socially active or high energy folks looking for excitement. Readers, single men and single women who are polite and gentle would find the quality of care and company of others very satisfying. The activities are good, and there is encouragement, but no pressure to be involved, if a resident would rather stay in his/her apartment.

Overall impression of Fountains of Hope is very good actually. It appears to be well organized, and very clean. The people I’ve met our competent, and the medical skills are good. I was able to get access to necessary medical professionals when needed.

On the whole, it’s a good community. The people are friendly and kind, the food (especially breakfast) is good, and the service is terrific. People going out of their way to help us. Very clean place.

My father lives at Fountains Of Hope and we’ve been very happy. The team cares, and takes good care of my dad, and that gives me peace of mind. It also seems to be very friendly and the staff very responsive. We’ve been happy.

I would recommend this facility to anybody, the level of certification which was a huge learning curve from us is outstanding here. They have higher certification so they are able to care for our loved one. The head nurse that was there our loved one was, had a dynamic personality. She was so caring and on top of everything. She was particularly outstanding. The activities person seems to be very caring, they always have the residents outside in the common area. The receptionist was an angel sent from God - she was truly amazing. They also had a lot of fresh fruit also and did not ever seem to really repeat meals.

My mom was forched to live in this facility. This facility was so poorly ran, not enough staff, staff not qualified to take care of my mom's needs. They did care to groom her and neglected her in every way. She was left alone and so were the other residents. The memory unit was dirty and smelled most of the time. I would NEVER leave a loved one in their care. The food was disgusting. I tasted all food just to know what my mom had to eat. Please run fast from this place. There are NO doctors on site and NO RNs either only LPNs......this is so unexceptable. My mom was left dirty and all her dignity was taken away. My mom was also left cold without covers, and left to go pee in her bed than lay in it.....horrible place.....run, run, run!!!!

They did not keep the residents' rooms clean. There is not as much medical care or knowledge as you would think. It is a nice group of people and a nice facility, It is a newer place. They are not very organized.

Management and employees come and go very quickly. CNA's are not taught proper care for the elderly in the memory care (lockdown unit). Fights have been wittnessed between residents and CNA's and falls have been wittnessed. Many violations have been wittnessed also. The food at this place was decent until they switched management to Pegasus....now food is not very healthy at all. Mostly carbs which is not good for diabetics or people with heart problems. When served food it lukewarm and takes about an hour to get. No hair nets are worn by the staff in the kitchen leading to hair in the food. Unfortunately people do not look happy here and do not get the PT or OT ftherapy that is need to build muscle and range of motion.

We setup Mom to be in a respite stay post surgery for hip surgery after a fall. She had already come down with Dementia, and she was needing extra care without being home where there were too many obstacles to walk around. The initial month was OK, but then things were declining rapidly. Since June, there have been incidences where the entire kitchen and wait staff have quit. They have many times asked for medical staff to wait tables. This causes a huge turnover in the qualified health care staff available to help patients, and they resign all the time. Food was once plentiful and tasty. Now the portions are so small, there is hardly enough caloric count to sustain someone. One afternoon, my wife and I were there to visit and stay for dinner. Dinner service starts at 5, and we arrived at 5:45pm. There were no staff cleaning tables, and no place to sit where there was clean dishes, glasses, and utensils. After 30 minutes, mom got up and went to the kitchen to yell at someone to get out here to serve dinner. And now it is Nov, and things are far more worse than that because they have lost the whole wait staff many times over and are on their 3rd chef in 6 months. In addition, there was a power outage and we found that the facility is in violation of State statute by not having any backup power generators as required by law. Our private duty nurse was helping other residents who were in total pitch black darkness, and their oxygen generators and other medical equipment stopped working. Fountains has only 2 personnel on call at night, and they were nowhere to be found. They each have an entire building to cover. During this power outage, the locks securing the doors in the Memory Care wing automatically shut off and allowed the Alzheimers patients to freely leave. One patient left the facility, and walked to a neighboring church, climbed into the back seat of a car, and was transported to another neighboring town (Venice) by an unaware driver. She was eventually returned. So during any storm or hurricane, the power can shut off leaving residents in total darkness, without medical equipment that requires power, and unsecured buildings that will allow residents to freely leave and wander towards the adjacent interstate, the woods and swamp and canals behind, or into local neighborhoods. I would not recommend Fountains of Hope to anyone who truly has any love for their parents/spouses. I was lucky because I was providing a team of my own private duty nurses to be with mom 24/7 apart from the staff. They ended up cooking food for her and bringing it in. Don't waste your money here.
