Mom constantly complained about food and service. She took food in her room and would say, I'll eat whenever it gets here. We picked this community because of the location, easy access from Freeway. Overall pleased, great care

The care was very good. [name removed] the wellness director was phenomenal. The billing and business management/financial end was inefficient, it was one mistake after the other. My advice to others would be to keep on top of that. Overall I would recommend Pacifica Senior Living San Martin to other families.

our families experience

We do not have anything really to compare the value to but overall we do think it is good. We do think the food has a reasonable palate for senior citizens. Our loved one has not been the most receptive going to the activities but what she has done she really has enjoyed. We would have families consider this community for their loved ones has it really depends on their level of needs and care.

Getting Better

I like the community. The apartment is very roomy with a walk-in closest with a full size fridge. I am disappointed the time to get housekeeping established on a regular basis. At first I don't feel the staff were checking on my loved one as often as they should have. The communication with the staff can be hard to get a follow up with the family's questions or concerns. There could be some renovations to update the carpet in the community. There are three meals a day and choices for the residents to choose from. The residents have the choice to make food in their apartment or have the meals in their apartment as well.

Our experience

We have been disappointed with the cost, bookkeeping, food is just awful and new management seems more attentive to needs. Staff is very friendly and helpful but seem to be overworked.

Do Your Homework

Cleaning sporadic, issue with theft, call buttons not answered, paid for hourly checks not done phone calls/emails unanswered, fire door/security camera not functioning, resident bully not addressed. In my opinion the staff that interacted with my mother were caring and kind and most were doing their best despite the complete lack of support and recognition of leadership

Dear Sally, We believe that this review regarding our services has already been addressed with you and your sister. To recap, the Nevada Department of Health and Human Services gives Pacifica Senior Living San Martin with an A rating for the care of seniors. This would not be the case if we were lacking in care and cleanliness. This has been substantiated by Adult Protective Services. With regard to security and allegations of theft, the police department does not find any credible reason to pursue this matter. We are sorry that your mother passed away and is no longer a part of your lives but it is home to many happy and thriving seniors. We ask you politely to refrain from unhelpful comments about our community in the future.

Our experience at Pacifica so far

They are having some staffing issues, so they are cycling through a lot of people. They are having issues with the food, the cleaning service and the activities. The new person who took over the cooking is better and is trying alternative recipes with more vegetables and fresher foods, especially at dinner. I am seeing an improvement there. They also have a new activities director who is working in improving those. A big thing I noticed was that staff were coming in at odd hours while she was sleeping and she is independent living. She brought it up in a meeting and they seemed to listen to her suggestion. I would recommend them, they are trying to make things better and we are hoping to see more improvements.

Dear Kim, Thank you for taking the time to tell us what is right about Pacific Senior Living San Martin. We are also happy to report that there were no complaints about our food service during the most recent resident council. We have looked into staffing, and while it is common for people to change jobs in this post-pandemic world, we are fully staffed at this time. In terms of staff entering a room. Many of our independent residents wear pendants and can fall asleep on the pendant. When that happens, we enter a room to turn it off. Some may also call in the middle of night experiencing confusion, which can be expected. We are glad your mom mentioned this and resolved her personal matter.

The food doesn't seem like it's very good quality for the price. Communication with the family is ok. I suppose I would recommend them to friends and family.

Our Experience at Pacifica San Martin

On the surface, it looks clean, which is one of the things that attracted us to them. But once you spend time there, you start noticing things that are not clean. However, even so, it was much cleaner compared to other facilities we toured. Some of the staff are super friendly and helpful, and others don't give you the time of day. The biggest problems we experienced were with maintenance and the medical team. Because the residents can push their own alert buttons, the staff don't respond to them quickly or take them as a serious matter. One night when I was there visiting my family member, another resident told me she was having a stroke, so I sat there with her while we pushed her call button. Eventually the staff would come by and tell her "Oh, you're okay" and then leave. I sat with her for several hours before someone took her back to her rom to take care of her. While my family member was there, he had fallen. They didn't properly document it or let us know that it happened. When we asked about it, they told us he fell asleep on the floor and decided to stay there, which did not match up with what hospice had reported to us. Maintenance requests took quite a lot of effort to be taken care of. We had a problem with theft perpetrated by a maintenance staff member that has since been resolved but was difficult to deal with in an already difficult time. Often our phone calls weren't returned. The food I tried was okay, but the other residents, including my family member, hated it. There was some staff turnover, and the director left while we were there, so we were tossed around to a lot of different people. The lead nurse is probably one of the most helpful people there. Also [name removed] at the front desk is great. She has been there even before ownership changed to Pacifica. She's definitely the face of the place. The residents feel she's one of the only ones who cares about them. She genuinely takes a true interest in every person who's there. Without her, they would feel like the don't have a voice. We appreciated her extra set of eyes for our family member. If we needed something, or if I needed to know how he was truly doing, I could get an honest answer. The other staff would just say "Oh, he's fine" when I knew that wasn't the case. In general, I was disappointed in the reaction time of the medical staff, especially if it was an emergency. I would not recommend them, especially for residents who need more care. The independent portion seemed perfectly fine. They have a lot of activates to choose from. But if your person needs extra care, it's not a place I would recommend.

We know that everyone’s time is valuable so we appreciate your favorable comments about our lead nurse and our front desk staff member at Pacific Senior Living San Martin. We have talked to our maintence team. They fulfill work orders throughout the day, except for emergencies which are handled immediately. Staff report that some families do not always follow the work order process which starts by calling the front desk. This causes confusion about the timing to resolve maintenance orders. With regard to medical staff, we have reviewed procedures with medical staff and have followed-up about your personal experience. Feel free to voice your concerns with the lead nurse moving forward.

Memory Care Side Was Great, The Other Side You Need to Stay On Top of Them

Our experience in Memory Care was much better than the Assisted Living side. The Memory Care unit staff were more attentive, took care of medications, and bandages for my loved one. The staff were loving and pleasant with my loved one. The hospice nurses were wonderful as well. The Assisted Living side of the community I was not pleased with our experience overall. The front desk staff members were nice to work with on the assisted living side of the building, as well.

My tour review

This community was okay but it was a little to much for my loved one. This would be great for a retired couple that wanted to go into senior living but still active. They had a lot of amenities but my loved one would not have been able to participate in a lot of them.

Our Experience

Everybody is super friendly and always willing to fix any issues as soon as possible. They are short staffed. Housekeeping is supposed to be cleaning once a week and it has happened once. We had medication issues, missing his medicine for a day. I honestly think this is due to a staffing shortage. They provide a variety of meal options. They said they have transportation for doctors appointments as well as a shuttle that comes for the residents. Any communication has come from the executive director and he seems right on it. They have a portal that you can make your monthly payments on. We have had issues getting logged into it as of now.

Pacifica's Not Bad

It's still a relatively new experience, so the jury is still out so to speak. I'm definitely not sorry we put my family member here. She's getting used to it. It's always a transition going from your own place to community living. But overall it's been good. She moved in during Covid, so there were some challenges related to that, but things have been getting better. One of those challenges was fewer activities, but they're just now starting those back up again. She generally likes the food and is not losing weight. The staff have been good. When we've had any sort of problem, they've addressed it without delay. And the director has been more than helpful and has gone above and beyond.

Not everything they promised

This community was probably one of the top places at one time and this is what I heard but things have changed and now it is different. I did not get a lot of what I was promised so right now I would not recommend this community to a friend. Rooms are not getting cleaned and options for diabetics are minimal

We are so sorry that our community has not lived up to your expectations. Since COVID came on the scene in 2020 one of our ongoing challenges has been staffing. As with all senior living communities across the country, it’s been tough to hire and retain the right staff. As a result of that, here are some processes we’ve put in place to mitigate those challenges as much as possible many of our current employees have been cross-trained to help pick-up in depts that may be challenged with staffing. This includes our dept heads. We have employed outside agencies as necessary to supplement services We have offered various incentives to current employees to ensure they know we appreciate them and their dedication during this time. We are seeing a difference as more and more people go back to work and things are opening up after COVID protocols restricted things for nearly 2 years. We’ve done the very best we can to ensure we are meeting all of our resident’s needs, so we again apologize if you felt that wasn’t happening and hope you’ll give us the opportunity to make it right. In terms of diabetic choices, we think we can clear up some of your concerns as well: Diabetic management is different now than in the past; we don’t serve a specific “Diabetic Diet” in our communities, because most doctors no longer strictly adhere to that theory. Instead we offer a variety of meals, and an always available that can be modified to meet your dietary needs ( i.e No sauce on chicken, smaller portion of a dessert instead of only sugar free, etc) We have enough menu items each day that you should be able to choose selections that are in line with your dietary requirements. Our goal is to have enough selections to allow you to make choices rather than telling you what you can and cannot eat. We have a registered dietician who approves all of our meals and 2 corporate dining directors who would be glad to meet with you (in addition to the on-site chef) and give you suggestions on items and portions that will work best with your dietary needs. We would be happy to sit down with you if you’d like, just let our Executive Director know and they can set up time to go through your preferences and needs.

Excellent from start to finish.

They are awesome across the board. From the first meeting with [name removed] in sales, all the way though when my mom passed, they were excellent. I couldn't recommend it more highly. The location, everything. The staff were great, including the on-site nurse. They did everything and more than I could have imagined.

Best option for assisted living in Nevada

There are really nice people at this community. Very elegant setting with competent staff. They have activities like games, they have a casino, arts and crafts, a salon ,recitals and more for the residents to do. They also have a good size room in the community which makes it nice. Hallways lined with furniture and big open spaces with t.v.s for group watching. They also have a little pub for the residents.
