We liked the community. The residents were a little older than my loved one, and this was the downfall for us.
The staff is wonderful, friendly, and welcoming - so are the residents. The food is good. In terms of assisted living, folks are able to assist in areas you need and allow you to be independent in other areas where you don't need assistance. We are very pleased with Harmony at Five Forks!

We have been pleased with our experience with Harmony at Five Forks, I would recommend the community to others for their loved ones. There are some items we are still trying to work through. The staff are nice to work with, and are apologetic when they can't get back to me in a timely manner. It seems the residents enjoy the food. Some of the activities that are available are outings, outside entertainment, bingo, puzzles, and more for the residents to choose from. The monthly fee includes the room, housekeeping, all utilities are included except for tv and internet. There are washers and dryer in the apartment and a full kitchen.

We chose Harmony at Five Forks because the location was very convenient for us. Also the atmosphere is amazing, when you walk in you feel like you are walking into an upscale apartment complex and not a nursing home. The residents were all very happy and social. Going through the hallways it is bright and clean and the staff all seemed super friendly and accommodating. Even during the hard time of staffing, they have just been phenomenal. I would definitely recommend to anybody looking for a place.

This is a good and clean community. They seem to have a lot of programs available for the residents. I feel like I have to find out a lot of things on my own. It would be nice to have more communication from the staff about my loved one. The evening staff doesn't seem to know as much as the day staff does. My loved one seems to enjoy the food here.

Harmony is a beautiful facility, very clean and nice. We were looking for more freedoms for my mother to come and go as she pleased.

Please practice fairness and equal treatment to all Memory Care residents. Below is the timeline of my loved ones eviction. He is a Vietnam vet who suffers from FTD. Who spent 2 weeks there. On 14th day: *Director CALLED & said my dad can only stay if he improves in 2 weeks. 4 hours later, she EMAILS & states he has 30 days to leave. *His meds were mismanaged; was only given AM dose of 2part med. *Did not wash his sheets for 17 days. Stained and bug ridden *Found urine drenched towel in shower. *Furniture was extremely dusty & had a bug. Saw 3 spiders in closet. *No ventilation in room. *Doorbell to facility is broken. Wait time for response 15 in+. *No diversity. Dad was only resident of color in Memory Care. *Staff untrained on assisting sufferer of Picks (FTD) *Tried for 5 days to return room keys. No reply to calls or email.

It was a very nice community. The community was too big for us.
The staff are so involved in the residents' lives and care. We were able to move our loved one from independent living to memory care when needed. The staff was always keeping us updated on our loved one's care. They have one wing for individuals that are truly independent.

The memory unit is not prepared for memory care patients. They are not trained on how to deal with them or look for what the issue is. The communication between the staff and the community could have been better. I would not recommend this community to others.

I met with [name removed]. I really like the facility. Everything is accessed from inside the building. It is only 3 years old. It does look like a hotel from the outside, but it does offer some nice amenities. The pricing is higher than some of the older properties I have visited. They have a third party - PT/OT facility on sight.

I loved Harmony and would like to live there now! However, it did not seem a perfect fit for my mother because of its size. Harmony is huge. It has four floors and covers a lot of ground as well. The activity schedule and the menu offerings seemed top notch. The staff was super friendly on our tour and greeted residents by first name. The residents seemed happy and healthy too. My mother would like something smaller and closer to the ground (she's afraid of heights!) but I think this is a five star facility myself

This is a switch and bait operation facility. They’re only two years old and have already had to change their name. Staff turnover is high, the remaining staff are mean and rude. I pulled my mother out after months of neglect and VERY long waits between her call button activation. Staff (if we could even FIND them) refused to assist her in the bathroom after she BEGGED them for help. She waited over an hour at meals between ordering and finally being served. The directors are nasty and only care about their own reputation & bringing in money. PERFECT place to place mom or sad if you hate them. Check out The Springs or Brookdale. Avoid this place like the plague.

Beautiful, huge facility. [Name removed] was great to talk to and very knowledgeable. On site staff is only LPNs. Contract physician and and NP services. Quality place but you definitely pay for it.

At the last minute, we decided not to move into The Crossings because they pulled a "fast one" and were going to charge us additional monthly fees. Not the kind of people I do business with!! I would strongly suggest that you NOT recommend The Crossings to anyone!!

We are so pleased at the Church services that the Crossings have every two weeks--the two or sometimes three guitarists are WONDERFUL and the Preaching from the tall bearded man is exceptional--thank you all for having them come every other SUNDAY at 9 am, Mother and Father just love it--they asked why more people do not come

New facility that was not ready for what they said they would provide. Assisted living side is run poorly. Would not recommend if you take numerous meds. They can’t keep up. Changes in meds sometimes not documented. Nurses station not staffed around clock. Complaints from staff they are very busy and short staffed. Turn around on staffing was a lot for short time open. No farm to table food as advertised. Bistro is a machine and half the time no cups, snacks, etc. No market place as advertised, Stopped giving out snacks. Not sure on independent side. But, would NOT recommend assisted side.

We are happy with the care from the staff here at this community. The community is clean with a nice atmosphere. The menu is appealing. I like that they offer activities for participation for all the residents. We would recommend this community for the good value of care that is offered.

I was looking into this community and got good feedback about the future of the community. However it won't be open until Spring and it felt too big and corporate. It looks like it is going to be super nice.
