My grandfather has lived there for 2 months, and so far he's been very happy. He in their dementia area, and they make sure to take him on outings and it's apparent they actually care about him. They got to know him, and when he seemed off, he was sent to the hospital. They saved his life that day. I've eaten there, and really liked the food I had, private bathrooms with a shower right in the private room. Lots of activities.

Facility is clean. The staff are pleasant, friendly and seem to care about their residents. Having an affiliated PCP makes addressing any developing health issues easy to address and very convenient for follow up care.

My Dad was only there temporarily (twice). Attention from in-house doctor was not up to my expectations and we did not care for their new pharmacy (charged for things that weren't my Dad's).

We toured with Oak Leaf Manor North and it was just ok, we noticed that it seemed to be very crowded and it also smelled. Those were the main reasons we did not choose that facility.
We picked this community, because of the inclusive health care. It has been very positive. I would recommend them to other families. My mom says all the staff are very friendly.

1. Overall experience was a 2. They were constantly understaffed and my mother was ignored most of the day. There was poor communication between the shifts. the cleanliness was good. One staff member (RN) was very friendly. Others didn't speak to me at all. They also didn't engage any residents at all unless it was to deliver food from the kitchen to the table. Quality of care was not what I had hoped for. I was desperate for a short term placement for my mother so I had no choice but I wouldn't leave her there again for long term care. Meals were fine. Weekly events were fine but daily activities were non existent. My mother just sat there with nothing to do. Value for the cost was not acceptable. The cost of dementia care is high for a reason. It's exhausting work. However, for the money that was paid, I expected more attention would be paid to the residents. I was grateful for the last minute ability to place her there but greatly disappointed in the service provided. I am extremely grateful for A Place for Mom for helping me find a residence at Oak Leaf Manor for my mother in such an emergency situation.
After touring other places, Oak leaf Manor North was up in our top 2 choices. The facility is beautiful and the staff that we had interaction with were all very friendly and accommodating. For us it came down to the location, we wanted some place closer so we could pop in whenever we wanted.
Our experience with our loved one has been good, but you need to make sure to communicate with the staff on a regular basis. The staff communication with the family hasn't been very good. The community recently changed administrators, and there was no communication towards the families with this information. The care has been good and I feel it could be better. However, my loved one seems to be in good hands and is in good spirits. Some of the activities that are available are movie nights, music, bingo, games, and more for the residents to choose from. The community tries to engage the residents by bringing them out to the common area to watch the activities. There is a good variety of food and the dietary options are good as well.

The staff members were friendly, it was a good value but you were not getting anything for it. We feel that our loved one did not receive the care and attention that she needed. We understand covid had an impact but it was not living up to our expectations that they had offered.

We chose Oak Leaf Manor North because it seemed like it was going to be a good fit for our loved one. The room sizes here are adequate for his needs. I have not eaten here, but he has had no complaints about the food. The best part about the community so far has been the nice atmosphere. I would recommend.

Dementia resident left the facility and was gone for hours and no immediate notification was made to the family. Pedicure was neglected for so long that the resident was no longer able to walk - facility's response was to deem the resident "non-ambulatory". Caregiver was diagnosed with Covid and passed it to several residents yet no notification was made to the family. Resident spent several days suffering from a fever, during a pandemic, no Covid test was issued and no notification was made to the family. When the facility was questioned about any of these incidents, their response was to stop returning phone calls to the family. Facility is beautiful and staff present themselves as friendly and caring but the reality is that they are more concerned with hiding their mistakes and irresponsibility than the well-being of the residents.

We chose Oak Leaf Manor because when I got there it was the only place that smelled nice and refreshing. Other places I walked into had a bad stench of urine. They are a very clean facility. The room sizes I feel as though are very good. He was in the dementia unit and he did not have to share a room with anybody which is something I liked about this facility. They were always on top of it when I called, especially during COVID when I was not able to go see him. I cannot say enough good thing about the facility, the staff members were very caring with their residents. I will say that some of the younger staff members might need some more education on how to handle residents. My overall experience here was very good.

Absolutely beautiful. Clean, light, bright, and very well decorated. I'm giving a 4 only be cause I didn't see or taste the food. If it is even half as good as the building looked it's probably fantastic. I will say that the daily options looked great!

Oak Leaf Manor North is a beautiful assisted living facility. It appears to be a top-of-the-line center. All areas I observed during my tour were newly renovated. Many activities are offered as well as services. Everything thing you could ever desire to have in an assisted living home is included in the monthly cost. I’m sure any resident would be very happy there.

This one is an option when we are ready - we liked the private rooms and atmosphere. They have snacks and sandwiches available in the evening which is a good option if resident does not feel like eating dinner.

When my Mom’s needs changed, we had to find a new place for her to live. We found this place and it’s been going really well. The staff has been very helpful and they’re very understanding of who she is as a person and what her needs are. I really like the facility and she enjoys the activities. They provide a calendar each month for the activities for anyone who wants to participate. The building itself is not as big as some other facilities, but it’s very nice and cozy. Everyone knows her by name, even the downstairs business staff, which surprises me. I really don’t have any complaints and I would recommend them.

So far, our experience with Oak Leaf Manor has been a good one. The staff seems very efficient with what they do, and the really do care about the residents. The food looks very nutritious, and they seem willing to make accommodations to the residents dietary needs. When I visit, there always seems to be activities going on as well. I'm really impressed with this lively, comfortable community.

This community is great. They communicate with me very well. They know my sister on a personal level. They offer physical therapy for her. [Removed] is always on top of the medical issues. The lady who answers the phone is very lovely and responsive when I call she'll get me to the right person. The caregivers are nice and attentive. The food is good and she goes to the dining area 3x a day. I feel like we are getting our money's worth they do everything for her. The only downside to this community is no nursing staff. I would recommend this to my friends and family.
