We were moving him from another facility and they were wonderful in helping us expedite that whole process. They coached us on how to get him out of another facility and were just super helpful. They went above and beyond to help us give him over there. Unfortunately he was not here for long, but we had high hopes that this was going to be a better place for him.

DO NOT SEND YOUR LOVED ONES HERE! This place has a new director who does not care about residents, employees, safety, or basic human decency! My mom would go hours without begin changed or turned (despite family and friends being with her around the clock, requesting said care! Her palliative meds to keep her comfortable were given HOURS late sometimes, again despite multiple requests from family and friends! The director blatantly disregards voiced concerns for safety and well being- even firing the few GOOD staff - for bringing negligence to her attention, they frequently go weeks without a nurse on staff (illegal, point blank) causing medications to be withheld and hospice orders to be ignored for days. Cannot keep any staff longer than a few days to weeks due to poor working environment. This place is GROSSLY NEGLIGENT, UNSAFE, and inhumane!

Colonial oaks was a good match for my husband. They were patient with him as he was a combative patient. The facility was clean and newly decorated.

We enjoy all the activities they have to offer, there is always something going on for the residents. They have different readings where the residents can listen to different story books, there are church services and different board games. Everything is pretty easily accessible, the hallways are shorter and residents don't have to walk to far. Based on our experience so far we would recommend this community to others

Administration helped us with a very difficult move for the family. They were very kind and compassionate. The current staff including nurses,and med aids are very professional and easy to work with. The staff is very kind to the residents and have an upbeat, positive attitude towards the residents.

The staff here were wonderful. They are caring and helpful here. I would highly recommend this community to others for their loved ones. The community isn't the fanciest but my loved one was very well cared for here.

My mom has been a resident here for almost 2 years. Their staff is so kind and caring. I never worried for her well-being during the lockdown in 2020 for COVID. I knew she was in good hands. The aides are wonderful with her and always have a smile on their face when I visit. [Name removed], the Resident RN, is so good with her and always responds quickly when I have questions or concerns. I frequently get pictures of my mom when they have events and fun activities. [Name removed], the activities director, is wonderful! She always plans fun activities and kept us in touch. I would recommend this place to anyone in need of memory care for a loved one! Again I can't stress how wonderful the aides are- they truly love the residents they take care of!

We cannot get in to see her so these ratings and review are based off our experience during covid. We Colonial Oaks mainly because it was closer to family and we thought we would be able to visit and we were able to get them down on price. They have been pretty good about communication and send out email to let people know what is happening inside the facility. The activities director is great, my mom really likes her and they have a good relationship. She gets them motivated to participate in activities. They also have a Facebook page where they upload videos and pictures so we are able to see what they are doing. Overall, I would recommend based on our experience so far.

This worked for our family and was a good community. This was better since they specialize in memory care. The staff was good to work, with sometimes the response time was lacking. This is something that the community can work on. The phone would ring more than I thought it should. The care for my loved one was very good.

May 15, 2023Michael, thank you for taking the time to leave a review. I want to ensure the phone system has been replaced and phones no longer ring for long periods of time. We have recently had a management change and want to ensure every resident and family in our community is KNOWN by heart.
Look at the reviews and watch the decline over time. My Mom was there for almost 4 years. We had a good experience at first, but staff turnover was depressing. I don’t think I would choose this place today, although COVID impairs my objectivity. My experience with the director was not positive, in contrast to earlier directors. Families should be aware that the contract requires payment for 15 days beyond death. This policy is exploitative IMO. If they require more money, raise the rate, don’t charge room and board to dead people.

This was to be a 3-way FaceTime video tour with my brother. After not having received a call after our agreed-upon time, I contacted the facility and was told that [name removed] was on the phone with my brother and had just finished up. So I was told that [name removed] would call me back, which she did immediately. I received my tour, actually was impressed with the layout of the premises and how they had labels for each section for their residents. I had a general sense that [name removed] did not have the experience that I felt with other facilities' care managers. She answered my questions, to be sure, but I just didn't feel the level of expertise that I was hoping for. Also, not a sleight against the facility but it is a bit farther than where we were wanting to place Dad.

We highly recommend this facility. 5 of us in our family looked into a lot of dementia care places for grandmother, looking at websites, called for pricing, virtual tour. It was exhausting! But Autumn Leaves stood because they were more interested in learning about my grandma that just trying to like sell us on their facility. And between their price and how beautiful it is there, we moved her in and are so pleased we did. Their activities are upbeat and cover different areas to help improve skills (not like some places that sit residents in front of a movie) and their restaurant-like dining and the fact they have plenty of caregivers has made us grateful that she is safe and well loved. If you are looking for a place for your family member you should check them out.

My family looked into this facility because my niece actually works there. We didn't think we could move my Mom in due to the Coronavirus thing. But [name removed] in admission was very friendly and we got a virtual tour. It's clean and you can tell the people living there are happy. Our niece is super happy working there so we decided to forward and move out Mom in there. I really recommend this place. A lot of the other ones in the area are nearly as nice.

I moved my wife in Autumn Leaves 8 months ago. Since that time I have seen my wife decline and need more and more assistance. Before COVID I visited my wife every day and we had lunch together, anytime I asked a nurse for assistance someone was there to help. The staff is very attentive and eager to please. The facility is clean and always smells fresh. I would recommend Autumn Leaves in Tulsa to anyone needing to move a love one into a memory care.

I love Autumn Leaves Tulsa and the care they give my AUNT [name removed]! From the minute she was placed there it has been comforting. She has been treated kind and with respect. The workers have done a great job and the facility is clean, calm and always having some kind of activity with the patients. Its a safe place for the people who need help. I can't say enough good about Tulsa Autumn Leaves staff. Way to go! The [name removed] family!

My mother was here for just over 3 years. The first 18 months was mostly good. The staff was consistent and when you had an issue it would be addressed. But the last 18 months the turn over among ALL the staff was constant. There have been 3 directors, 4 nurses, 3-4 activity directors and all the CNA's have changed and often. They went months without a nurse at all. The latest director has been the worst director so far. We attempted to get help from corporate many times to no avail. The care was so bad we decided to put in the 30 notice and move mom to a different facility. Prior to the 30 days being up, she had a bed sore so bad we took her to the hospital where she stayed 3 days. She was in such bad shape, we did not feel we could take her back to Autumn Leaves. She passed away 7 days later. Per the contract, since she passed, we should be reimbursed for 2 days of care. But they refuse to reimburse us now. After 39 months they want to refuse 2 days.

May 15, 2023I am sorry for the experience you had with the previous company. We are currently under new management. We have changed the culture to ensure everyone in our community is KNOWN by heart.
This place is terrible! We had hoped it would be a good place for our mom, but the care she received in just two weeks was horrendous! The staff (even the head LPN) was caught in multiple lies about bruises and injuries we discovered. Staff prefers to medicate residents, so they are easy to handle. We moved her out after two weeks because we honestly feared for her safety. The food is disgusting and the staff doesn’t care If residents eat. Residents weren’t allowed to have butter knives and the food wasn’t even served in bite sizes, so how are they supposed to cut their food? We saw residents inappropriately groping other residents with staff no where to be found (even though there are cameras). The layout is a bit of a maze and saw multiple residents fall. We had to inform staff because no one was around. We were also guaranteed a refund because of the multiple incidents that occurred, and it’s been almost a year & continue to get the runaround. Please don’t put your relative here!
