The place is beautiful and peaceful. Clean. They are very short staffed. We had to break my mother out of the place as they left her sitting in a chair for 16 hours without any attention. They didn't feed her take her to the potty nothing. We do not recommend the assisted living with this facility. No staff. It was crazies running the looney bin. When we found her in this state she had desperation in her eyes and she smelled. There were no attendants around and I had to continually go to the other side to get help.

I toured this community and it was great. Clean atmosphere, friendly, great tour!
I toured this community and it was nice. Clean community and nice staff. They had a lot of extras that could be great for some, but wasn't the best fit for my loved one.
We had a resident who was fixated on our loved one. She unplugged her lights, tv, and modem. She stole pictures off of her walls, stole her belongings, and took her hearing aids. She eventually threw water at a family member who was visiting our mom and presented our mom with her poop. Despite this being in direct violation of the code of conduct explained in the contract, they did NOTHING. The Management team explained all of the reasons that this was happening, but was not willing or able to solve the problem. A male resident showed up in our mom's bed and wouldn't leave. A lot of explanations about why it happened but no solutions for how to solve it. How did Charter Senior Living of Oak Openings respond to these problems? They discharged our loved one! Yes, they were cited by the Ohio Dept of Health, but the stress that they placed on our family during an already difficult time was unimaginable. Having a loved one go to a memory care facility is stressful, but they made it worse

If your loved one needs Memory Care, I can no longer recommend. Care has drastically deteriorated since [date removed] and especially in memory care. Turnover for chef resulting in cardboard like pizza, burnt hot dogs, kool-laid, poor resident to caregiver ratio , shortcuts on cleaning (dirty toilets with mold rings!)! 3 inquiries in less than 6 months with Ohio ombudsman, and lack of mgmt accountability.

I struggled with the hardest decision of my life when my wife of 50 years reached end stage dementia and I started exploring possible memory care communities. My sons and I visited many in our area and narrowed it down to 2 facilities. But in the end it was the "feeling" we all got after our visits to Charter Senior Living in Sylvania. The marketing staff helped us to feel confident and comfortable with this tough decision. Everyone we met exemplified the "team/family attitude" and were very personable and professional in their greetings and discussions. My wife had challenges when she first entered the community but with the consist caring of all the staff members, she became comfortable and is enjoying her new home. The staff at Charter truly cares about the residents and their families.

This was a lovely community. We didn't need a secure unit for my loved one.
Our family had an in person tour and it was an excellent tour experience
We were overly impressed with this community and what all they do for the residents. This community was fairly new and was very nice and clean, there were no smells and we just had a really good feeling that our loved one was being well cared for just like somewhere we would go ourselves. It was a very difficult time but this community I would go myself so I felt safe sending my loved one there.

Charter Senior Living is a very good place. The program director here went above and beyond and tried so hard with my mom and we really appreciated her. We were extremely pleased with our experience and have nothing but good things to say about the facility.
We did not have a good experience here. She had a lovely apartment but overnight they told me she immediately had to move into a memory care shared room that was much more expensive but much smaller, so that was the last straw for us. We moved her out and she is at a new facility.
We really liked it here. Their food was excellent. The staff was really friendly. It would have been 2nd choice for us. It was really clean, very modern, and I liked that it was a smaller community. The rooms were very beautiful. If I were looking for a place for myself, that’s where I would want to go.
We haven't been able to go back and visit since March due to COVID. We initially had some reservations about our loved one fitting in, as well as figuring out the amount of memory care she would need. At this point today we are pleased that we put her into memory care versus assisted living. Our loved one has rented what would be two rooms, so for one person it is like a double room which is good. She has a little living room and a bedroom that are separated by a wall. The single spaces in memory care are more like studio apartments. The rooms that are provided on the assisted living side were more of what we were looking for, but overall we are happy. When our loved one moved in at the end of February she really enjoyed the food. In June she had a couple of falls where she spent several weeks in a skilled nursing facility. While she was there she has to be isolated where there was no community dining, she had to eat in her room by herself. During those weeks she had lost some weight. Once she came back to Charter community she was able to put back on that weight she had initially lost, which we were thrilled about.
