Here, residents receive consistent care assignments, meaning they’ll be able to build a long-term relationship and develop familiarity with their caregiver. Residents can also feel safe because staff is available 24 hours a day, seven days a week to be alerted to an emergency. A 24-hour security system and keypad entries are also incorporated.
The memory care programs at Brookdale Plymouth provide residents with a person-centered approach to care. Through the Clare Bridge program, residents participate in six activities a day, which can range between brain games, music programs, fitness classes, walking programs, dancing, and more. The staff is excellent. The facility is very nice. We did not rate a 5 because we see there could be improvement in having more organized or structured activities for the residents. Our family member states he is often bored.

I would tell others that they will always take the time to talk to me when I have a question or concern. They also will look in to something and get back to me as needed. The room that my loved one has is a very comfortable size for the needs. I would recommend Brookdale Plymouth to others.

Current staffing shortages are affecting the quality of care. The location of the facility is good because we can visit frequently, but communication with the family has been poor since both the Executive Director and Sales Manager left. It is starting to have the stench of a nursing home/urine- never was like that early on. Would I recommend this facility to others? I'm not sure. It has pluses and minuses, and hard to say what I would recommend. I guess I probably would, with reservation. It's a lot of money and once you are there, you can't leave because you have to private pay for 2 years.

This was a good community and we almost choose this one. The residents seemed happy here. The bathing situation for the residents would have been in a community area.
It was a good facility overall. Just make sure that you stay on top of your loved one's care to make sure they're getting the proper care. When our family came to the staff with concerns, they did respond appropriately but not above and beyond.

We moved in in Aug and the office was great following up and then the executive director and a lot of other staff walked out of the center. The other staff tried to pick up the pieces from those staff leaving and it wasn't working well. My loved one did not respond well to the no activity and it made him anxious. It was supposed to be secure and he got out of the community through an emergency exit. I feel like the staff and those that are there have done the best that they can do but they are struggling.

I really liked Brookdale Plymouth. I liked the director, who spent time with me and took me on a tour. I could see the activities director rounding up the residents for activities. He was very outgoing and included everyone. And I saw him bringing everyone back in wheelchairs and with walkers. I was really impressed with him! I chose a place that was closer in proximity to where I live and therefore easier to visit. Hindsight is 20/20, but I wish I had based my decision on other more important aspects of a community.

They normally have everything clean all of the time, my loved ones bathroom looked like it needed some up keep but normally things are clean and well.

This is a good place to put someone. It is a smaller community, and only deals with memory care issues. The staff is caring.

Brookdale was terrible. My dad was there for 1 week and had to be taken to the hospital for being over medicated and severely dehydrated. He was filthy,and so was his room. He had fecal matter under his nails and on the wall by his bed. He hadn't been shaved and there was food in his beard. My dad never recovered from the dehydration and died a little over a week later. Brookdale is a terrible place!!!!

Brookdale definitely over-promised services. The best thing I can say about Brookdale Plymouth is that the staff is very friendly and smile a lot. The Activities Director is also fabulous and my mom really likes him. However, my mom doesn't understand most of the rest of the staff because of the language barrier (a lot of the staff have thick accents and I myself even have a hard time understanding what they are telling me). The thing that bothers me the most about Brookdale Plymouth is that my mom is paying $461/month extra for dressing/grooming assistance, but she rarely gets assistance. Mind you my mom tells the staff that she doesn't need assistance, but this is not the case. I was told by the Director that the staff are trained to "re-approach" when encountering resistance. I just don't see that happening at all with my mom's care. For example, I've noticed that my mom wears the same dirty clothes for days in row. Where is the "assistance" from the staff in getting her to change her clothes? I also have noticed that my mom sleeps in her clothes many nights. Again, where is the assistance from the staff in getting my mom into her pajamas? Same thing with the showers. My mom pays $173/month extra for assistance with bathing two times a week. After two months I learned that my mom had only taken 1 bath since arriving. (I have no idea why I wasn't informed of this sooner.) I learned that my mom was refusing the shower and that the staff were unable to get my mom to take one. The solution was for me to be there when it's a bath/shower night. Another very bad issue is the cleaning of the rooms (or lack there-of) There is very little cleaning done (if any) of my mom's room. I wind up cleaning and vacuuming once a week. The Director is very nice but appears to constantly give me lip service ("that shouldn't have happened"; "we are retraining/re-educating our staff", etc.). My mom's clothes and bed linens often get lost. (2 sets of sheets have disappeared). From what I understand, this is common at all memory care facilities. Brookdale promised me that I could be the "daughter" again (instead of my mom's caretaker). Unfortunately that hasn't happened. I launder my mom's clothes and bed linens, I am the one to convince my mom to take a shower, I am the one who is cleaning my mom's room, etc.. I still feel like the care-taker. Having provided mostly negative feedback above, I can say that I feel my mom is safe at Brookdale Plymouth and that the staff is always willing to listen to my complaints. I have definitely had to adjust my expectations. One other good thing about Brookdale is that they allow my mom to have her cat as long as I keep the litterbox cleaned and take care of the cat. This is one of the reasons I chose Brookdale over many other places which don't allow pets.

My relative has lived at this facility over a year, and I visit her at least once a month. There is usually dried feces on her toilet seat, and I often see the same scraps of paper on the floor that were there during my previous visit, meaning the housekeeping is pretty bad. I visited last week, and there was no toilet paper in the bathroom nor any towels. At lunch, the help just put the food in front of the residents with a plop, then picked up the plates when they were done. The food wan't cut up for those needing it, so they ate with their fingers, and coffee wasn't poured until the residents yelled. Otherwise, the staff just stood around.

My wife was there for about 9 days. 3 days in, she fell. On the 9th day, she fell in the same spot and broke her leg and hasn’t been able to walk since. I guess the staff was okay, I think if the fall hadn’t taken place then it might’ve been alright. But ever since then, it’s all been downhill. The cleanliness of the facility was fine. However, after 9 days of her being there she still had on the same under clothes that she went in with. I don’t think anything had been cleaned the entire time she was there.

I had a tour and got to see the atmosphere of the community. It is an all memory care and they are a great facility. My mother was just not like the other residents, and since she was not at their level it was not going to be a good fit for her.
