Boulder Creek Assisted Living and Memory Care

Boulder Creek is an excellent facility. While my Mom was only there for a short time, she received very good care. The Administration and Staff were very kind and helpful. While there are always a few hiccups and learning moments throughout the process, everything went smoothly and I was very pleased. I would definitely recommend Boulder Creek to those in need of Assisted Living or Memory Care.

They try very hard to ensure a person settles in their room. If there is a concern it is fixed in a timely fashion. Need to have more variety, fresh fruit and vegetables, and plate the food so it doesn't look so drab. Food is usually served cold when the item is intended to be hot. The activities don't change drab and my mom finds them boring. Not a lot of resident choice engagement, ie cooking. Mostly tapping the balloon, bingo, and travel dvd. How about let residents do a show and tell about their life or hobby.

I think it is the best in the area compared to what I have heard about other places. It is closest to me and has a good reputation.

Care staff is excellent - helpful, kind, respectful, responsive. Peaceful, positive atmosphere. Care staff & leaders promptly address concerns. Activities are a highlight - especially exercise. It is fun and beneficial for mood too.

What a wonderfully caring community! The staff is so thoughtful, helpful, and kind ... my mom has felt very at home since the day she moved in! So many activities have helped her connect with other residents.

The caregivers need to understand dementia better, even with a high functioning person. You can't really as would you question, but rather that let's do this etc. There is a new cook, so things are improving. The activities person works hard to engage everyone. There need to be more variety and less changing of the times. The residents can't understand why a specific activity time keeps changing. The price seems a little high when comparing to the previous place, especially the meals/snacks that are offered. Overall, my mom is happy and the family is pleased. They need to improve a lot on communication, as there have been many days and weekends that no one answers a phone. When the phone then goes to trying to locate a person you are hung up on. My experience has been varied whether staffing is strained or not.

The facility is very nice and cozy.Meals are great!Activities by [name removed] and [name removed] are great. [name removed] at the front desk is friendly,some of the care helpers are fantastic & some didn't change my moms diaper/clothes while she was at the end of her life & couldn't speak for herself.The biggest problem I had is with the Executive Director.I needed papers filled out for the VA so mom could collect benefits & it took 2 months for her to fill them out.I was lied to several times stating they have been sent in to the VA, when I was in her office and saw them still on her desk.We asked for a larger room & was told we would get room #55 when available. In the meantime we purchased furniture from the prior resident.The facility gave the room to someone else. I talked to the director & asked for her superior & called her about my issues. She didn't even have the courtesy to call me back.I insisted they hold my furniture till another room was available. When it was they lost some tables.

I really like it here.

I am very comfortable having my mom here. I know she is safe and also well taken care of.

This place has such a warm home-like atmosphere! The staff make this place somewhere you'd want your loved one to live. From the activities to the care, it is truly a gem north of GR located in the heart of Rockford.
