Everything is a 5, except for maybe the meals, 4 since they lost their long time chef (still really good in comparison to the other 2 places they've been in!). The staff is lovely, the place is super clean (I think it's the only place in town with no current Covid outbreak!). They have been wonderful helping my Dad adjust. Lots of activities if one chooses. I would highly recommend Heritage to anyone!

They seem to be doing a pretty good job. The staff is pretty attentive, and they keep it clean. She is getting very good care, and a lot of attention. I don't get complaints about food, and when we go in it smells good. It's still pretty new, but so far, it's working out.

Assisted Living

Assisted living communities administer social events, oversight, and healthcare to individuals who are about 65 years of age or older. Assisted living communities commonly provide care supervision, assistance with appointments and transportation, and support with the activities of daily life. Under certain circumstances, communities may offer continuing care, hospice, or dementia care. Wisconsin maintains specific standards related to this type of community. The Wisconsin Department of Health Services, Bureau of Assisted Living, Division of Quality Assurance, regulates community-based residential communities (CBRFs) regulates this type of community. Staff must receive training in a variety of areas. These areas include standard precautions, medication management, needs assessment of prospective residents, and provision of personal care. This community type has a number of provisions pertaining to medication assistance. These provisions include Residents may self-administer medications if they are capable, If the medication administration is supervised by a licensed healthcare professionall, the community must ensure that these individuals coordinate the medication administration process and system, and when medication administration is not supervised by an RN, practitioner, or pharmacist, the CBRF must arrange for a pharmacist to package and label a resident’s prescription medications in unit dose. There must be one bath or shower for every 1 residents and one toilet for every 1 residents. Public assistance is available through The Elderly and Physically Disabled 1915(c) Waiver program, the Community Options 1915(c) Waiver program, and the Medicaid managed care 1915(b)(c) Family Care Waiver program for the aged and physically disabled. Before a resident is accepted, they must go through a service plan screening to guarantee their needs are going to be met. This screening includes a number of areas of interest such as community must develop a temporary service plan to meet immediate needs, initial comprehensive mental and physical assessment, the plan must specify which program services will be provided to meet the resident’s needs as identified by the assessment and the frequency with which each service will be provided, and reassessments must be conducted at least annually, and when residents have a change in needs, abilities, or condition. This community has a number of provisions concerning food and diet which include therapeutic diets must be provided if ordered by a physician, communities must provide at least three nutritious meals a day and a nutritious snack, these meals must meet residents special dietary needs, and these meals must meet the recommended dietary allowances based on current dietary guidelines for Americans. This type of community is required to offer a number of services. These include leisure time activities, independent living skills, information and referral, and behavior management.
