The nurses do not come when the emergency button is pressed. At one point, my mother in law waited for 40 minutes before someone came. Another time no one came because there was a server malfunction or something. This happened on at least 4 occasions during her 2 month stay there. When we found out she had fast progressing ALS and needed hospice care, we had to take her out of the facility because she was not getting the care she needed. We did not give 1 month notice because she did not have that long to live and in fact died two weeks later at her home but they will not waive the charge for the two weeks that she wasn't there because we did not give 30 days notice.

Extremely small all-indoor memory care section and activity/dining area. Few windows. Claustrophobic. View out resident's window appeared to be a cinderblock wall. I didn't get a good look but rooms seemed to be small. Questionable how fast staff were responding to resident's request for help with the bathroom. Street parking only.

Property would be better suited for a mobile resident that likes the great outdoors because the bungalows are separate from the dining room and activities. Very secluded property with large oaks and rustic buildings from the 1930s. Property is rustic with natural oak tree litter - good place for rattle snakes to hide. A person with disabilities or using a walker would find it difficult to navigate the walk ways, especially if it were raining. The bungalow we looked at the rooms were very small and odd shaped. Only a full size bed would fit and a TV would be either very close to a couch or very far from the couch. Very little hanging closet space. Closet/cabinet doors were not plum and did not close completely. The toilet paper holder was too far from the toilet and a senior would fall trying to get TP. The handicap rails were not useful because they were too far from the toilet on the side. The walk in shower was as expected, handicap compliant.

Had a beautiful setting that makes you feel like you are at a retreat. Not too big, not too small, the size of the community is just right.

What a wonderful place. The environment is really wonderful-absolutely not your typical nursing home or assisted living facility. On five acres with walking paths and cottages for those who are more independent. One thing to note is that if the patient is too far along in their dementia or Alzheimer’s disease they may not be a good fit. Patients need to have a certain level of independence to be a good fit there-but it is a special place.

Caring staff in a beautiful setting. Great living spaces and activities. My mother is thriving after living on her own for several years, and had nothing but good things to say about the care, food and facilities.

We couldn't be happier with Twelve Oaks. After a four-year closure, Twelve Oaks Senior Living, formerly known as Twelve Oaks Lodge, is open!I More than $1.5 million has been spent improving the property, which sits on nearly five acres of gorgeous park-like grounds amid towering oak trees. Community amenities include a hair salon, library and daily activities. Landscaped grounds with walking paths, a large outdoor patio shaded by oak trees, gardening beds, the original rose garden, and oak and fruit trees all provide a peaceful and tranquil environment for residents to socialize and to walk their pets. What residents and their families like the most is the value. Twelve Oaks is one of the most affordable assisted living facilities in the San Gabriel Valley. Starting at $2,800 per month for a studio unit, residents will enjoy three restaurant-style meals per day, optional additional services, daily activities, a full-time nurse, a call system, 24-hour staffing, weekly housekeeping, laundry, and cable TV. I'd highly recommend this location for those looking for a tranquil, affordable and community-centric facility.

Beast. The greedy folk at the board level managing a charitable trust as a non-profit decided to no longer do what was en-trusted for them to do and that was operate this community facility. So many people of the community supported this facility to make it a great place for our parents to have care now that they are having difficulty. It is really sad because My Mom fit into that place as a life long Glendale Pioneer it was so perfect. These people thought they could cut a fat hog and run away with a hefty profit. We must expose the flaw in their nature and keep alive the philanthropic spirit of the Fifields Family, and The National Charity League who donated this Gem so seniors could have a peaceful respite in their days of need. Shame on you Be.Group under the disguise of Presbyterian Homes.
