1) Management doesnt respond to concerns unless it's in their interest. Does not respond to emails or phone calls, however lower level personnel usually respond. 2) Management appears to prioritize financial objectives over the best interest of the occupant. 3) The Memory Care Unit is horrible. It desperately needs to be updated with newer furniture as the existing decor looks like it was procured from a thrift store. 3) MC staff exhibited minimal interaction with the residents. Usually one person sitting in the back of the room telling the residents to stay seated - I observed no activities during my visits and multiple people asleep with their heads on pillows seated at the kitchen table. 4) I had multiple visits to the MC unit where there was no staff present to oversee the residents. 5) My mother was basically unsupervised while in the AL portion of the facility at night. This makes no sense since the initial objective of this arrangement was mitigate the fall risk. Not sure I understand unless the objective was to either get rid of her because they couldn't meet her needs or they wanted to keep her monthly payments coming. 6) Absolutely no security in this place. The front desk is a joke -anyone can walk in and have the run of the place. 7) No effort to coordinate with the various caregivers (Nurse, PT, occupational therapist, etc) to formulate a care plan for those that need it. When I asked about this no satisfactory answer except awesome people want to make us look bad and you need to utilize our caregivers 8) Management and most of the staff will go out of their way to avoid you during a visit - strange! This facility can be so much more than it is - what a shame. We now have my mother in facility where they put the residents first - what a change.
I have to be honest - the initial experience was amazing - [name removed] (sales) was beyond kind & great at sales. The location, Rooms & balcony view was amazing. Beautiful landscape, and facility. Our Dad was happy to go there. And we thought it was the perfect location for him & his cat. However, after time went on, it became a significant challenge. Billing was often incorrect, made excuses for random charges not applicable, extra fees assigned. Shuttle was seldom available, no driver, or broken down & my sister consistently had to take time off work to provide a ride. Medication management charges kept changing & seldom did the clinical staff follow up with his physician. One day he left the building with another resident (we are aware in FL, that the residents legally are allowed to go out on their own being in ASL unless in SNF. They are to sign out & back in when doing so. However, no staff at the front desk means no one had a clue he or the others left or returned. Which he had been gone for several hours & none of the group he went with signed in or out! No one had a clue at meal time they were not back. This item was escalated & no return calls occurred until we escalated to the corporate office & even then dis not get a call back for a week! I've worked in health care for 36 years & the fact of not even knowing if they returned is beyond disturbing. FL law or not. The staff(s) after about two months of his residence, started resigning. The front desk was serving meals, medication management was delayed & at one point they "doubled up on medications" because they did not have enough clinical staff to distribute the medications at their assigned intervals. Honestly, I could go on. This was an experience where the pretty landscape, facility & packaging as they say absolutely did not represent the product. I apologize for the lengthy response but I didn't feel my reply to the survey only & answering with a number response would accurately represent what is really going on at this location.

My daughter says that the facility smelled like urine! The staff seemed nice. My sister didn't like the food very much. She didn't stay long enough to experience any activities that might have been offered. That is about all I can say.

We are very impressed with the caring and compassion of the staff and their interest in doing their best. The activities are excellent. Even though our mother does not participate in all of them, we can tell they are well thought out. We receive a schedule every month so we can encourage her. We have attended some activities with her and the staff is impressive. I would recommend them to others.

I moved my mom in about six months ago. She's very happy there and has a nice room. Have had a couple of maintenance issues. I wonder how things would be if I didn't live in town and check in on her weekly. That's the only thing - you have to check in, otherwise the staff are quite nice and back area by the lake is quite beautiful.

It was not a good fit for my mother due to her cognitive state and she only stayed 4-5 weeks as a result. That being said I thought it was a great community. The food was good, activities were planned very well. I was very pleased with it.

The facility was beautiful and the staff was also, but the food was horrible and the price was crazy. I know it's not the chefs fault, they have to work with what they have, but it was terrible. I had to order Uber Eats half the month and it was just way too expensive

I rated everything except for Care Services/Quality of Care at a 5 (great). As for the Care Services/Quality of Care, sometimes it is difficult to get ahold of someone there and follow through with appointments can be difficult.

I like this place. I like the general feel of it. Casual and for the most part caring. I think they could elevate the food a bit. And tighten up the management. My mother complains but not in a nasty way. My mom is an entitled 94 year old woman who has been independent for 40 years. This is a new experience for her. I feel fortunate that we found this place. She has a balcony that looks over a lake. I wish she would be more social. We looked at many different places. When they showed us the room at Colonnade, room with a view, it was the best on all points.

Everything is great, but the communication with the family has been poor recently. Finally today [name removed], the Director of resident care, called me and answered my 3 questions. It shouldn't have taken two weeks, but I'm glad that she finally called. It used to be that the communication with the family was very good.

Do to a staffing issue, another member of management did the tour for us. Certainly adequate and informative nonetheless. The outside grounds were nice, overall activity rooms and dining areas looked adequate as well. Only problem was that there were no available rooms at this time to meet our needs, only small units were available. Otherwise, would have inquired more about faculty.

The Colonnade was a godsend for us. My mother was well cared for by the staff and happy to be there. Mom had a beautiful end in hospice after only 8 days of enrollment. The Colonnade nurse, [name removed], helped guide me in choosing a hospice, something I am grateful for. We choose Accent and were pleased with Mom's final days in their care.

Beautiful facility & grounds. Lots of windows, light, many rooms have fully screened in patios with views. Staff in general very kind. Offer shuttle services for appts & activities. Only down side (after the fact) was lack of coordination with sales staff, medical staff, & activity staff. Shared that this was available but after the "sale" part was done, none of the other care/or specific staff were introduced or had the opportunity to sit & understand the process, communication, or otherwise. Many options just a lack of coordination. It's been a challenge to obtain this information. And a-lot of work on the family that should occur up front. Also should be noted - this location is a "private" facility meaning if your loved one is eligible for aid & assistance they don't take it. So use the medication assistance & other medical services as that part is covered. They also offer in house physicians, podiatry etc. But Aid/Assistance does not go towards rent. In general our family member is happy & made friends quickly. The beautiful grounds adds to positive mental health.

The staff that I have interacted with have been helpful and friendly and determined that my mother have a good experience. The communication between the community and the family I found to be less than adequate. It would be really helpful to know when housekeeping comes in, when laundry day is and how we stay on top of these things, so my mother is not inconvenienced and family members don't find themselves doing laundry and changing the bed. I have not had the food, but my understanding is that it is acceptable quality but not much variety. The move in process was easy. Paperwork was uncomplicated. The staff should hold orientations with new families, explaining all the ins and outs and the best way to make the most of their family members stay at the residence. I would recommend this facility, it offers fair pricing for the level of care.

The family like atmosphere, the ambiance as soon as you go in the door. We also love the friendliness and helpfulness of the staff. I witnessed a resident fall in the dining room and the response from the staff was outstanding. That makes me feel comfortable. I am a retired nurse. I am impressed with the nursing supervisor and the marketing person ( their names have escaped me). I am especially impressed with the receptionist. She knows everyone and she also possesses a customer service mentality that caters to the residents and family. She is the main "go to" person for everything and she is amazing in her ability to keep track of everything.

My mother had to be placed there after being in the hospital and refusing to go into assisted living. It was kind of an emergency, and she had to be placed in memory care. Everyone involved could not have been more helpful. They were amazing. My mother lives in Florida by herself, and me and my brother and sister live in the Chicago area. Her move was quick and very easy. The process was explained to us, the staff at The Colonnade always takes pictures and will send them off to us or post them on Facebook. All three of us have gone down there to visit Mom, and The Colonnade is clean, friendly, and extremely helpful. The staff are excellent. We could not be more happy.
