Generally things are going well. The piece of mind I have is tremendous. They could use a full time driver to take residents to appointments and a wider variety of activities. It is expensive but not as much as other facilities we toured. My mom does not need any assistance currently, but when she does, it will be costly. Location is convenient for family to be there every day. However, recent charge of [fee removed] per month for room trash removal is not appreciated when cleaning is included in rent. Following family instructions are not followed. Instructions and following protocol for Having resident ready for pick up for appointments, etc is nonexistent. Laundry often missing. Wait time for assistance can be from 1 hour to no show.

I most like the friendly and dedicated staff and the nice facilities. I like least that the staffing levels seem too lean in Memory Care at times to support all the resident's needs in a timely manner.

Sage is in my community where I live. I've heard a lot of negative things said about this place especially during the pandemic. They have new owners now and we have been very pleased so far especially with the good people that work there. It is a beautiful facility atop a mountain over looking the Conejo Valley. Food is good and activities are good too.

I have to say I give everything a 5 so far. I am extremely pleased with them and the only issue is they need to hire a driver to take the residents places since their old one quit. Sage Mountain Senior Living is an absolutely beautiful facility. For those still active, I think that they need to have more exercise or activities to keep those individuals busy. I think they were getting a new activities director or maybe there are more things to do now though. The musical entertainment for special occasions was great. I would recommend Sage Mountain to others who are looking.

The caregivers are bad. The showers are bad. The med techs are bad. The laundry is bad. The nurse is bad. No driver full time. No phone service in room. Small furniture only will fit in. The average wait for a caregiver is over 2 hours. Overall rating is one star.

As a team, we recognize that there are improvements and enhancements that can be made to our community to make sure it's a wonderful home for residents. We also have a new Executive Director, who started in late July. We hope that our changes will help earn back your trust.
Sage Mountain was beautiful, and very hotel like. And it is fairly new. But it is huge and way out of our price range. The tour guide was great.

Sage Mountain is a very clean community with excellent care services. The director on the memory care side really showed off her deep knowledge and care taking skills for those who struggle with mental decline. She really understood all the different ways to care for the residents as individuals. The end of life process at Sage Mountain was very difficult, but everyone there was amazing. They handling everything in such a professional manor and we are hands down just very thankful for them. The meals and dining service is beyond 5 stars, it was like hotel gourmet absolutely wonderful quality food. They have activities, but given the circumstances with covid and trying to build of there level of staffing sometimes it was a struggle but the activities director still kept all the residents as engaged as possible. The situation is out of their hands, but they continued to make the most of it. Sage Mountain is a beautiful facility, with uplifting staff members, and a great value for the money.

Beautiful place and great uplifting atmosphere. When I was looking for the right place for my mom I noticed how the people there were smiling and overall happy . We decided on Sage because of the people who work there and live there ! My mom has been in memory care for 5 weeks amd I am very happy with the staff and care she is getting . They have really good food !! The staff listens to your questions and needs . [name removed] in sales is so kind and knowledgeable . [name removed] in the moving team attention to detail was amazing . The memory care unit truly cares about each resident . My mom has improved so much and she eats better and smiles ! I am very happy with my decision for my mother at Sage !

My dad has been here for over a year. When COVID happened they didn’t get it until January and worked hard to Keep it out. Everywhere had COVID. It is a pandemic. I thank the care staff who do their best to care for my dad and make sure he is safe. They’ve had management turnover but the heart of the community is their staff. I read the other reviews and felt like I had to say how wonderful the staff are. This isn’t an easy time and they are doing their best. Thank you Sage Mountain staff.

Sage Mtn is a beautifully kept property, wonderful setting, peaceful, and very calming. This was my mom's favorite choice after visiting other AL locations in the area. She felt the good energy and, after 4 days of living there, she is very happy and, I am VERY happy too !!

This place looks beautiful on the outside but once you go inside it's totally different. My father was there for 2.5 years. Huge turn over in management . Quality of care is a zero. Always understaffed, but they deny it. Always a different sorry when a caregiver calls vs what management says. I called corporate twice in regards to their management and how things go there. Always going to do better . If you dont want your family lives to be shortened, dont take them there. Over price for the quality of care and the food is horrendous. COVID is rampant there . Staff and residents. Then they lie to you about that.

Staffing in particular has and continues to be a challenge for all assisted living nationwide through COVID and even today, but we are thankful for a core group of staff who have continued to care for our residents as we pull out of the pandemic together. In the two years since Sage Mountain opened, they have been through 4 managers, a corporation purchase, heavy staff turnover, and several cooks. The communication to residents and families is horrible. My parents complain about the food -- definitely not "gourmet" as advertised. Weekly amenities such as house cleaning and laundry are not consistently done (or not done at all). My parents are paying a hefty price for services they are not receiving. Management passes off blame and does not adequately communicate policies and changes with staff or with families and residents. We had such high hopes for this facility for our parents as it is new and made lots of promises. However, we have been disappointed time and time again. Trust is important in facilities like this. Broken promises, lack of disclosures, not following policies have created big safety concerns.

We operate among the top assisted living providers and use a robust system to deliver care to all residents according to their documented needs. I would encourage you to follow us on social media to see the food coming out of the kitchen every day – we’re thankful for an excellent culinary staff and hear good things from the residents.
As of [date removed] there are 41 residents with COVID and 20 staff. They are awaiting the latest test results so this will likely go up. They were not consistently following their own COVID prevention guidelines. Often staff entered the facilities without ppe and/or removed masks while on duty. They encouraged residents to visit family during the holidays and did not consistently quarantine and test them after they returned. As a result by [date removed] the outbreak had begun. By [date removed] it had spread from memory care to assisted living when 3 residents exhibited symptoms. Still they waited 4 more days before testing everyone, all the while not telling AL residents and families that they already had 22 residents in memory care and 16 staff with COVID. They then traumatized many residents when they forced them to go to different rooms with just a bed, no personal items., no tv, and in many cases no phone and stay for 10+ days. They were crying and alone. Families could not get updates.

New community with a beautiful view (at the top of a hill). They addressed my concerns about brush fires and evacuation protocol very well. I think residents are very safe in this facility. Seem to have an excellent care system, activities and dining menu. The few residents I saw seem very happy.

From our understanding they have had no positive cases of COVID. When we were able visit and eat their with our loved the meals and dining services was very good. We would absolutely recommend this community to others.
