When we toured the memory care unit it was sparsely filled. We were told it was because of the pandemic, but later felt it was because of poor care. The Oak Creek locaton had problematic staff turn over both in care, sales and nursing. Often we couldn't find staff on the floor when we visited. They also had a theft ring preying on residents and their belongings. This was true for many residents. There was inconsistency in medication, laundry, showers and little to no housekeeping. Only the activity staff was consistent and connected to residents. Please be very careful in checking this location before placing a loved one there.

My loved one states that the food isn't that good. I don't think they clean like they should especially the bathroom of all places. They are suppose to change her bed weekly, I don't believe they do that. I believe they are probably no different than any other facility.

They adjusted the amount of food dad was given, based on his eating patterns, always making sure he had enough and on days he didn't eat, made sure he had plenty of snacks in his room to nibble on, and when his appetite increased, they increase the amount of his servings. They had wonderful musicians they brought in, and daily exercise groups, news groups, crafts, etc. when dad could no longer attend activities. They would bring him music tapes, movies, books, and even snacks, and would take the time to visit with him several times a day.

My experience with Oak Park Place has been terrific, from my first tour, through the prep & support for move-in, and to now - the current care and programs for my Mom. She loves the quiet of her apartment, the activities, she says the food is good, and the staff really are there for her when she needs help. The nurse is very helpful when we have questions about medicines, and the facility is clean and modern, with lots of room to roam. Their front desk staff is so friendly and helpful, too. I recommend that you take a look, as they also seem to have great pricing, compared to several others I looked at in the area.

My loved one is a resident at Oak Park Place and overall things have been mostly great for him at this facility. The food varies from good to very good, depending on the day. I would like to see more activities after dinner hours. However, the things that are presented are very good. Our loved one's needs versus the level of care that he is labeled seems high. But we are very grateful for those who care for him.

All of the staff are very friendly and always willing to help at Oak Park Place Oak Creek. The housekeeping and laundry service is nice. The meals are adequate, but often are not what was ordered and not served hot. Mom likes the layout of the facility and that it is a one level building. My only complaint is that there isn't easy access after hours. There isn't a key fob for residents or family, so I have to call every time I visit after hours. I understand it is a security issue, but other facilities have them.

Within the first two weeks of moving in, money disappeared from my mom's purse. I know that she had too much in her purse, but still. Overall my mom likes it there. They do a good job assimilating residents into memory care. The staff is overall great and kind to my mom. However, they regularly miss laundry days, and cleaning her apartment. They have a communication problem. For example, my mother fell twice and I was not notified. Her toilet handle was broken for days, and my husband had to fix it temporarily to get her through the weekend. Some food is served cold or barely warm and is not as satisfying as it could be.

I would recommend you check everything out thoroughly. The community has good activities for the residents to participate in, if they choose to. The medication management has been good for my loved one. I believe the community is giving one bath or showers a week. I believe they are trying their best is assisting my loved one with dressing.

My mother moved into Oak Park Place/Oak Creek March of 2020. At first, it seemed like a nice place. As time went on, I realized my mother was not getting the care we were promised and were paying for. Housekeeping was never done; laundry was not done unless I asked staff 2 or 3 times; her medication was not being managed correctly (staff were supposed to watch her take all her meds, they did not. We found pills under her chair, in the bathroom, kitchen counter, etc.) We were told there would be a full-time nurse & there was for a while, until she quit. Then just part-time nurses came in sporadically. During the week between Christmas and New Years 2021, my mother was very sick and also fell out of her chair and had a bump on her head. No one from OPP called to tell me. I was told there was no Director of Housing on site that week and that they were short-staffed. After I called an ambulance, she ended up going to the hospital w/3 infections where she died a week later.

The Director of sales, [name removed] is awesome. We were in a bind, being from out of state, and he bent over backwards to help us. The food is great, the place is clean, and the staff are so compassionate.

When we first moved in, it was awesome. They had a slight issue with staffing retention, but it did not last for long. They take good care of my loved one and the community is clean. They should improve in making sure the residents look presentable, as I have noticed my loved ones hair has not been combed. There are photos online of the residents engaging in activities, but their hair is not up to standard. My mom always had her hair done and took care of herself, so I do wish they would do that as well.

The decision to find an Assisted Living facility for my husband was not an easy one. Concern for his care was of the utmost importance to myself and family. A visit was made to Oak Park Place in Oak Creek, a brand new facility that would be able to provide the care he needed. The director with whom I spoke was very helpful, knowledgeable and always available for concerns and questions regarding our decision for placement. . The staff was efficient and friendly and always showed concern for the residents. Some of the services and activities generally offered were not immediately available when he moved in mid- January and are now available to the residents. We were fortunate to be able to have the opportunity for him to have their care during these past few months. He is now living back home. Thank you to everyone who helped make this possible.

Oak Park Place- Oak Creek is a brand new beautiful facility. They have very large rooms. They are a much better value then any of the other places we looked at and considered. My mom has been very happy here so far.
