Grand architecture and elegant interiors welcome you at Encore at Mahtomedi, where residents receive assisted living and memory care services in Mahtomedi, Minnesota. The staff prides themselves on creating a warm and supportive environment — a place where residents can strike a healthy balance between their independence and care, with customized care plans tailored to fit the personal, medical, and mental needs of residents. Licensed nurses and caregivers are available on-site 24 hours a day, offering assistance with daily living activities and other care needs.
This community currently offers all residents pet-friendly, private studio and one-bedroom apartments. The primarily private bathrooms have a variety of safety features, so residents can maintain a high level of mobility in the privacy of their own space.
For those with memory loss, specially trained caregivers are available for those with early to late-stage dementia or Alzheimer’s disease are available in the memory care unit. Unique and interactive life skill stations are scattered throughout the community to help facilitate feelings of joy and resonate past memories. I removed my mom after thirty days after several med errors, poor staffing, incompetent nurse and staff education, activity director rude and not competent with patients like my mom, one pca was verbally abrupt. My mom was a lot to handle but they seemed incapable. Even the physician who covered the place said the care especially around a meds was unacceptable. I should have reported them to the state .Unless the nurse and activity director have been replaced I would definitely NOT recommend. I saw many other care issues related to other patients I felt were unacceptable. I did like the exec director. She was brand new and trying to fix things without having to fire people .It was terrible. It was I think 2 years ago so hopefully it's improved.

Regional nurse was not personable, didn’t even know my loved one after coming face to face, was optimistic after change in nursing May/June, GREAT NURSE…. Sadly, disappointed. VP of Opps is lacking engagement, putting residents needs behind profit

It was clean and the people were nice. They got my mom checked in quickly. It was an emergency and they handled it perfectly. It was also 5 minutes from my Dads house. My mother has a difficult personality and they were able to deal with it.It was a great experience. I wish it could have lasted longer.

We toured Encore at Mahtomedi and it was okay. Our tour guide that walked us through seemed well informed. The overall staff were very friendly and informative.
My decision for not picking Encore was that they still had not done any outings, Whereas [facility name removed] takes them somewhere several times a week. That was a big deal for me for my mother... In addition it had low ceilings and was very dark inside. And I didn't see people out of their rooms. However the lady who was showing me around was lovely.
The nursing care was fragmented . The medication management was non existent, the RN was disorganized and overwhelmed, med techs did not follow protocols. Activity director was unfriendly and not encouraging. I felt the PCAs were most were inattentive and one or two were down right rude . I removed my mom after one month. The director was very nice. She is new and trying to fix things and get staffed up. Follow thru to questions was poor . I DO NOT recommend. The facility was clean. Food seemed good.

My mom Was a resident here for about a year and recently has passed. We very very grateful for the wonderful care she received. They made her feel at ease when she had to move out her home into an apt. I would definitely recommend them to care for your loved one. Thank you again for all the kindness you showed.

My step father stayed at this facility for approximately 6 months. We started looking for a new facility recently, due to ongoing concerns with his care. Unfortunately, he's now in hospice. He started out in one level of care, broke out because of lack/care of staffing, and was found on the highway where he'd walked. He was moved to the more secure level of care. We found him without proper undergarments, he would be soaked with urine, sometimes feces, and the apartment reeked. Last week, his hair was shaved off and no one could explain what happened. he blocked up his toilet, likely from flushing things that shouldn't be flushed. When my mother arrived, the room had been shop vacuumed but not shampooed, and smelled strongly of sewage. When he arrived at the hospital, he had feces crusted on to his buttocks and [removed]. He was wrapped in a blanket with urine and feces. The lack of staff, lack of care, lack of basic humanity is astonishing. I really hope that this place is shut down.

Excellent facility with great staff. Residents with higher needs in the memory care unit receive great care and all are treated with respect. My mom is very happy with her meals and activities including an inclusive music program. My mom's independence is supported to the extent her memory limitations permit. Smaller size means less to remember and ease of access to the common areas.

I almost moved my mom here. It's a nice community staffing seemed nice the location was the same as where I put my mom but something just told me not to place her here.

I am very happy with the care that my mom is getting from this great community. The staff is wonderful. We like knowing that the community is clean and maintained.The meals are outstanding and the atmosphere of the dining is great. We like knowing she is active with the other residents. We would recommend this community for the care of a loved one.

I toured this community for my father and his needs. My tour went well. It is a pretty nice community. From what I saw the community appeared to be nice, and clean. Staff was kind, and friendly. Everything went well. It was a good experience.

Overall it has been a very positive experience with Coventry of Mahtomedi. The facility is very nice and clean, and they helped to put us in touch with the care providers that we need when we need them.

My visit last week went well. The rooms were smaller than those at Cerenity & NO furnishings come with them. Also, there are only 16 rooms that aren't memory care, and so it would not be easy to get into. Common areas were very nice.

I have nothing negative to say about Coventry of Mahtomedi. The facility is beautiful! The place is wonderful, and staff is very caring. I'm satisfied with everything.

We're pleased with this community so far, it's a clean, nice looking place, and the staff are friendly. The food service is phenomenal! At the time of this review the care services leave a little to be desired, they are going through a transition to a new director of nursing. But they are definitely making the effort, and I am seeing improvement.
