I have only visited my friend once at your facility, in talking to her family, we felt [name removed] needed time to settle in before visiting her. When I visited [name removed], she seemed to be doing well. She has not been eating, but this is nothing new. The staff there seem to care for her, and they are skilled at redirecting her when visitors leave. I hope [name removed] continues to do well there.

I thought the staff was wonderful with my Dad. Was very patient and caring with him and the family. The food could have been a little bit better but all in all it was a good experience.

If a family member comes in and is a nurse in their uniform please tell the staff if they have issues with something that happened in the hospital with a family member please don't go off on the nurse that is there to visit their family member.

Although my mother was there for just a short time, I can’t say enough positive things about this assisted living facility. They are warm and welcoming. The staff is truly there to meet the needs of the clients. My mother truly enjoyed visiting with all the staff that would come in to see her. I just wish that she could’ve stayed longer!

I would say the staff is amazing although sometimes I just don't remember their names. It would be good to see maybe more name badges and the facility is VERY CLEAN. Very impressed with that! Newly updated so that's nice. The name tag thing might just need to be a in house request! And the residents are really nice too. I've met several of the ladies and absolutely love [name removed] who has taken my mom [name removed] under her wings!!

My loved one is no longer here and I would not recommend them to others looking. The nurses and the director of nursing are not good at all. My loved one fell and was on the floor - I do not think they should have ever taken her in here as a fall risk.

We moved both my parents to Colonial Oaks 5 days ago and could not be more pleased!!! Cleanliness is very big for me and I was extremely pleased when I toured the facility and it smelled and looked clean. The head of nursing [name removed], has been extremely patient and helpful in getting my parents organized with their medications and their specific care/needs. The Community Relations Director, [name removed], is so bubbly, welcoming and kind. The Executive Director, [name removed], is absolutely amazing!!! She has been so patient with my family, makes sure to check in on me and my parents needs/questions and definitely goes out of her way to make you feel like your loved ones are "home"!!! All the other staff, from office, to nurses, to maintenance, have all proven we without a doubt made the right decision by choosing Colonial Oaks!!!

I have been to visit a couple of times and everything was very clean, neat and updated. As soon as I walked in the door I was greeted and the staff have always been very friendly. The location of the facility is great for our family so we can visit often and take her out to do things. My mom is still able to get out and about which is very nice that she has that freedom still. I would recommend Colonial Oaks to others looking.

I have finally found a sense of relief! My sister and I have looked and finally found a facility that we trust. Our mother is 71 and has vascular dementia and we need a facility that could meet her needs and our standards. We found that with the staff and facility at Colonial Oaks at Spring Hill, KS. I toured this facility while it was still Blackhawk and I was not impressed with their memory care section. I know that it has recently been bought out and I am really hoping it improves. It would be nice to have a place like this out south.
You need to know about Blackhawk. The staff was very friendly and helpful. communication between the staff and our family was good. They kept us well informed and up to date. I would recommend this community to others and if i ever need senior living I would want to be here.

This facility overall has good communication, but there are times it could be better. I am concerned right now that some of the staff are not wearing masks inside the facility. Room sizes in memory care are nice, with a good layout. I love the smaller size of the memory care unit because they get more attention from the staff. I would recommend but please do your research to make sure they can meet your care needs.

I've not been into the community due to the shutdown. The residents are starting to go back to the dining room for meals. The staff was able to take things to my loved one, and they even sent me photos of my loved one. The community was able to show me several different rooms on the tour. Blackhawk is an older and a smaller community. The community is a single floor. They have different wings with the dining room in the center. I get to choose the placement of the room size for my loved one. They even painted the room for my loved one, prior to moving in. Everyone was very friendly and they help move my loved one's furniture into their room. The aides were extremely friendly. All the staff know all the resident's names. The price is all inclusive for all the care for my loved one. They had an online offer, and we were able to take advantage of the offer. The community helps get my loved one ready for any appointments. They have bingo, crafts, go out to eat (when they can) on a bus, and more for activities. The community had several different departments call me and helped evaluate and assist my loved one when after moving in.

A nice facility and was good to our Mom but whom ever evaluated her to go there did not do a good job. She was really not eligible as this was an assisted living facility and Mom needed Memory Care as she is not able to care for her self. After a couple days they realized that she could not stay there and they didn;t have the staff to care for her so we were ask to find some place else to move her. They didn;t have an opening in there Memory Care.

My Dad lived here 5 years ago and we (and he) were very happy. 3 years ago my Mom moved here. Over this period of time the staff has turned over a number of times. In the last 6 months the quality of care has dropped significantly. My Mom has dementia and needs the staff to help with a variety of personal services which they used to do. Under the current management, they no longer proactively provide this help. When we ask them about their service levels, they claim "she never asks for help". With her dementia, she doesn't have the awareness to even know she needed the help. For example, her shower was never cleaned - it was full of mold. They claimed she never asked for help cleaning. During this same period of time, they notified us that her rent was going up by 50%. The only option to avoid this (she could not afford that increase) was to accept a roommate. A recent hospital stay found Mom at a Nursing home for rehabilitation. We made the decision to permanently move her out of Blackhawk and into the Nursing Home. She is very happy now and we are very glad she is out of Blackhawk.

Rooms were small, tour guide was not very professional in the end, one of the rooms I looked at didn't smell good at all. Most of the building was under renovation, so probably not a great time to get a feel for things. The residents seemed happy. Very simple place, very rural feel to it. Price is good and all inclusive--that is why it has 2 stars.
