Laurel Glen at Stephenville Assisted Living and Memory Care

I would choose Oakwood over other options in the area because of the level of care my loved one received. Her aides and the nurses were very responsive and always friendly, and they were very good about being very sweet and compassionate through the ups and downs of dementia. They were also very good at talking her into doing what she needed to do as far eating, grooming, physical therapy, etc. Oakwood was very diligent about cleanliness. The care was good, but turnover made things difficult sometimes. The director and one nurse in particular were not very engaging, and we saw them go through three different directors while we were there. But the aides were just amazing. My loved one didn't participate in a lot of activities, but when she did she enjoyed them.

I would give the food a 5, the staff a 5 also. I had zero complains while my mother was living there. I feel it is hands down the best facility in Granbury.

This is an awesome place! The staff are compassionate, caring, and kind to the residents and families. They go above and beyond to love on your loved one. At the time when my loved one was there the community was between chefs. The maintenance team ended up filling in for the chef. Due to this the food wasn't always consistent. They did try to assist my loved one in providing good that they would enjoy.

  • Laundry
  • Social activities and outings
  • 24-hour emergency response

Assisted Living

An assisted living community is an organized living community which offers support, care, and supervision to medically stable individuals who are not in need of skilled nursing care. The types of service offered often include support with ADLs. Residents may be kept under observation and assessment. Under certain circumstances, communities may provide skilled nursing care, Alzheimer’s care, or “aging in place”. The state of Texas maintains a number of stipulations related to this type of community. The Texas Department of Aging and Disability Services (DADS) licenses several types of assisted living communities certifies this community type. Staff members obtain training in a variety of areas. These areas include basic emergency first-aid, basic principles of management, resident characteristics (including dementia), and assisted living standards. This community type has a number of provisions pertaining to medication assistance. These provisions include assisting residents with taking medications, reminders to take medications at the prescribed time, and residents may self-administer their own medication. This community has a number of provisions concerning food and diet which include exceptions must be specifically approved by the Department, Therapeutic diets as ordered by the resident’s physician must be provided according to the service plan, menus must be prepared to provide a balanced and nutritious diet and such as that recommended by the national food and nutrition board, and communities must provide at least three balanced and nutritious meals or the equivalent per day. This type of community is required to offer a number of services. These include general supervision or oversight, activity and/or social program for residents, personal care, and assistance with activities of daily living. Public assistance is available through Medicaid 1115 demonstration managed care waiver program–called STAR+PLUS. There must be one toilet for every 1 residents and one bath or shower for every 10 residents. In total, there can be no more than 4 residents to a room. Before a resident is accepted, they must go through a service plan screening to make sure their needs are going to be met. This screening includes a number of areas of interest such as must address the residents individual needs, preferences, and strengths, a comprehensive assessment , and interviews, and home visits.
