I had my dad over there for about 2 years. The facility and staff took excellent care of my dad during his end days. They were very caring and genuine during his stay there for him. I know that the building can be upgraded, but the care is second to none. My aunt is there now too. Awesome

The staff are professional, friendly and caring. As well as providing a home friendly, clean environment, with home cooked meals. Thank you Heart-to-Heart for being there for us at a time when we needed assistance the most.

We chose this facility because i felt it was a homey place, it did not feel cold and institutionalized. My favorite part about this facility is the people there. The workers are very outgoing and will help in anyway they can. My mom has no complaints about the food, I have never eaten there before, but the coffee is very good. The first room my mom was in was small but nice, now the room she is in is much bigger and brighter. I would definitely recommend this facility.

Public assistance is available through 1915(b)(c) MLTC program. This community type has a number of provisions pertaining to medication assistance. These provisions include placing an oral dosage in the resident’s hand, opening the container and removing a prescribed amount of medication, and unlicensed staff may assist residents with self-administration. This type of community is required to offer a number of services. These include personal care services, assistance with self-administration of medications, assistance with activities of daily living , and nursing services. This community has a number of provisions concerning food and diet which include the community must provide a variety of regular meals that meet the nutritional needs of residents, the community must provide therapeutic diets as ordered by the resident’s health care provider for residents who require special diets, meals must be adapted to residents physical abilities , and meals must be adapted to residents preferences. Before a resident is accepted, they must go through a service plan screening to guarantee their needs are going to be met. This screening includes a number of areas of interest such as monthly nursing assessment of resident as ordered by physician , communities must document such assessment, community must document any substantial changes in a resident’s status which may necessitate relocation to a more specialized community , and medical examination report. There must be one bath or shower for every 1 residents and one toilet for every 1 residents. The minimum number of residents for this community type is 1. This type of community is required to have a staff member on duty 24 hours a day. Staff members receive training in a number of fields. These areas include universal precautions, 12 hours of continuing education every 2 years on topics related to assisted living, resident behavior and needs, and community sanitation procedures.
