This place is terrible. My mother was in the new Swan Lake recently [date removed] and she kept falling. They never put a bed alarm or rails on her bed, and they never checked her out after the falls. She laid in bed for two days with a broken hip, in severe pain, and they just let her. She can’t communicate that well so they just let her stay in bed for days with no food or water, and her diaper was saturated when I arrived Thursday night. When I went to see her, her lips were crusted shut and she was severely dehydrated. She also had a bruise on her hip from the a fall. Friday, after ANOTHER fall they finally did an X-ray and she had a fractured hip. She wasn’t taken to the hospital until 830 pm Friday night. This horrible place allowed my mother to sit there for days with a fractured hip. Don’t ever bring your loved ones here unless you want them to die. This place should be shut down!!! My mothers hospital blood work showed her dehydrated and malnourished.

Please don't believe the three pictures posted about this facility. The residents never sit outside. My family has been waiting 3 months for a discharge plan. Itis under new ownership Swan Lake and I spoke to the new administrator about the issue. He promised to get back to me and as of today 3 weeks later not one word. The social worker never follows through or calls with the discharge information. The facility smells terribly. The social worker that left had great follow up. You never get an update about anything. I would not recommend this place to anyone. Initially when we visited 5 months ago there was no one at the desk to check anything so my entire family just walked in. I will be picking my family member up this week to live with me and use community services to get the equipment he needs since no one at Suffolk Center/Swan Lake has obtained anything. So sad

I couldn't get my family member out of there fast enough. They had a patient who was supposed to be watched continuously. He ended up choked to death by his own seatbelt in the bathroom. That doesn't seem to me like competency. It's dirty. Administration is nasty. Stay away.
