Silver Leaf Assisted Living at Lavendale Circle

The meals are just okay. They seem to serve a lot of items that don't appear to be too healthy for elderly people. Not enough fruits and vegetables. Communication with the family is excellent. The move-in process was very easy and went very smoothly. The place could be cleaner and have nice plantings in the front of the home and the walkway to the home. Everything is dead for the entire year.

Several different staff were rude every time we went. Constant mix ups with medication or medication not being given at correct time/doseage. Had to ask for laundry to be done and room to be cleaned at times. Staff would argue with patients. Had to ask the staff to give showers because they would not stick to schedule. Brought issues up to house manager who was very unprofessional and rude and then brought that up to the area manger who said she would speak with house stuff and nothing really changed. Meals we're decent. Little to no activities.

Monticello West seems to be the perfect place for my friend. She has dementia and the staff is always very helpful to keep her engaged in activities, both physical and mental. My friend is very happy and her brother came for a visit and he met with the staff and is very happy with the place.

My family and I have been impressed with the community. Aesthetically it is beautiful, and clean. The aides are friendly and responsive. My loved one has had some complaints about the food and the variety offered along the freshness of the food. My loved one doesn't care for the activities offered.

They ripped us off. It said in the contract that if she passed away that we would get a refund. My mom passed away and they owe us money and we still have not received it. They talk a good game but they do not follow through with anything, they did not keep any promises made. I would tell others to make sure that you get things in writing from them because they will cheat you.

Terrible place. My dad was there less than a month and sent to emergency room with infection. Not enough staff.to help with clients. Owner money hungry. Gave them 30 day notice the first week. They got mad i was moving him. Don't trust them. Worst of the worst. I'm warning you to not find out the hard way.

Don't send your loved one here if they need a lot of care. It is okay for folks that are more independent. They expressed that they could handle a high level of care and we have found they can not. The room size was okay. There is very little oversight. They do not have nurses on staff. No medical person is in charge. My mom ran out of an important medication, they did not let me know. She was out of it 3 days, they felt it was not a big deal.

Beautiful home and kind staff

In the short time that my father stayed at the house on Lavendale Circle, he was treated very well. [Name removed] and her staff provided around the clock care and did everything possible to help him feel included and at home in his new surroundings. The home was beautiful, the staff was helpful and caring and I felt like I could pop in at anytime. My father enjoyed the sitting area outside and definitely liked the food that was served! We appreciate everything that was done for my dad to make sure he was comfortable and happy with the service. Thank you again, [name removed] for [name removed].

Super friendly, nice, caring staff and a lovely home/location. Sadly this did not work for our dad because his needs were a bit more than they could accommodate. It was a bit steep in price with the move-in fee, along with the rent and the 45 day cancellation which hit hard since our dad moved to the hospital and wasn't able to return back to this place.

This wonderful place with guardian angels taking care of a few residents was an answer to prayer. They have small numbers of residents to caregivers, unlike nursing care or assisted living homes that pack as many in as possible and do not have trained or qualified staff. One place started out fine, but resulted in good caregivers leaving because they were marked "problems" by the administration, when they reported abuse of residents. My mom was smothered 4 times by an animal because she wouldn't comply. Another place, [removed] was a nightmare. My mom only stayed 6 days due to way too many violations of conduct to list. LAVENDALE, they do what they say. Their caregivers are truly loving toward residents giving them their gentle care and undivided attention. PLEASE SAVE YOUR LOVED ONE AND YOURSELF FROM PLACES LIKE THESE.....LAVENDALE US YOUR ANSWER.

I'am comfortable with my fathers move to this care home, but I do have some concerns. I need to have a talk with the staff about the amount of sugar, and sweets he gets. He is a diabetic, and needs to be careful with his sugar intake. Other than that issue that staff have been terrific. They do a good with keeping the home nice, and clean. At the moment I'am 50/50 if I would recommend this care home. It is a still a work in progress overall.

I didn't tour this home because after speaking to them about the services they provide, they were not able to accept us because we require a feeding tube. I had no issues during our interaction and they actually helped me make sure I was picking the right place.

This is a beautiful, secure home which has been converted. I like the idea, but I felt there was no true structure, and limited exercise and interaction opportunities. This is not the type of environment Mom needs.

I like that it is a community and my mom feels like she is apart of family there. The staff understand the need of everyone making it cozy for them. My mom has not complained about the meals and activities they provide. From what I know it is all going good and I am totally pleased with the community.

I have no complaints . Happy and pleased with the staff and services received at Lavendale Circle. This was the best choice for Mother considering her age and physical condition. I recommend this option for elderly 's care vs nursing facilities. She has been in a nursing facility for years before Lavendale Circle. There is no comparison as to how much better this home is compared to a nursing facility, at this point in her life. More personnel attention and care from the staff. I , 100 % , recommend Lexington Place and their organization. [Removed]

The staff have been patient with my dad since my parents moved into Lexington Place. They do not seem bothered by my dad's wondering. My mom is in better spirits. I believe the staff are qualified to take care of my parents. They seem to generally like the food. A Place for Mom was a good service for us.

Lexington Place - Lavendale - is amazing. I've been to 500 places over the years and this is by far the best. They stay on top of the residents general health. If there is ever any issue the staff are very good about communication. I am very pleased with this facility.

I visited Lexington Place 6/16/15, when I saw a friend that is staying there. The place was very impressive. It's a huge home and well maintained. The backyard has a nice covered gazebo with chairs and has a calm and peaceful effect. The rooms are large with a private bath. My friend there is very happy. She moved out of another facility that was not taking proper care of her. We enjoyed her room, which her family was able to customize for her. The staff were very friendly and attentive. But what I noticed more than anything is that it's a very calm and peaceful place. My friend also has 3 windows in her room, which bring in sunlight and the outdoors. It's a huge improvement over the last place where she stayed. She is very happy there and that says it all.

I love them! We are just so pleased that we came across Lexington Place! My family and I can't believe how perfect it is for my loved one! There is great communication with the staff, and if there is ever an issue, they call right away. They also have good documentation, which is important! They own up to what they say, as well! The room is huge and it was at the perfect price! There's a nice patio and everything! Only issue is that they could improve on having more activities, but that could always change in the summer. They are really great!
