I am very sure that working on a memory care unit is extremely difficult for staff. Staff have to be mind readers with the patience of a saint when dealing with folks with dementia who can be violent, angry, incontinent, non verbal, etc. In my opinion, therefore, staff should communicate with family members more than they do. This lack of communication would be my main complaint. Staff are often folks I have never seen before and I never know who is in charge. In the long run, however, I think they do the best they can.... Read more

Oak park is not a facility I would ever recommend. It's assisted living and skilled nursing are chronically understaffed, solely due to an unwillingness by the owner to spend more money. The cheapness bleeds into everything, including quality of meals, and underpaid staff don't perform their 100%, either. Oak park does not treat employees right or well, and this also bleeds into everything. It is plain to see in an hour in the assisted living section or skilled nursing section.

A family member went to Oak Park Place for rehab and the facility was horrible. On one occasion she went to the hospital for a drug overdose. On two occasions, she was given another residents medication. The rehab portion was fine but staff is overworked and on several occasions they were non existent. The social worker and management are too chicken to meet you in person with your concerns and prefer phone or email conversations. On one occasion they were hiding in their office when confronted. A very poor sign! I would never return or recommend this facility.

My loved one was previously a resident at Oak Park Place Madison and I visited about once a week. Overall, his experience at this facility was good. From what I could tell, the staff was nice to my loved one. Like all facilities, they are busy. Some staff were better than others. The facility did seem clean. There was not very much communication with the family, I didn't have a person to communicate with. However, hospice communicated with me. So families should just make sure that they have that information.

We toured Oak Park Place Madison during our search for Assisted Living for our loved one. This facility seemed okay. It was very clean and modern. The staff were helpful and welcoming.
Expensive and did not feel welcoming or friendly. Friend had stayed there and did not like as it seemed there were no activities to do there. Here if you go from independent living to assisted you do not move from your apartment.
Response from Oak Park Place Madison
May 4, 2023Hello, we appreciate you reaching out. All feedback and concerns are taken very seriously.
My dad loved the positive attitude and friendliness of the assisted living staff. He had been on his own for over 40 years, so the move to a facility was a big change, but they were very respectful of his opinions and ways of doing things. When his care needs increased, they were very responsive. This was a perfect fit for him and for us.

Response from Oak Park Place Madison
May 4, 2023Hi Dorothy! Thank you for your kind word. We are glad your dad is doing well!
My loved one has been at the Oak Park Place location on Nakoma Road for about four weeks. We toured twice and thought it was lovely. Even though there were very few people (neither staff nor residents)around when we toured (not a good sign), my loved one was happy with the apartment. We thought it would be fine. The rooms are very nice and there are a couple great staff members who are qualified and able to deliver good care. But in general, they are extremely short-staffed in many departments, the staff on the floor is inexperienced, and they don't seem to be able to deliver the care that is necessary for my loved one. Considering the price, it is extremely disappointing. I know it's extremely difficult times right now for hiring and retaining staff at care facilities, and residents can be challenging to care for. Even so, I would not recommend Oak Park Place Nakoma.

Response from Oak Park Place Madison
May 4, 2023Hello, we appreciate you reaching out. All feedback and concerns are taken very seriously.
Our family member was a resident for several years at the Arbors where the community and the evening meal was greatly enjoyed. A change in medical condition and a hospital error on discharge created a difficult situation. The housing director and the nursing home staff were incredible in assisting us to getting the resident safely placed and eventually transitioned into an assisted living apartment. We are grateful for all the competent staff that made a hard situation much easier. We would highly recommend the Oak Park Community.

Response from Oak Park Place Madison
May 4, 2023Hi Beth, thank you for sharing your experience!
Our family had an in person tour with Oak Park Place and we thought the tour was very good. We believe at the time when mom would of been on a waiting list because they were full, but it was an excellent experience.
I had a great tour experience at Pak Park Place Madison. The facility is clean and from what I saw is well run. They had a mix of independent living, assisted living and memory care offered.

May 4, 2023Thank you for your feedback!
My one suggestion would be to keep the resident's rooms clean. My loved one's was awful. Many times I asked for a vacuum to do it myself and was told there wasn't one. The bathroom was rarely cleaned....I did it myself. He is no longer a resident here.

Oak Park Place was a nice facility, but it was too large and we were scared our loved one would get lost in the shuffle. It was brand new when we toured and not a lot of people were around, so it was hard to get a good feel for what it would be like.
My loved one is at this facility and really enjoying her stay. It has a great community feel and everyone is close knit which she loves. It is on the pricier side, but I would recommend them to others.
Our family had an in person tour and Oak Park is a wonderful facility, and normally on the tour they allow the families to eat meals the residents but since it was during covid they packed us up an evening meal to take home and we thought it was very good. However our big concern is the activities are all on different floors, and if our loved one started having memory loss we did not want to have to worry about them getting lost.

[name removed] , our case worker went above and beyond to make sure we knew everything and actively coordinated her doctor's office, therapists, and her new memory care residence. thank you to all the staff for getting our mom strong again.

My mother was sent to Oak Park Place for rehab after being in the hospital for a fractured pelvis. We then had to search for an assisted living facility and Oak Park Place graciously let her stay until she could be placed in her new facility. They took very good care of my Mom and our family is very grateful to the whole staff.

Oak Park Place Madison did a wonderful job with the care of our loved ones. Everything was taken care of and available for our loved ones in the community. The exercise classes was one of the many activities the community offered for the residents. Our loved ones enjoyed the programs in the afternoon that the community had prior to the pandemic. My loved one really enjoyed the community and everyone was friendly. All the staff were easy to talk to, and answered all of our questions during the move in process.
