Quality of care and staffing has declined significantly within the last year. The prior Director of Nursing (DON) and facility Administrator were competent, qualified skilled professionals who cared about the residents and their families. In addition, a significant number of the facility staff were also competent, qualified, experienced, and caring professionals. However, the current DON, administrator, and nursing staff is the exact opposite. The DON is a registered nurse with a significant number of newly hired/appointed young and inexperienced nurses. This condition is made worse as a result of newly hired/appointed CNA personnel who are lazy, uncaring, and just looking for a paycheck. Residents are frequently allowed/forced to walk around /lie in their beds in urine soaked/feces containing undergarments and clothing. Unless they are assigned to the one dedicated, competent CNA employed by the facility, residents also go without showers/baths for long periods of time. CNA's justify this by using their standard rationale that the resident "refused his/her shower/bath." When they (CNA's) are not using this rationale, they justify their actions/inactions stating that laundry has no clean bedding or towels. Unfortunately, laundry personnel also frequently support this rationale. CNA's assigned to certain high risk units also frequently leave residents on these units alone and unattended. In addition, they often [Removed]/punish residents by denying them their smoke breaks simply because they have the "power" to do so. Further, despite several reports of other forms of resident [Removed] by at least one male CNA, no action was taken to protect the residents. Meals are generally high in sodium content and provide no nutritional value. Meals that contain what would be nutritional foods like broccoli, carrots, etc. have no nutritional value because these foods are generally overcooked, mushy, and unappetizing. The meals are also usually cold when they are served to the residents. The typical weekly meals include at least one - two servings of a dinner meal that leave residents hungry and unsatisfied (e.g. 1 slice braunschweiger and potato chips, iceberg lettuce salad and a cornbread muffin, two thick pieces of cold white bread and one slice of meat and cheese, etc.) As a result, a number of residents have lost a significant amount of weight. Unfortunately, the DON and facility administrator have locked themselves behind closed doors ( and locked all of the residents in) and only come out when there is a code "green or blue" announced.
