They continued to feed my grandpa when his stomach was clearly distended and he wasn’t digesting at all; he was dying and they kept shoving food in the tube. He went on hospice for a short while -far too short for a place that should be well-versed in the dying process- and died. The people there are just doing a job and checking boxes and don’t seem to care very much about quality of life or details. They can’t hire enough people or pay them what it’s worth to be kind. If you want anything done here, you have to create a paperwork trail. My grandma's vision deteriorated until she was legit blind and no procedures changed. Her food is supposed to be on a plate with a little fence so it doesn’t fall off, and cut up for her. It’s been about 6 or 8 months and it still isn’t, she’s forced to eat with her hands. Why can’t anyone spend 45 seconds with her to help her locate things? Only when you start a formal complaint does anyone spend two minutes to do simple compassionate acts.

This facility, at the start of the COVID issues in March, refused to admit my father-in-law's RN from his hospice provider. They also lied about his condition to my husband, and they let him die without admitting the family for end-of-life. I wouldn't put my dog in this place. We filed a complaint about them with the state of Texas and an investigation is ongoing.

It is beautiful. The care is marvelous and the beds are at a good height for the patient.

The staff has been dang good! We are semi-happy with their nursing care. The problem is, my loved one was in over the holidays is the problem. Just getting used to how people operate. I would just like for them to check on her more often. My loved one is using the therapy services now but there hasn't been much change. I am afraid that might be because of her own condition.

There was only one nurse and one aide on my loved one's floor when he arrived but I am much happier now.

There was an odor from the air freshener and it was too strong. She had to shower herself and they did not help her right away.

The people are really on top of things and they are very informative. They contact me anytime I have concerns. I am confident with a short stay.

The initial lady who did the evaluation was very caring and spot on. I have seen people and they are caring. We came in on a weekend. We did not know what is going on. He was unsure of what is happening. It is hard to transition on a weekend. I asked about a shower and they gave him a shower right away. The rooms and bedding could be cleaner. I saw something spilled under his bed. I heard the meals are dry but they have a decent selection. He complains about everything. No one has called me about his care. I have to go to the nurse's station for an update.

At first arrival, it is very clean. I am shocked about how clean it is! Most of the staff is extremely helpful! With the exception of a couple of nurses, they are very nice people! I am disappointed with the nurses.

I am really pleased. The woman in charge of therapy is nice and very positive and tells me whatever I need to know. It's clean. The therapy and social worker have been very helpful with information. The nurses have attitudes and are short. I would like a little bit of time spent with me. That's the big difference between the therapy team and the nurses.

The staff are just great, caring people. A nurse went above and beyond during my loved one's stay here. He is a challenge to care for, and they have been nothing but supportive and caring. He is a picky eater, and he said the meals were okay. The facility is very clean and very nice. When I call or visit, the staff are very informative.

The staff, for the most part, is very friendly. There are times when it seems like they are a little short-staffed, but other than that, they are very nice. The CNAs come off as too tired. More attention and time per patient from the nurses would be a nice improvement. Communication is my main concern. I would like them to reach out or give some sort of report to the family. It would be nice to have a clearer way to see how my father is doing.

So far, it is going very well. The staff, as well as the nurses, have been absolutely amazing. They have been so helpful in keeping us informed. They are providing us with information and helping us through this adjustment.

A Place for Mom has scored Fundamental - San Gabriel Rehab and Care 1 out of 10 using our proprietary review score.
We assign review scores to give a more reliable view into senior living communities and home care agencies. Our review scores prioritize reviews that are recent — the past 24 months — because we know families need current information when choosing senior care.
Those with many recent, positive reviews receive a high review score, while providers with few recent reviews — regardless of how positive — receive a lower review score. Communities with no recent reviews will not have a review score, even if older reviews are positive. The maximum A Place for Mom review score a community can receive is 10 points.
