Some of the girls are very caring but overall please don't send a family member there. Giving wrong or to much medicines. Losing clothes. Staff that should be able to answer your questions cannot. They have excuses for everything. Go through employees like crazy. Hide from you when you want to talk. Contacting corporate they just send complaints to [name removed] and [name removed] (they have NO idea about their job). Half the time have to complain that not getting showers. Smells like urine.

Based on the experience we have had with Franklin Place Memory Care, I give them all a 3. Things went down the last 6 months +. New management is taking over, and it looks like things will improve. Too soon to tell.

I would encourage others to look into the community for their loved ones. The staff are very caring towards my loved one and all the residents. They put the residents first which is wonderful to see. They reply quickly to my questions about my loved one. There are three meals a day served to the residents. Some of the activities offered included balloon box, arts and crafts, music, games, and more for the residents to participate in.

This facility is very nice. It's very well managed and taken care of. The food is good, good quality and variety. Housekeeping seems to be pretty good. There have been a couple of times when I visited and noticed my brother's room was a bit messy. I brought it to their attention and they took care of it without issue. Communication with the family is very good. The staff is great. They're all very kind and caring. When I have questions during a visit or call with questions, they are good about getting answers for me. There's been some turnover with a couple of key staff players, but it seems to have settled down. I would recommend this facility to family and friends.

Autumn Leaves is a good sized community, not too big and not too small. the staff engages well with the residents because of that size. it's an easy location to get to and find, as it's close to the freeway. I think anyone who has to go through this move or do this with a parent experiences the same concerns and knows there's going to be challenges; but I've had no problems since dropping my mom off there. the staff made sure I knew they were going to take care of her and I don't have to worry about anything. within 30 days of being there, my mom tells me how friendly and nice the people are, how good the food is, and that I did a good job finding this place - so the feedback from her has been great.

We can see they have a lot of activities based on their activities calendar but we have no idea if our loved one is participating in them. We would love for the community to have away for families to keep up with what their loved ones have done for the day.

We are very grateful to Autumn Leaves for taking care of Dad during non-Covid conditions and during Covid conditions! The administration and staff were wonderful! Dad was very special and had his own daily rituals and they were very accommodating to his needs. Dad and I really enjoyed the walks inside the building looking at all the beautiful paintings and photographs! He really enjoyed the outside corridors and their comfortable chairs. Dad enjoyed the special activities and the special visitors that came in before Covid conditions. He also enjoyed a few special trips out also before Covid conditions. Dad enjoyed chatting with all the staff members, often in a teasing manner. Most of all, they were great listeners and provided accommodations consistent with the final months of care. Our family is very appreciative for all that they have done for Dad!

They are a clean community, The staff is friendly. The food is good. The room sizes are good. The cost is about average to other places in the area. They have a secured outdoor space for the residents to enjoy.

The meals lack sufficient calories and are inadequate in calcium in a population susceptible to bone fractures. We have requested a weekly menu plan but it doesn't appear to exist. The facility is beautiful but I fear it lacks maintenance of landscaping of the grounds inside and surrounding the facility. They should to hook up with the Master gardener program.

I absolutely love this place, I feel confident leaving the care of my mother in the hands of the staff here. [Name removed] and all of his staff go out of there way to make sure mom gets the very best. This facility is a blessing to the franklin community, and as long as it stays the way it’s operating now mom will be here throughout her end of life, and Dad once he needs such care. STELLAR JOB FRANKLIN TEAM!

Autumn leaves is a new place and very nice inside. The care is not up to par. They are short on the weekends and at night don't have many caregivers on staff. Also there is no nurse or doctor on staff if an emergency happens. People who do laundry loses it and does not find it. IF you text or place a call you do not get a response. But you can judge for yourself.

We have a loved one in your facility. The place is beautiful inside and out. Residents are able to go outdoors to enclosed patios. We actually had 2 loved ones here. The people in charge went above and beyond. We seen a lot of one on one care for my parent. The rooms are spacious. Wonderful care.

Autumn leaves is a nice facility because it is new. There are not enough caregivers when needed and also no nurse or doctor on in the evening. It is highly priced. It needs to get better with the care.

New facility planing to start taking residents Nov. 21 so we were not able to see staff interacting with residents. Anticipated staffing ratios to be 1/7.75. Beautifully decorated without being overdone. Room sizes were generous and bathrooms huge. Divided into 4 "neighborhoods" for resident's rooms but community rooms are spread throughout the 4 neighborhoods which may be a concern if self mobility becomes an issue. Total of 54 beds. Hallways are wide. Inner courtyards are completely surrounded by the building and those with elevation changes (landscape beds) are alarmed so residents are escorted outside. Residents fed restaurant style in 2 dining room areas and there is also a community kitchen area where hydration station and snacks will be available for residents. Had a small sensory room with aroma therapy, fidget blankets/pillows and changeable lighting for those needing less stimulation. Facility is laid out in a figure "8". Will take Medicare/Title 19 after private funds run out but would be transferred into a companion room. Rates based on room choice not care needs. This may not be the right facility for our family member although it remains on our list of possibles. Feel it may be too big and overwhelming. May be confused by all the white doors in the hallways. Dining rooms and some public areas may be too large for hearing well. Outdoor views in courtyards only have view of the building walls which would be tough for dad who is a nature watcher.
