Having moved an older friend and her brother in Aug. 2024, I am most impressed with Azalea Manor. The staff is extremely friendly and helpful, the place has "southern charm". The new manager is very accommodating. Food looks 5 star quality-better than what I am cooking.. They seem to really care that their residents are happy. Stop by and visit- that's what I did, and again, was extremely impressed. Can't rate the activites because I haven't been there for any, just heard from the staff and some of the residents that activies are good!... Read more

[name removed]-Ex-Wife, Was impressAzalea Manor and upon initial evaluation and touring. At the time the director was [name removed] and her assistant Director was [name removed]. Everything seem to have been running very smooth and they were on top of things. And three weeks later, [name removed] was gone, and three weeks after that [name removed] was gone. And then I felt that everything fell apart. It seems as if the facilities not covered properly by anyone in charge. No one taking responsibility for the clients. Lack of staff and Attention. Cold rooms floors not swept. Unable to get a hold of the manager or Director [name removed] by phone. That’s very disturbing. [name removed] was out of his blood pressure medicine and no one informed anyone of the situation until his blood pressure was accidentally caught which measured 174/104. Unsafe!!! Medication was supposed to have been given to pharmacy to bubble package and that has not been done yet. That information was not passed along or carried through.

The staff at Azalea Manor are kind and it is very affordable for the area but it is a no frills place. It was one of the few we could afford.

My father quit doing his rehabilitation when he moved into the property. He is not very out going either and kind of a loaner. The staff has done a great job of making him comfortable and they bring his meals to his room. They are very friendly and do there best to make him feel comfortable.

I love my visits at Azalea Manor. It is such a family-like environment. All of the residents that live there are so kind and inviting. [name removed], the administrator is so kind and goes above and beyond to make sure the residents as well as staff are well cared for to the best of her ability. The staff that I interact with truly care about what they do.

Based on 2022 stay - some areas we think they could improve on based on our experience with the quality of care, the family contract and just the overall experiences for residents/family relationships.

Response from Azalea Manor
January 11, 2024Hello! I am the new executive director for Azalea Manor PCH. I'm very sorry to hear that you had a negative experience. We were aware that changes needed to be made and we have been working very hard the last 2 months since my arrival to streamline our move-in process as well as getting a great team of folks in place in order to offer our residents and their families the best care possible. We are excited about all that we have achieved and look forward to having the opportunity change our reputation within the community going forward.
We think the staff are nice and there doesn't seem to be high turnover. My mom's room could be cleaned better. Azalea Manor is an affordable home but needs a few tweaks.

January 11, 2024Hello! I am the new executive director for Azalea Manor PCH. We were aware that changes needed to be made and we have been working very hard the last 2 months since my arrival to streamline our processed as well as getting a great team of folks in place in order to offer our residents and their families the best care possible. I can assure you that a housekeeping schedule has been put into place and that I am checking daily to ensure that those assignments have been completed. We are excited about all that we have achieved and look forward to having the opportunity change our reputation within the community going forward.
We escaped after a month of horror stories. I would tell others not to use them unless the person needing facility is sound of mind and ambulatory and not needing a larger wheelchair as they don't fit through the door to leave the room. Manager ordered staff to withhold meds if payment wasn't received as we were leaving (payment already had been given).

January 11, 2024Hello! I am the new executive director for Azalea Manor PCH. I'm very sorry to hear that you had a negative experience. It saddens me to hear about what happened to you. I am aware that one hallway has rooms with more narrow doorways than others. We have folks in those rooms that use a standard wheelchair with no problems. One resident that we currently have did request a larger wheelchair and had issues with the doorway, but we were able to move him to another room that had a larger doorway. We have been working hard since my arrival 2 months ago getting a great team of folks in place in order to offer our residents and their families the best care possible. We are excited about all that we have achieved and look forward to having the opportunity change our reputation within the community going forward.
If I were being treated for depression this is not a place I would want to stay. The atmosphere was dark and the rooms were old and drab. I also was not impressed with the dining facility. I know the directors heart is it in, but she runs 2 different facilities and I just felt she had so much responsibility and a lot going on.

January 11, 2024Hello! I am the new executive director for Azalea Manor PCH. I'm very sorry to hear that you had a negative experience. We were aware that changes needed to be made and we have been working very hard the last 2 months since my arrival to make this a more vibrant environment. We are painting and renovating the rooms and adding daily activities. In addition, my sole focus is on THIS community and not split between 2 separate communities. I am proud of the team I have in place at this time. We are working diligently to offer our residents and their families the best care possible and making constant improvements. We are excited about all that we have achieved and look forward to having the opportunity change our reputation within the community going forward.
When he left home, he was placed here so the location is not great but still able to see him at times. Communication on getting back to me with info is not great at times but overall is good place.
Azalea Manor is more catered to someone who is more assisted living, than memory care residents. My loved one was there for a short stay. I was highly dissatisfied with the overall experience, especially with management. There were times that I would find my loved one's room not cleaned and it was sticky. There are times my loved one wouldn't be clean or in dirty clothes and there was poor hygiene. There were times that the community would tell me that my loved one was combative, and my loved one would let me know me the staff was rough with them. The community was good at taking care of the residents who only needed assisted living. If there were other residents needed more care for memory care, this wasn't happening. [name removed] is a very nice person, and then when we got into the community the statements from her didn't come true. Things were promised and things weren't followed through upon. I gave notice that my loved one was coming to pick my loved one up, and the staff was either not communicated or miscommunicated. Anytime that things happened that my loved one did, they never communicated this with me until well after the fact and that it was wrong. There was a time that the community had an outside activity for the residents, and said that my loved one was participating. The director lied to me and my loved one was letting me know that they didn't participate in this specific activity. My loved one wasn't dressed for the activity and this was very upsetting to me. The customer service was great during the time of bringing my loved one into the community. The management tried telling me not to go to a new community as well. The director kept saying that I could have items, and so they just took the items from my loved one and let me know after the fact. In addition the director kept saying that I never let her know about issues that I had with the community or care my loved one was receiving. I did try and let her know of all the challenges. We also had billing challenges as well. The community didn't want to refund me money that was owed.

January 11, 2024Hello! I am the new executive director for Azalea Manor PCH. I'm very sorry to hear that you had a negative experience. I came in knowing that changes needed to be made and we have been working very hard the last 2 months since my arrival at getting a great team of folks in place in order to offer our residents and their families the best care possible. It has been my experience that when you get negative behaviors from a resident with dementia, it is more often than not due to the way that WE are approaching them or something that we are doing that they do not want. Almost always, they are simply attempting to communicate and frustrated that we are not understanding. I have worked and continue to work diligently with my team educating them on better ways to interact with those in our community that do have dementia. We are excited about all that we have achieved and look forward to having the opportunity change our reputation within the community going forward.
The everyday staff are wonderful and they are so friendly and sweet. I do think they could do better with cleaning of the rooms, her bathroom is always dirty when I go to visit. Communication has not been very good, I have a very hard time reaching the administrative staff and every time I try to call to talk to my mom, no on answers the phone. I do think I should be getting more for the money I am paying.

I would recommend Azalea Manor to others for their loved ones. The staff interaction has been good. The caseworker, hospice care coordinator, and the Executive Director have been good about reaching out to me to keep me involved in my loved one's care.

We were promised so many things and that our loved one would qualify for the assistance that came from being on hospice. However that was not true, our loved one was lucky to get a bath once a week, they messed up and forgot their medication and they did not get the physical therapy needed after having surgery. If you do not qualify for their hospice care you will not receive any assistance. The hospice care team is wonderful, however we would not recommend to other families.

It was a short stay for our loved one but the lady who runs the place is very nice and he really seemed to enjoy the meals and dining. However, they could work on some improvements like the cleanliness, staffing, and just overall communication. Also there were no activities that we were aware of.

It had a very home feel, caring, and they seemed understanding during the tour. They also seemed to be understaffed.
This is geared for someone that is more comfortable in a home type of environment rather than a clinical environment. The cleaning and the care services are better than acceptable. The staff are very friendly and well trained. I do feel like there may be a shortage of staff but everywhere is going through this right now.

A Place for Mom has scored Azalea Manor 7.3 out of 10 using our proprietary review score.
We assign review scores to give a more reliable view into senior living communities and home care agencies. Our review scores prioritize reviews that are recent — the past 24 months — because we know families need current information when choosing senior care.
Those with many recent, positive reviews receive a high review score, while providers with few recent reviews — regardless of how positive — receive a lower review score. Communities with no recent reviews will not have a review score, even if older reviews are positive. The maximum A Place for Mom review score a community can receive is 10 points.
