My loved one is currently a resident at Trinity Shores of Port Lavaca and everything has been great for him at this facility so far. The cleanliness, friendliness and staff are great! The staff goes above and beyond to make everyone happy. The care services, meals, activities and value for cost are also great. I would highly recommend Trinity Shores of Port Lavaca to others. This was the best move ever for my loved one!

Sodalis is clean, serves a variety of hot meals and the staff is very friendly. The activities director has gotten my mom to join in some of the activities which is something new for her. She is not a social butterfly.

Assisted Living

An assisted living community is a residential community which offers supervision, care, and support to self-sufficient individuals who are not in need of skilled nursing care. The services provided often involve aid with ADLs. Inhabitants may be kept under observation and assessment. In certain cases, communities may provide hospice, dementia care, or nursing. The state of Texas has a number of stipulations associated with this kind of community. The Texas Department of Aging and Disability Services (DADS) licenses several types of assisted living communities certifies this community type. Staff must receive education in a number of subjects. These areas include abuse/neglect, basic principles of management, federal laws, and skills for working with residents, families, and other professionall providers ethics. There must be one bath or shower for every 10 residents and one toilet for every 1 residents. In total, there can be no more than 4 residents to a room. This community has a number of provisions concerning food and diet which include The meals must be served daily, at regular times, with no more than a 16-hour span between a substantial evening meal and breakfast the following morning, Therapeutic diets as ordered by the resident’s physician must be provided according to the service plan, exceptions must be specifically approved by the Department, and communities must provide at least three balanced and nutritious meals or the equivalent per day. Before a resident is accepted, they must go through a service plan screening to assure their needs are going to be met. This screening includes a number of areas of interest such as a comprehensive assessment , must address the residents individual needs, preferences, and strengths, and interviews, and home visits. This type of community is required to offer a number of services. These include activity and/or social program for residents, personal care, general supervision or oversight, and supervision or direct administration of medications. Public assistance is available through Medicaid 1115 demonstration managed care waiver program–called STAR+PLUS. This community type has a number of provisions pertaining to medication assistance. These provisions include supervision of a resident’s medication regimen, returning medications to the proper locked areas, and reminders to take medications at the prescribed time.
