He enjoyed the social aspect of visiting and playing cards with his friends at Sunnybrook. His mental and physical health also improved while at Sunnybrook. We credit Sunnybrook with giving us a couple more years with him.

This community type has a number of provisions pertaining to medication assistance. These provisions include medication reminders, documenting whether or not the resident took the medication, and reading the medication label to residents. This community has a number of provisions concerning food and diet which include communities must provide three daily meals. The minimum number of residents for this community type is 3. There must be one bath or shower for every residents and one toilet for every 1 residents. This type of community is required to have a staff member on duty 24 hours a day. The maximum occupancy is 1 residents per room. Public assistance is available through A Medicaid 1915(c) waiver called the Supported Living Program and a 1915(b) waiver called the Managed Long-Term Services and Supports program. Before a resident is accepted, they must go through a service plan screening to make sure their needs are going to be met. This screening includes a number of areas of interest such as provider must ensure that the resident and the resident’s representative, if any, are informed of the potential consequences of activities that the provider considers to pose risks to health and/or safety, evaluation of a prospective resident’s physical condition, A comprehensive assessment, and evaluation of a prospective resident’s cognitive condition.
