After being admitted into rehabilitation in December, my sister and I made the difficult decision to find an assisted living community for our mother. After touring many other places, we met with the owner of Rainbow Gardens. They have an open-door policy and invited us to tour both of their homes whenever we were ready. We met with the manager, the staff and the owners three different times before finally choosing to bring our mother to Rainbow Gardens. It was a big decision, and they were extremely patient and accommodating. Since then, we are very pleased with deciding to place our mom here. The staff is great! They are extremely observant and loving, and they do put the residents first. The home is always clean and the meals look amazing. My family is very close, and we also appreciate that this is a family-owned business. We recommend them to everyone!
The communication needs improvement as it is difficult to get accurate information on our family member. It seems very challenging to get answers to our questions - we can't get a bill, find out easily when certain things are scheduled like showers and when she had the last one, or when laundry is done or whether she ate her meals. Also I would recommend a system of wearable alarms that have fall detection if someone falls and isn't able to press their call button. It would also be helpful if check ins could be scheduled for folks who need some regular assistance with toileting.
Dad had a pretty severe stroke and was able to be placed at Grace Manor where many ALF places would not accept him. The staff is wonderful, caring and very observant. They help with all his ADLs and encourage him to participate whenever he is able. They were able to tell when there was a difference in his weakness and immediately call 911 to get him the treatment he needed. He loves them and knows he is in a good place. PT, OT, skilled nursing and diabetes management have all been arranged by Grace Manor, even coordinating with the VA! Grace Manor is a godsend, and helped keep dad from having to go to a nursing home!

Before a resident is accepted, they must go through a service plan screening to guarantee their needs are going to be met. This screening includes a number of areas of interest such as monthly nursing assessment of resident as ordered by physician , community must document any substantial changes in a resident’s status which may necessitate relocation to a more specialized community , communities must document such assessment, and medical examination report.
