Transition House

Surrounded by mature trees in the South end of Sudbury, Transition House at the Regency offers accommodations and care to people who are in a transitional state of health. We are located within a five minute drive of Health Sciences North and the Cancer Treatment Center.

At Transition House we offer a variety of services specific but not limited to assisted and retirement living.

Eligible Guests are:

Currently on a waiting list for long—term care, assisted living or retirement living who are generally mobile You need assistance and/or supervision to prepare for and complete your personal care and instrumental activities of daily living.

Currently convalescing from hospital. You have a medically stable physical or mental health condition or have a minor functional limitation.

Currently diagnosed with early onset dementia. You require assistance and cuing with activities of daily living, but have no complex medical needs requiring on-going nursing-care. You are medically stable, have a minor physical or mental health condition or a minor functional limitation.

Currently living in an extended family environment. You have a medically stable physical or mental health condition or a minor functional limitation.
