The Fountains

The Fountains Assisted Living is a community is Sweeny, Texas. We are located in a safe, secure location on the same campus as Sweeny Community Hospital, Sweeny EMS, Senior Counseling Services and a medical office building. This provides us a quick access to an emergency room and ancillary services as needed. We are also directly across the street from a dialysis clinic. We have van transportation to physician offices in neighboring towns.

Sweeny is away from the hustle and bustle but within an hour's drive of medical specialists in Houston and Galveston. It is a civic minded, friendly town with the motto "A City with Pride". The town is close to beaches and shopping, has a low crime rate and hosts many churches and several restaurants.

The Fountains provides individualized care plans that focus on individual needs, interests and desires. You'll know we care by the compassion and respect shown by our dedicated care providers. We offer a warm and loving environment and provide a perfect balance of independence and assistance.
