We would definitely say all categories are a 5. We are extremely happy with The Addison which just became Primrose as of February 1st. The new owners have some great new ideas are take great care of mom.

My loved one is currently a resident at The Addison of Pleasant Prairie in the Memory Care unit. This is a nice facility. The move in process went pretty smoothly. There are times I feel that they are short staffed and only meet the bare minimum requirements for that. However, I will say the staff that they do have is wonderful and very nurturing and patient. For the most part, I would recommend The Addison of Pleasant Prairie to others.

Obviously my scores represent more care needs and cost concerns. Sometimes , mom isn't tended to in a timely manner. It's very concerning, especially if she really needed help. I. E. Like if she fell. When it comes to care, communication, follow up and meals - that could be improved. The food is nominal and our loved one will not eat it.

The Addison of Pleasant Prairie has been a wonderful experience so far for our family, and we would absolutely recommend them to other families. We really appreciate that when it comes to the activities they are adult activities, they do not treat them like children. They have exercise classes after breakfast, our love done enjoys painting throughout the week with their acrylic paint. There are music classes and she sings in the choirs on Sunday's. We are totally thrilled with The Addison.

They had a lot of new staff and they have not been trained yet, so the transition was difficult. I know they were trying their best. My mom also came in as a oxygen dependent and the staff did not know how to use that which was concerning. I understand it is hard to staff these facilities. I talked with them about my concerns and they got on top of it right away. Overall it was very good, but there were just some issues with the transition and the communication. I would recommend The Addison of Pleasant Prairie.

This is a great place. I feel that our loved one is in super safe hands. The staff are very helpful and they try their best. There has been times in the beginning that we didn't have all the answers to our questions. There also has been times it has been hard to get in touch with the right person. My loved one doesn't feel the food is up to par; however, the residents are working with the staff to improve this. The monthly fee includes utilities, cable, room, laundry, and housekeeping. The apartment has a full size fridge and a microwave for the residents to use.

It is a nice place. It is working out fine for my loved one. The best thing about the community is that things seem to be running well. We wish there were more outdoor setting areas for residents and families that visit.

There are many comments about being understaffed. I agree, there is not a lot of the appropriate level of staffing to support the amount of people who live here. They rely on CNAs who are fresh out of school and really don't have enough training. I believe the staff is caring, but not capable. The value for the money is not there. They raise rates annually, however changes that occur are often to make things cheaper for the company. The biggest example is the food. The food options are unhealthy, do not fit the dietary needs of 90% of the residents, and quite frankly not much better than a cafeteria in a hospital. I have observed lack of fresh fruit, water and sodium soaked ham, heavy gravy, heavy cheese, etc... None of the food is good for a diabetic or cardiac patient. The facility is unwilling to listen to the residents or family members. They have a "resident advisory committee" however the sole purpose is for the various directors to tell people what is going on. They do not allow feedback and if it is given they say thank you and that is the last it is ever heard. I am also disappointed in the transportation services at the facility. When we toured we were told that it was available daily, but the reality is that it is only available Tuesday and Thursday first come first serve. My mother has had to pay for rides to doctor's appointments because the bus is too full for hair appointments off site. I am also concerned about my mother's safety on the bus. There have been numerous accidents where patients in wheelchairs were not properly secured and had been tossed to the floor. This has happened on a couple of occasions. The driver has not been disciplined to my knowledge. I have also found that my mother is out of medication and has to wait the weekend because they are not getting delivery until Monday. I find this unacceptable, and we are filing a complaint with the state. With the new increase, I continue to question what I am paying for. My mother complains regularly about the food, that some of the nurses are mean (though the turn over in nurses is about 200% - so typically resolves quickly), and the lack of things to do. Overall the staff is friendly, and my mother is aware enough to make appropriate choices and prevent things. I just questions her staying here based on the issues I have seen.

There is no where near adequate staffing, and the staff that is provided is not trained properly (especially to handle the memory care residents). I witnessed staff in the memory care yelling at residents, and residents were constantly falling and walking around in soiled clothing and briefs. My grandmother will not even eat the food here because it is so unappetizing; it is either under or over cooked. I have witnessed my grandfather being given another reisdent’s medications. Sometimes call lights go completely unanswered. The aids and cleaning staff are constantly complaining to my grandmother about the poor staffing and how they are forced to come into work while sick. The building is new and pretty, yes, but do not let this obscure your judgement, as the care is simply very poor. Additionally, nearly every month we have had to argue the billing because we were overcharged.

Please do not place your loved ones at this facility if you expect for them to have competent care. The care provided is terrible, if you can even find a caretaker to help anyone. They are severely understaffed. The workers share this staffing information along with their other disgruntled complaints freely with anyone who visits the facility.

I stopped over to look at Addison, and I thought they were great. The community looked great, and I was able to take a short tour and see some of the rooms. I was worried that they didn’t take state aid however, so I didn’t want to risk moving my loved one here.

The Addison is outstanding! It was bright, cheery, and somebody is always at the front desk, even on the weekends. Walking through the hallways, you felt a great sense of community. Everyone knows everyone, even the staff walks down the hallways saying hello to the residents by name. The dining room is amazing and there are menus for everyone that you can choose your meal from so it doesn't just feel like a cafeteria. Not to mention the fact that the food is amazing. I definitely would have chosen this place, unfortunately it didn't become available until I had already found a place for my mom. I would absolutely recommend this community!

This is a nice community bright, open and the staff did a very great job showing me around the assisted living and the memory care plus answering all my questions. They suggested that my mom would do well in the memory care but I felt that this was just not the best fit for her. I would suggest taking their tour here as it is a nice place.

This is a very nice community, clean bright and open and the staff did a great job with our tour, they took their time showing us around and they answered all our questions. The rooms were nice and we would suggest setting up a tour here to see things for yourself and your loved one.

We toured The Addison of Pleasant Prairie. The staff was friendly and they gave a nice presentation of their community. It was a clean and well-kept community. We wanted a little bigger room for our loved one.

This is a beautiful community for care for a loved one. We would of like the menu and the activities. The community has a nice environment for some needing good care.and a loving touch. When I toured The Addison, the tour guide took over an hour showing me the place and answering questions. So far, my loved one is adjusting well now that she has moved in. The place is clean, well lit, and it is working out well.
