I stayed 24 hrs. and the room was filthy. My roommate laid in her feces & urine all day calling for help. The “nurse “ administered morning medication at 1:30 pm….and my 4-6 hr meds X2 in 24 hrs. “ Breakfast was ice cold and my coffee cup was half full with food floating in it. There was a TOXIC BIN FOR DIRTY GOWNS overflowing with garbage on the floor…2 staff sitting with their butts on the nursing station. Disgusting!!!

we had two loved ones at the facility. the communication is awful and the responses are inconsistent at best. I would not recommend this facility to my worst enemy. Look for a better option for your loved one!

Sorry but would not send my dog here. I found the place dirty when I came and half clean now. Still waiting for a drink from dinner. Staff is lazy or just not trained. You have to ask and ask for something. My bed was not make up today nor was my roommates. I said something and was told “well this is a new shift now”. I am lucky to be able to talk, see, and get around.

For the patient I have been visiting, the care has been of the lowest quality. The main complaint is the changing of the bandages or actually the lack of changing. They are supposed to be changed every 8 hours or as needed. At times when I have been there it has been 12 -14 hours. In this time the bandages have become completely saturated with fluid, fallen down and sometimes almost fallen off. The socks become wet and then the fluid runs unto the floor. A trail of fluid is left on the floor at all places the patient goes. The wet feet for such a prolonged time cause the patient to be cold all the time. All of the above complaints have been witnessed by me. It is reported to the nurses that the bandages need to be changed. They say it will be taken care of, but it never is. Keep your loved ones away from this place. I will not disclose my name as I think there may be adverse repercussions.

A Place for Mom has scored Salisbury Rehabilitation and Nursing Center 1 out of 10 using our proprietary review score.
We assign review scores to give a more reliable view into senior living communities and home care agencies. Our review scores prioritize reviews that are recent — the past 24 months — because we know families need current information when choosing senior care.
Those with many recent, positive reviews receive a high review score, while providers with few recent reviews — regardless of how positive — receive a lower review score. Communities with no recent reviews will not have a review score, even if older reviews are positive. The maximum A Place for Mom review score a community can receive is 10 points.
