My dad's dementia is such that he was placed on the memory care floor. A locked ward, so it removed any worry of him getting lost and creating a Silver alert if he wandered off. He needs reminders to do certain things, such as putting his hearing aids in the charger at night. Brushing his teeth, and taking his partial out. There have been several days where he was missing a hearing aid when we came to visit, and usually found in our under his bed. Which meant he slept with them in, and not in the charger. He also was wearing the same clothes for several days often. We've done his laundry at least 4 times when they haven't done it, even though it's suppose to be done every Tuesday. He has issues with his shower, some days nothing but cold water, other days nothing but hot. The staff is very nice, friendly. He gets fed well and the food is good. There are not enough activities for him. Luckily, we're close by and me and my daughters take him out weekly, often spending the weekend.

The food is better than the last place. We picked this community, because the staff was amazing, they really seemed to care about loved one, and the price was better than other places. The caregivers do a good job with passing the meds, but it would be nice if they had a full-time nurse, to better manage the medication. I would recommend to other families.

Oak Park Place was a beautiful facility and I liked it a lot. The only downfall was I really liked the concept of a facility that focused primarily on memory care rather than offering different care services all in one. I felt like we would be paying for things that she ultimately would not be utilizing.
The care staff is trained and if they are seeing someone that is just not picking up on it quickly enough - especially with new staff coming and going all the time they address it quickly and work to train them efficiently. They have good communication and that is something we have noticed. We do think the value for the money is good and we would highly recommend them to others.

6 months ago, I was asked to take a Lymphedema case at this facility. I was beyond impressed the moment I walked in. The atmosphere was so inviting & the staff was extremely welcoming. I knew from that first day, I wanted to establish a relationship with this company. Every one was approachable & I never felt like I was inconveniencing anyone. Management works so hard to make sure the care needs of all of their residents are met. [name removed], Director of Healthcare Services & [name removed], Director of Housing are a dynamic duo!! I can’t say enough about the activities department…one of the best around! [name removed] the beautiful lady at the front desk is one of my favs! [name removed[ Therapy is now one of the preferred therapy providers at this location & we are blessed to have a strong relationship with this fabulous company! If you’re looking for a top notch Assisted Living Facility in this area, look no more! Make that call & schedule a tour with [name removed] DOS! You’ll be glad you did.

I think there is a range of caregivers from the very good to the mediocre. And that's to be expected anywhere I think. The administrative staff have been very willing to listen to us, and that's important to us. And all in all, we feel that our family member is safe there, and that's what really matters. She's not crazy about the food, but I know they're working on it. They have a new chef, and they're reworking the menu to be more senior friendly. And they seem to offer a lot of activities.

My loved one loves it here. The community hyped up a bunch of things they would do, and their promises aren't being upheld. There is no housekeeping or laundry that is being done properly or returned properly as well.

Our family was able to have an in person tour and the experience was good, we got a good look at everything inside. The community seemed very clean, well kept and organized. Only reason we did not choose Oak Park was the value, it was pretty significant.
Facilities were newer and very clean. [Name removed] was very personable and willing to help. No studio apartments currently available. The staff we observed seemed somewhat disengaged. Care level fees were more reasonable than other facilities we toured. We decided that Oak Park Place was the best fit for our loved one. We look forward to moving her there by the end of the month!

My mom has been here 6 months and we are looking for a new facility. Not meeting her needs is an understatement. Never enough staff here to even meet basic needs. Keep looking beyond this place for your loved one. We try working with staff and management for 6 months now and never get any problems resolved. We are prepared to move my mom at the very first opportunity we can get.

I would recommend you take a look at this community. Oak Park Place went above and beyond for my loved one. I was very pleased with the care my loved one received. My loved one was able to make friends with the staff. It was a great experience for my loved one. Most of the residents were older than my loved one., as well This is a very clean community. Oak Park Place worked with my loved one and their finances.

Highly concerned about administrative level. There are no systems in place. HUGE medication management issues that caused us the greatest concern. I understand the head of the organization [name removed] has finally come in the beginning of the month to, maybe, find out what is going on at this facility. I would like to know what steps are taking place, because although we have left, I care deeply for those remaining families who have no one to advocate. Thank god they hired [name removed], RN before we left, excellent care and respect for the residents, we did not find that from the other nurse, still there. Total disrespect and disregard. PCA's we found to be caring and open to suggestions, and no problem there! Additional concerns regarding process in place for shower assist, cleaning (in the room and facility!), meals and supervision at meal time, and building/resident safety. And yes, we were told to 'speak up'. We did and unanimously we were always told, "well that shouldn't be", but no change! This is why we just did it ourselves, because we didn't want to make to big noise so our family member wasn't retaliated against. This increased her frustration and anxiety as well. Knowing she is paying an astronomical amount of money, and her family still has to be doing all the work. Not a good place to be in. We'd like to let this go, but this brings us so much sadness. I want this to be the place the [name removed] family must have envisioned according to their website. Maybe was like that in the beginning, but something drastically changed.

Caregivers are quite friendly and caring. The building / decor is comfortable. Facility is understaffed at times. Administration and Corporate administration was not responsive to the concerns that were brought to them by our family.

My Mother is 87 years old and has been living here at OAK PARK PLACE for 5+ years. I want to mention that my mother has only gotten healthier and more engaged in a social life since her arrival. I use to worry when I would try to call her room and all I talked to was her answering machine. I would find out that she was busy getting her nails done, at exercise class, at mass, playing blackjack or at the movies. I also appreciate that they have quarterly family meal nights. So nice to meet so many family members of the residents. The caring staff at Oak Park Place has truly energized my mother. [Name removed], the director has been a great asset to her staff and our parents needs...after all we are now the adults looking over our parents live and needs. Could not due this without the support of Oak Park Place and my weekly visits.

My biggest regret in life was choosing Oak Park. What a disgrace of a “memory care facility.” Not one person in the entire building is qualified to care for the special needs of an Alzheimer’s/Dementia patient. As we discovered they have plenty of loopholes to cover all their mistakes/false advertising. During the initial walkthrough they make the place seem like a dream come true, however it is a house of horrors. My family member passed away due to heedlessness and we are pursuing a case against them. Between the uneducated, uncompassionate, limited and constantly turning staff/management, the director of housing and regional nurse were among the most stressful part of our time with Oak Park. As leaders they failed exceedingly, they provided no training for the steadily turning staff and complete disregard for making any changes to better the residents. We brought several issues to their attention, as well as corporate, that never got solved. Their feeble responses lacked any compassion and came across as sarcastic and merciless. It felt as if we were annoying them with our concerns. A few of those matters that our family member suffered from include: dehydration, improper wound care, neglected hygiene, toileting was unsafe, unsanitary and not often enough resulting in open wounds/hospitalization, no housekeeping as promised and paid for, sudden weight loss, minimal interaction with activities (they just wheel them in a room with a TV and leave them there unattended), residents put in unsafe conditions because of lazy staff, bed falls are not being reported, hospitalization for pneumonia due to failure to follow treatment plans, and just downright lack of communication, knowledge and complete negligence. This is how our family member lived their last months. The response we got in return “It's the disease, Alzheimer’s, they are old it happens.” I will not rest until this place is no longer in operation and the residents still residing there get the love, care and attention they deserve.

We toured here, and thought that the community itself was nice. However, the neighborhood nearby is not good. The residents windows overlook the town dump and the police station, which just didn't seem very comfortable to us.

I was given a very nice tour here, the staff were very friendly and took their time showing me around and they answered all my questions. The community was very nice, spacious and clean and offered nice amenities for the residents. At the time of my tour, they did not have any availability.
