The facilities were clean. Floors were swept and moped regularly, along with vacuuming. The cafeteria area was free of crumbs or food sitting out. The individual rooms were cleaned weekly or more, if needed. Staff was extremely helpful, very nice, and answered any and all questions we had from the first visit to the day we moved her belongings out. The nurses were great with her meds and at the beginning of the end, kept us updated on changes in behavior, eating habits and so on. They also helped with getting hospice help.

Not much good I can say, some staff are great. Others don't deserve to be around people in general. Management...well not much to say about people that disregard good employees and stand by worthless employees. God forbid having a loved one live there, then raising the rent and blaming it on having to pay staff more.

I am a current resident. August 2023. I feel very fortunate being here. The staff are excellent and attentive, the food is very good and consistently hot which I have found in other places it's difficult to find. There are plenty of daily activities. I appreciate that there are church services here from several churches in the community. The residents are quite nice to socialize with.

Actually everyone has been friendly and helpful. They learn the names of the residents. [name removed] was great to work with to get things rolling to get my mom an apartment and inform me of what they have to offer. The maintenance man (I just can't think of his name) was on the ball when mom had troubles with her air conditioner and toilet seat. They always have activities going on. Mom likes the library. As far as the food goes, I can only go by what mom has told me. That bring said - hamburgers very dry, cold cut sandwiches (one slice lunch meat, one slice cheese, little piece of lettuce)lacking in items and bread dries out before served, chicken breasts are usually dry as well.

The new director is good about open door policy. The staff is a waste of time to remind me when it is meal time. I regularly go with no reminder.

Disorganized upper management. Poor quality of food and unhealthy. Short staffed. Many empty rooms now, since rates raised 2 times in 1 year. Big turnover with staff. Here today is gone tomorrow. Front doors are left unlocked at night. Too many problems to list.

I was very apprehensive about moving my loved ones, Mom and sister, into any assisted living facility. I didn't know much about the process and that's why I contacted A Place for Mom. With their help, I was steered toward Liberty Village in Tomah. I can't tell you how grateful I am that I moved them there. I looked at other places, but decided on Liberty Village. There are individual apartments for the residents that are efficient, well maintained, bright and cheery. My Mother and sister enjoy a 2 bedroom apartment where they were able to move in all their own furniture and belongings. I have peace of mind knowing that no matter what may occur, someone is also there to help them out and take care of them. The staff is absolutely wonderful, caring and loving. The food that is prepared for the residents is healthy and really good. If I could give more than a 5 star rating, I would. And best of all, Mom and Sis love the place.

Liberty is a nice community for people who are more independent, as the personal living area for residents is more spacious than some other places. The staff is helpful and accommodating, and the community seems well kept and tidy. I haven’t had a meal there yet, but my mother says the food is average tasting. The portion sizes are good enough for her, and the meals are nutritious. They do have lots of activities like Bingo, which my mother loves, crafts, music, and others. We liked Liberty the best out of the other places we toured, and we’d recommend the community to others.

The staff was awesome, my Mom loved the attention. She enjoyed the exercise program and entertainment. . She was very well cared for. Her rooms were spacious and nicely decorated. Can't say enough about the wonderful staff. On the down side she did not like the food. We also noticed an unpleasant "nursing home odor"
