The facility is attractive and clean. The staff is caring and capable. Their target demographic is independent seniors. While they do offer special care for those that may need it, they are somewhat limited with what they can do if the client is not mentally or emotionally sound. My mother's deteriorating condition quickly overwhelmed the staff. However, they really did well for longer than was probably best for them or my mother. I would say that my loved one is well taken care of but the cost is very high. The staff are kind and attentive and there are lots of activities for residents to take part in.

I could never recommend leaving a loved one at Bell Trace skilled nursing facility. Three weeks after being admitted after surgery, my father was constantly complaining of upset stomach, diarrhea, nausea, inability to eat, etc. I am in another city, and on the phone he was telling me he was going to die. When I spoke to his nurse she would say he's "fine, resting comfortably." After a few days of this I insisted they send him to the emergency room where he was diagnosed with C. DIFF. When I called the Bell Trace nurse to tell her, she DENIED that he had c. diff and said "it's only colitis." It is now 4 weeks later and my father was unable to overcome this setback; he is going into hospice. He contracted this infection at Bell Trace and likely had it for at least a week before going to the ER. Another nurse was extremely rude to me and there were other issues - food is terrible. There is something really off at this place, and with all my heart, I recommend you stay away.

My mother-in-law was here for 2 weeks. During that time her bed was only changed 1 time and that only happened because I went to the nurses desk and asked for it to be done. They only offered to help her get a shower twice. Once at 8 o'clock at night (she declined at that hour) and then the rehab nurse asked her if she had been given a shower (after she had been there for 10 days without a shower) and offered to help her. They are not very responsive to patients requests. Sometimes making them wait for a half hour and longer to get a drink and even to have help to the restroom. The lady beside my mil was completely helpless and she put her light on for help to the restroom. After 20 minutes I tried to get a nurse and was told they knew her light was on but they were busy going over patients. After another 15 minutes the lady was trying to move to the restroom on her own. I again went for a nurse because I was afraid she might injure herself. There were 3 nurses behind the desk working on their computers. One of them did come help the lady to the restroom. I then left. My mil called me 20 minutes later telling me the nurses had left the lady in there and she was calling for help. She wanted my mil to call 911. My mil is on oxygen and had to have a walker so she is not able to help anyone. My mil tried to get the nurses attention to help her. They finally came back in after 20 minutes. The next day they left the same lady in a soiled bed (because again they didn't come to take her to the restroom when she needed to go) for an hour. The food they served was sub par. They don't always come ask what you would prefer for their meals and the patient is just stuck with whatever they get. There was a mess up with prescriptions getting sent to the incorrect pharmacy when my mil was discharged. In all, it was not a good experience.

The only thing that worked was their physical therapy--scheduled baths not forthcoming, up to two hour wait for cns to answer help buttons, yeast infection not treated after many requests, food often inedible. When we went to check out, found a bag of someone else's dirty laundry in the bottom of the closet. Apparently very understaffed.

Remember--you must be the advocate for your loved one. We placed my recently widowed sister-in-law in their care. We presumed the nursing staff would do a full-body assessment, but we were wrong. The open wound on her elbow was not dressed for the first two weeks she was in their care. (Even though it was bone deep and oozing.) The nursing staff wasn't even aware she had the sore! I had to be the one who addressed her need with the nurse. But when it came to a psych evaluation, that was done immediately, followed by subsequent "treatment" visits, for which she would later be billed. She was not told she would be charged for what she believed were "friendly visits".

My mom passed away there after only being there a few hours. I never did meet the doctor in charge, if there was one. The nurse on duty said she did not know my mom was so ill or much about her. How could she not know? The place should be shut down.

Terrible experience there. Not enough staff to care for your parent. A woman across the hall from my parent yelled for help for 15 min before someone came. Best tp stay away from here.

Understaffed as well as uniformed staff, whose fault that is I cannot say. My loved one did not survive there for very long. My questions were not sufficiently answered surrounding the passing. Definitely DO NOT recommend this place.

my mother went after surgery for rehab, by the time she went from there to the ER the docs didn't know if she would live overnight. She got a UTI bed sores cellulitis and pneumonia while staying at Belle Trace for only a couple weeks. She had a previous stay there with no problems. After this I would not send anyone to stay there for anything.

My mother got the very best care at Bell Trace....the rehabilitation was outstanding.
