They keep the residents clean and well presentable. I’m unsure of the new director but the girl who was running it for a couple months always went above and beyond. I can’t seem to remember her name but she worked as a med tech and worked her way up to sitting in the director position. She was the highlight of Azalea Gardens her and the current nurse made an amazing leader group they worked well together always. Everytime I was there I witnessed her interacting with the residents and staff and making sure they are all in a GREAT mood. I seen her working every position in the facility when it was needed, I wish she would come back and help brighten the atmosphere again. We are no longer there as of earlier this year, but the biggest thing that bothers me about ANY facility like Azalea Gardens is the smell. I understand it is most difficult to stay on top of but it was really overpowering at times.

I like the facility and it's kept clean, but the housekeeping staff didn't keep the bathroom clean and didn't sweep under the beds. The staff were very friendly, but it seemed they were understaffed. When visiting my mom once, a man fell in the dining area and we couldn't find anyone to help. My brother wound up picking him up. I found someone in the office who called for help. All 3 women working that day were outside smoking. I was upset about that. If they want to smoke, fine but not all at the same time. Someone has to be with the patients. Also, my sister had made payment arrangements to be automatically paid each month, but they billed her for 3 months, $12k, which overdrew her account and it took months to figure it out. In the meantime, they continued to bill her monthly. Additionally, the extra fees for toilet paper, shampoo, activities etc wasn't clearly explained. We thought basics, like toilet paper, were part of the monthly cost. I would recommend it only because it is so much less costly than other places, and some people, like us, simply don't have the resources for another facility.

Activity director is worst they have had in years. She never engages with any of residents unless they are a few of the ladies. That she has them participate in every activity. The others are left out. She is loud, rude, and very unprofessional. I go on weekends so I don't have to leave with a headache. If she works on weekends I make my visit with family quick. I won't go into other details about activity director. The other staffing are great. Food needs a do over and better cooks.

Azaleas is where I would put my love one when that time comes. Which I know can be sooner or later. I have engaged with the staffing and they do show they care. I have seen them in action many many many many times and never was disappointed. I am so glad Azaleas is in a centralized area and easy to travel to and from. Each discipline deserves a chance to care for your love one. They do bend over backwards for their residents. They also have a fantastic hospice company(Elara) that goes and cares for the residents that's on service. I can sit and type all day about the awesome Azaleas Memory Care. But acourse there isn't enough space here to do so.

It is a very good place for our loved one. Lots of activities and friendly staff. I feel like the week day staff does a good job. Good communication with them. I have not been a big fan of the weekend staff. Far less knowledgeable and less friendly. It is a place I would recommend if their loved one needed memory care. It was not our first choice but has been an overall good experience.

Azalea Gardens seemed to be a top notch facility. The staff were all very friendly and caring. Since it is strictly memory care, the residents we saw seemed to need more care than mom does. Mom is more alert, so we went with an assisted living facility instead so she could socialize.

I like this community. for the care for my mom. We are happy with the staff and the care.The community does need some small improvement in the cleaning. They meals are average.but does need some improvements. We do like the value and the care and have informed others of this nice community for the care that is received.

Due to health issues that were a result of my dad took a fall the amount of care dad needed was out of their range. The staff is extremely caring and shows concern. Before dad has taken his fall he was participating in the activities and social hours and making friends. He was loving the food. Over all the place is nice clean and well maintained. They do check on the residents that are how they knew my dad had taken a fall is when they came to call him for dinner and found him.

I went to Brook dale to see the Manager in regards to a personal matter. I was greeted and taken to her office. I spoke with [removed] , she was very kind and helpful. The overall atmosphere was very warm and inviting. The patients were all in the dining room waiting for dinner, they were all dressed and looked very happy. I felt very at home and at peace with the facility. I would highly recommend this facility to my family and friends.

The community is nice, but they need to work on the activities. The food needs improvements. They need to give a menu for the special needs. They need to have security due to money was missing from my mom purse in her room. This is a big adjustment for my parents. They need to have better lighting in the parking lot area. The staff is great, but there are a few issues that need to be resolved.

We are getting settled in and things are going fine now. The staff here is great and they have tried to make the adjustment as easy as possible. The facility is clean and seems well maintained. The other residents are very nice too.

The only thing they could improve is to have someone available to let you in and out of the door when visiting. This seems to be an ongoing issue. The food seems to be really good.

It's smaller than the one we were at in Broken Arrow. The rooms are still nice and spacious. They are working very hard to make her comfortable and happy. One thing I really like is that even though she is in memory care, she is high functioning and since the assisted living and memory care is combined, higher functioning residents are still able to join in for the field trips that they have here. So far we have been pretty happy with it, but it hasn't been very long yet.
