Heritage Homes is my choice for the place I call "home". It offers the amenities I value, such as central air conditioning, a parking garage that is easy to navigate in, craft room usage for my prized stash of craft materials, spacious living in square footage the size of a house, quiet home space, and privacy, privacy, privacy. Opportunities for socialization are available for me to choose or decline. Communication from administration is excellent. Respect and friendliness are abundant. Heritage Homes is regularly part of the community newspaper due to its focus on senior advocacy and connection to local living. In subjective summary, I would [removed] I left my house to live at Heritage Homes, but I did not leave my home, sweet, home.

The staff that is there are wonderful, they have done a great job given the circumstances but there is a high turnover rate outside of the day to day staff. The director is trying her best, but communication with the staff and families has started to lack. The life enrichment coordinator is fantastic and wonderful, she wears many hats there. We want them to improve, once stability is provided everything will fall into place.

Assisted Living

Assisted living communities provide coordinated social events and apartment style housing as well as personalized help. Services provided often involve aid with ADLs. Residents may be kept under monitoring and evaluation. In some cases, communities may provide nursing, hospice, or continuing care. The state of Wisconsin maintains specific rules associated with this kind of community. The Wisconsin Department of Health Services, Bureau of Assisted Living, Division of Quality Assurance, regulates community-based residential communities (CBRFs) governs this community type. Before a resident is accepted, they must go through a service plan screening to make sure their needs are going to be met. This screening includes a number of areas of interest such as reassessments must be conducted at least annually, and when residents have a change in needs, abilities, or condition, initial comprehensive mental and physical assessment, the plan must specify which program services will be provided to meet the resident’s needs as identified by the assessment and the frequency with which each service will be provided, and community must develop a temporary service plan to meet immediate needs. Staff members obtain education in a number of fields. These areas include development of service plans, reporting abuse, standard precautions, and needs assessment of prospective residents. There must be one toilet for every 1 residents and one bath or shower for every 1 residents. This community type has a number of provisions pertaining to medication assistance. These provisions include when medication administration is not supervised by an RN, practitioner, or pharmacist, the CBRF must arrange for a pharmacist to package and label a resident’s prescription medications in unit dose, If the medication administration is supervised by a licensed healthcare professionall, the community must ensure that these individuals coordinate the medication administration process and system, and Residents may self-administer medications if they are capable. This type of community is required to offer a number of services. These include behavior management, independent living skills, activity programming for persons with dementia, and supervision. This community has a number of provisions concerning food and diet which include therapeutic diets must be provided if ordered by a physician, these meals must meet the recommended dietary allowances based on current dietary guidelines for Americans, these meals must meet residents special dietary needs, and communities must provide at least three nutritious meals a day and a nutritious snack. Public assistance is available through The Elderly and Physically Disabled 1915(c) Waiver program, the Community Options 1915(c) Waiver program, and the Medicaid managed care 1915(b)(c) Family Care Waiver program for the aged and physically disabled.
