The facility and staff at Tender Reflections are wonderful! The facility is located in a beautiful setting with big windows that provide a nice view of central Wisconsin nature. The common areas are open, with clear walking paths, interactive screens and nice seating available. There are rooms available for larger family visits and several options for smaller visits too. The facility is secure for the residents and staff, yet accessible for guests. The staff is very caring, compassionate and responsive. They take really good care of the loved ones entrusted to their care, treating them with the utmost dignity and respect. They meet the residents where they're at and help them through their personal struggles as their medical health progresses. Individual care plans are in place and closely monitored. The family is cared for as well. The staff answers questions and supports them through the journey and loved one's illness. We are blessed to have our loved one in their care. I highly recommend Tender Reflections. It's a really nice memory care facility and the staff are wonderful!

Unfortunately my father in law was only in this community for 11 days. He wasn't happy, so he moved back home. However I did feel this was a nice community. Its a smaller community compared to others, but it is a great facility overall. I would recommend it.

Assisted Living

Assisted living communities administer personal care, social events, and supervision to individuals around 65 years old. The types of service provided often involve assistance with ADLs. Inhabitants may be kept under supervision and assessment. Under certain circumstances, communities may provide “aging in place”, respite, or nursing care. Wisconsin has a number of provisions related to this type of community. The Wisconsin Department of Health Services, Bureau of Assisted Living, Division of Quality Assurance, regulates community-based residential communities (CBRFs) certifies this type of community. Before a resident is accepted, they must go through a service plan screening to make sure their needs are going to be met. This screening includes a number of areas of interest such as initial comprehensive mental and physical assessment, reassessments must be conducted at least annually, and when residents have a change in needs, abilities, or condition, the plan must specify which program services will be provided to meet the resident’s needs as identified by the assessment and the frequency with which each service will be provided, and community must develop a temporary service plan to meet immediate needs. Public assistance is available through The Elderly and Physically Disabled 1915(c) Waiver program, the Community Options 1915(c) Waiver program, and the Medicaid managed care 1915(b)(c) Family Care Waiver program for the aged and physically disabled. This type of community is required to offer a number of services. These include behavior management, including personal care, transportation, and leisure time activities. This community has a number of provisions concerning food and diet which include these meals must meet residents special dietary needs, therapeutic diets must be provided if ordered by a physician, communities must provide at least three nutritious meals a day and a nutritious snack, and these meals must meet the recommended dietary allowances based on current dietary guidelines for Americans. Staff are required to obtain education in a number of areas. These include: needs assessment of prospective residents, reporting abuse, job responsibilities, and resident rights. This community type has a number of provisions pertaining to medication assistance. These provisions include when medication administration is not supervised by an RN, practitioner, or pharmacist, the CBRF must arrange for a pharmacist to package and label a resident’s prescription medications in unit dose, If the medication administration is supervised by a licensed healthcare professionall, the community must ensure that these individuals coordinate the medication administration process and system, and Residents may self-administer medications if they are capable. There must be one bath or shower for every 1 residents and one toilet for every 1 residents.
