I am so disgusted with Warren Center. My grandma was placed there after being extremely ill and this is supposed to be a rehab to get her better( this was the only place that had an available bed.) These are the meals they are feeding everyone. We brought her about 6 outfits and some how they have all gone missing, when we visited she was wearing someone else’s clothes because they couldn’t find hers. One night my grandmother got really bad and they had her taken to the ER and never told us (we found out later on from my grandma) they don’t have phones in any of the rooms so we never can get a hold of her. They have had 71 citations already. I have never seen my grandmother so depressed. $600 a day for these people to treat my grandmother like a prisoner?? We are getting her out of there asap but I want to make everyone aware of how disgusting this place is. Whoever runs this place has a lot of explaining to do. They blocked me on Facebook after sharing this!

Our experience at Warren Center was brief but they were all very, very nice. We got registered and they brought us down to a nice room that he was staying in. There was a beautiful welcoming card on his dresser and even a goody bag waiting for him. They were all so nice. The place itself is lovely my loved one is just not used to being away from home so this was tough for him. There really are no complaints about our experience. I would definitely recommend Warren Center to a friend.

The overall experience at Warren Center is going really well. As far as I know, my loved one is happy and well taken care of, they are treating him well there. He has started the therapy and that has been great. I would recommend them to a friend.

The facility is very attractive, nothing wrong with the facility. It just seemed like they were understaffed. The therapy people were very nice when we met them and the staff is also very nice. I think the overall experience has improved from when we first arrived.

We signed my loved one out from Warren Center. To start, I was not happy that they changed all of his medications when he arrived. They were not taking very good care of him both in terms of the nursing care as well as his therapy.

We are very satisfied with the services provided. They are attentive to my moms needs. Everybody stops in her room and I asked if she had lunch yet. They brought her lunch and they were going to give me a meal too. They are very friendly. The floor is mopped and clean. I am waiting for a call back from someone about my moms care after she leaves.

They have been doing good things for him. My loved one is getting the best care he could get. They are all very friendly. It is very clean. They will answer any questions and are very good.

Everyone has been extremely helpful so far. I have not felt uncomfortable with this whole transition one single time. Everyone has been very patient and compassionate. I haven't noticed anything I would complain about, and I would absolutely complain. No issue with communication either!

The day I was there my loved one was not happy because her meals were not according to her diet. They have fixed that. The staff are good and comforted her when she was crying. She likes her roommate. She is a very difficult patient. From what I can see it is good but she is complaining. She always complains. From what I could tell it looked clean. She has not had a good dining experience.

My loved one seems to be improving and is getting stronger. The staff are friendly and approachable. They gave me a current medication list. The nurses and aides are all very friendly. My loved one shared that he has a bed sore and I told the nurse about it. The unit nurse is forthcoming with information and asked if we need anything else. He is pleased with the meals. He changed his mind about a meal and they brought him something else to eat. All of the staff are good at communicating and updating me.

The staff seem very nice and very helpful. The therapy is good. My friend has made improvements and progress. The nurses are very nice and helpful. The aides are very nice and helpful. I didn't see any problems at all with the cleanliness of the facility. The communication is great. I got a phone call very early one morning after my loved one fell and they wanted to make sure I knew right away.

I thought it looked pretty nice in there. My son is doing okay with the okay with it, he's making progress and improvements. The staff all seems to be nice. I thought it looked pretty nice and clean and tidy in there. My son seems to have all the information he needs about his care.
